Prognostic impact of resection margin involvement in surgically managed HPV-positive tonsil cancer.

OBJECTIVES: To find out the role of resection margin involvement in surgically managed HPV-positive tonsil cancer. MATERIALS AND METHODS: The study included 94 subjects with HPV-positive tonsil cancer undergoing surgical treatment. We evaluated the relationships between the resection margin status, clinicopathological factors, and oncological outcome. RESULTS: The rate of resection margin involvement was 22.3% (21/94) after ablative surgery. Margin involvement, lymphatic invasion, and extracapsular spread were associated with the 5-year disease-free survival (DFS) and disease-specific surviv

Lymphoma presenting as a peritonsillar abscess.

INTRODUCTION: Peritonsillar abscess is generally seen as a complication of acute tonsillitis in young subjects. It may, however, in rare cases reveal a malignant tumor of the tonsil: most often squamous cell carcinoma or, more rarely, lymphoma. We report a rare case of tonsillar lymphoma revealed by a peritonsillar abscess. CASE REPORT: A 66-year-old woman, without history of recurrent tonsillitis, was admitted for right peritonsillar abscess with fever. She underwent incision-drainage of the abscess with 10 days' intravenous antibiotics. As tonsillar hypertrophy persisted, tonsillectomy was

[Hyperbaric oxygen therapy as a method of treatment of laryngeal and pharyngeal radionecrosis].

The authors described two cases of palatine tonsil carcinoma treated with surgery and subsequent radiation therapy after which occurred severe radionecrosis of the larynx, pharynx and oral cavity. These subjects were treated with hyperbaric oxygen therapy (HBO). Complete resolution of postradiation changes was observed. The authors described: 1) basic changes in the tissues caused by radiation therapy; 2) the mechanisms of healing activity of hyperbaric oxygen.

Extramedullary plasmacytoma with crystal inclusions arising from the palatal tonsil.

A case of extramedullary plasmacytoma of the left palatal tonsil in a 51-year-old woman is reported. The histological features of the tumor are typical, except for the appearance of crystal inclusions in plasma cells, histiocytes and matrices. The developmental process and the nature of the crystal inclusions are discussed, based on electron-microscopic findings. The patient has been followed for one year post-operatively, and no clinical evidence of recurrence has been observed.

Rehabilitation for complicated dysphagia after synchronous head-and-neck and esophageal cancer surgery: A case report.

RATIONALE: Surgical intervention for synchronous head-and-neck and esophageal cancers often results in complex dysphagia, significantly affecting postoperative quality of life. Swallowing dysfunction may become permanent or worsen, with potential impacts on noncancer-related mortality. PATIENT CONCERNS: We report a rare case of multiple synchronous squamous cell carcinomas of the head and neck (tonsillar and epiglottic cancer) along with esophageal cancer, presenting for dysphagia rehabilitation following surgery. DIAGNOSES: Comprehensive evaluations-including magnetic resonance imaging, lar

Carcinoma of the tonsillar region.

A retrospective analysis of 171 patients with carcinoma of the tonsillar region is presented. The majority of patients (79%) presented with stage III and IV disease. Three year determinate survival was 81% - T1, 74% - T2, 59% - T3, and 20% - T4. The presence of nodal disease decreased survival by one half. Radiotherapy alone was highly successful in controlling early T1 and T2 lesions of the tonsillar fossa. Surgery alone or combined radiotherapy and surgery was more successful than radiotherapy in controlling early T1 and T2 lesions of the tonsillar pillar. More advanced T3 and T4 lesions als

Cervical nodal metastases in squamous cell carcinoma of the head and neck: what to expect.

The treatment and management of malignancies of the head and neck is directly altered by the presence of metastatic cervical adenopathy. The treatment of nodal metastases in squamous cell carcinoma of the head and neck (HNSCCA) is determined by the lymphatic drainage of the upper aerodigestive tract. The lymphatic drainage is site-specific and occurs in a predictable manner. The purpose of this text is to provide an overview of the normal routes of lymphatic drainage in the head and neck and correlate this with the current nodal classification system. The specific aims of this manuscript are t

Improved local control for advanced oropharyngeal carcinoma following twice daily radiation therapy.

