PURPOSE: Median sternotomy, which generally is used as a standard access for atrial septal defect (ASD) and mitral valve operations, has a significant risk of postoperative instability/osteomyelitis of the sternum. Moreover, especially in young women, the resulting large scar is a poor cosmetic result that may have adverse psychological consequences. A 13-year-old boy, who had undergone a Rastelli procedure 8 years before, developed severe extracardiac conduit stenosis and infectious endocarditis. Sternal re-entry appeared to be dangerous and to require extensive adhesiolysis. Through a left anterior small thoracotomy, however, good exposure was obtained and the operation was performed safely and much less invasively. The main pulmonary artery was reconstructed with autologous tissues only, i.e. fibrous tissue bed and broadly based pericardial pedicle. There was no residual stenosis. The patient convalesced rapidly.

BACKGROUND: Surgical closure of the atrial septal defect (ASD) is a low-risk operation with little post-operative morbidity involved. The standard approach to ASD closure is via a median sternotomy. However the post-operative midline scar is cosmetically undesirable.
PATIENTS: We report our experience with four female patients who have undergone closure of ASD through a right anterior thoracotomy and a left groin incision for femoral cannulation.
RESULTS: Post-operative recovery was uneventful. The cosmetic results of their operations were good.