This paper presents the results of treatment on 99 patients with squamous cell carcinoma of the oropharynx irradiated by the twice-a-day radiation therapy program at the Massachusetts General Hospital. The program consisted of 1.6 Gy per fraction, two fractions per day with 4 hours between fractions, for 12 days, 5 days a week. After 38.4 Gy, the patients were given a 2 week break and then resumed twice-a-day radiation therapy for a total of 64 Gy and occasionally 67.2 Gy. Fifty-two patients had carcinoma of the faucial tonsil and 47 patients had carcinoma of the base of the tongue. For the en

Palatine tonsil metastasis from small cell lung cancer.

INTRODUCTION: Palatine tonsil metastasis is an uncommon manifestation of disseminated malignancy and is exceptionally rare in small-cell lung cancer (SCLC). Because it may mimic benign tonsillar disease or a primary oropharyngeal tumor, diagnosis can be delayed. CASE SUMMARY: A 71-year-old man was admitted after pertrochanteric hip fracture. Preoperative evaluation revealed a left tonsillar mass and a left hilar pulmonary lesion. Computed tomography showed a 3-4cm lesion involving the left palatine tonsil, a 7.5cm left hilar mass with mediastinal lymphadenopathy and multiple brain metastases.

Metastatic Tonsil Squamous Cell Carcinoma: An Important Consideration in the Differential Diagnosis of Malignant Basaloid Neoplasms in the Skin: Case Report and Review of the Literature.

Malignant basaloid neoplasms of the skin are frequent, and their accurate diagnosis holds paramount importance for treatment and prognosis. However, these neoplasms can present diagnostic challenges because of their extensive differential diagnosis, which encompasses cutaneous metastasis among many other possibilities. We present a case of a 74-year-old man with a history of p16-positive palatine tonsil squamous cell carcinoma (SCC) treated with surgery and adjuvant radiation with no prior evidence of recurrence who presented to the dermatologist with 2 chin papules. The initial histopathologi

Is the retropharyngeal lymph node the first echelon node for carcinoma tonsil? Prospective evaluation and literature review.

PURPOSE: Tonsil cancer being predominantly treated by non-surgical means, there is a paucity of data on lymph nodal drainage pathways and histo-pathologically confirmed metastatic rates. This study assesses the retropharyngeal lymph node (RPLN) in N0 squamous cell carcinoma tonsil as a possible first echelon node and a site for occult metastasis. METHODS: Prospective study involving treatment naïve N0 carcinoma tonsil treated by primary surgery and adjuvant treatment from June 2017 to March 2019. In-vivo lymph nodal drainage patterns were assessed by sentinel node mapping by preoperative SPEC

Analysis of the intensity of immune cell infiltration and immunoreactivity of RCAS1 in diffuse large B-cell lymphoma of the palatine tonsil and its microenvironment.

Non-Hodgkin lymphoma of Waldeyer's ring constitutes a small percentage of cases of palatine tonsil malignancies and its precise etiology remains unknown. RCAS1 (receptor cancer-binding antigen expressed on SiSo cells) has been demonstrated to be associated with poor prognosis, the development of lymph node metastases and participation in tumor microenvironment remodeling. Our aim is to analyze the potential role of RCAS1 expression in the tumor and tumor microenvironment in the development of early-stage palatine tonsil B-cell lymphomas. We selected 20 patients and analyzed tissue samples from

Palatine tonsillar metastasis of rectal adenocarcinoma: a case report and literature review.

Cases of primary colorectal adenocarcinoma metastasized to the palatine tonsil are extremely rare. To the best of our knowledge, only 10 cases have thus far been previously documented in the English literature. A 37-year-old Chinese woman presented with a right palatine tonsil swelling and odynophagia 5 months after a surgical resection of rectal adenocarcinoma was performed. The patient underwent a tonsillectomy, and a metastatic poorly differentiated adenocarcinoma from a colorectal origin was revealed by immunohistochemical analysis. The manner in which tonsillar metastases are involved rem

Clinical nodal staging of T1-2 tonsillar squamous cell carcinoma stratified by p16 status and implications for ipsilateral neck irradiation.

BACKGROUND AND PURPOSE: Previous studies have reported <5% incidence of contralateral nodal metastasis in patients with T1-2 tonsillar squamous cell carcinoma. We analyzed the nodal staging of T1-2 tonsillar squamous cell carcinoma stratified for p16 status, a marker of human papillomavirus positivity. MATERIALS AND METHODS: Clinical and radiographic nodal staging and p16 status of 41 T1-2 tonsillar squamous cell carcinoma patients who were treated between January 2002 and June 2009 were retrospectively analyzed. Patients with a history of prior head and neck cancer, synchronous cancers, base
