The transition from oncology to palliative care: barriers and facilitators explored through an integrative review.

BACKGROUND: Patients with advanced cancer often experience difficult symptoms near the end of life, yet the beneficial integration of oncology and palliative care is frequently lacking or poorly coordinated. Transitioning from curative treatments to palliative care focused on symptom relief and quality of life remains a common challenge. Palliative care is often underused or introduced late in the cancer treatment process, leading to suboptimal care outcomes. Understanding the factors influencing this transition is crucial for improving patients' overall care experience. This review aims to ex

Evaluation of a Pharmacist-Led Intravenous Iron Therapy Transitions of Care Service in Heart Failure.

BACKGROUND: The guidelines recommend intravenous (IV) iron for patients with heart failure (HF) with a reduced or mildly reduced ejection fraction. IV iron is often administered during hospitalization. However, care transitions from inpatient to outpatient settings for IV iron are suboptimal, and only about one-third of patients receive follow-up iron study measurements to assess the need for IV iron at outpatient settings after inpatient IV iron. Limited data is available to improve the current practice of IV iron during care transitions. OBJECTIVE: To evaluate the post-discharge performance

Important antimicrobial dosing considerations for transitions of care: Focus on thrice-weekly dosing in hemodialysis.

PURPOSE: The objective of this clinical review is to evaluate current literature and propose optimal dosing strategies for thrice-weekly postdialytic administration of select antimicrobial agents in individuals receiving chronic intermittent hemodialysis (iHD). SUMMARY: The optimization of outpatient parenteral antimicrobial therapy (OPAT) for patients receiving chronic iHD presents a prime opportunity for stewardship intervention. By utilizing the existing vascular hemodialysis access instead of inserting an additional peripheral catheter for antimicrobial administration, the risk for potent

[Perception of the transition process from pediatric to adult care in preterm and full-term infants].

The number of premature births is rising globally, and medical needs are often complex. The transition from pediatric to adult healthcare is a critical phase that, without adequate support, may lead to difficulties. Especially extremely preterm infants face long-term health challenges requiring specialized care. This retrospective study examined how mothers perceive the transition process of their preterm and term-born children. We analyzed whether perinatal factors such as low gestational age (GA) influence transition experiences and adult healthcare. Additionally, healthcare utilization and

Using Implementation Science-Informed Strategies to Improve Transitions of Care for Patients with Venous Thromboembolism.

BACKGROUND: Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, are common causes of preventable hospital death. Most VTEs diagnosed in the outpatient setting are directly linked to a recent hospitalization or surgery. METHODS: A type 2 effectiveness-implementation hybrid study was conducted to develop and implement targeted interventions to improve care for patients with VTE in six US health systems. Primary outcomes included (1) 7-day follow-up after VTE diagnosis (phone calls, office visits); (2) VTE-related hospitalizations or emergency department (ED) vis

[Transition from pediatric to adult care in chronic respiratory diseases: The cystic fibrosis model].

Increased life expectancy in cystic fibrosis has made transition from pediatric to adult cystic fibrosis centers a crucial step for patients, their families and caregivers. This transition must be gradual and carefully prepared. A formalized process, early discussion with patients and families about transition, patient's empowerment prior to transfer, and close links between pediatric and adult teams are key points to succeed. Therapeutic education, validated questionnaires, personalized action plans or connected tools can help. Transfer will take place at the appropriate time for each patient

Optimizing transition from pediatric to adult care in short bowel syndrome and intestinal failure.

As the majority of children with short bowel syndrome (SBS) and intestinal failure (IF) are now surviving into adulthood, there is a paradigm shift from short-term management to long-term outcomes and a growing need to focus on healthcare transition (HCT). It is imperative that adolescents and young adults with SBS and IF receive disease education, empowerment, and support as they navigate the transition from pediatric to adult care. Furthermore, both pediatric and adult healthcare providers who manage these patients should be aware of the challenges faced by this population, barriers to their

Navigating the transition of care in patients with inborn errors of immunity: a single-center's descriptive experience.

The transition from pediatric to adult care is a critical milestone in managing children, especially in those with complex chronic conditions. It involves ensuring the patient and family adapt correctly to the new phase, maintaining continuity of ongoing treatments, and establishing an appropriate follow-up plan with specialists. Patients with Inborn error of immunity (IEI), formerly known as Primary Immune Disorders (PID) are part of a group of disorders characterized by alterations in the proper functioning of the immune system; as the diagnostic and treatment tools for these entities progre

Preparing Parents for Discharge from the Neonatal Unit, the Transition, and Care of Their Preterm Children at Home.

Preparing parents to care for their preterm children is one of the principal challenges faced by nursing professionals within the neonatal care contexts. This process seeks for parents to acquire the skills to safely provide the differential care required by children at home given their prematurity condition. The preparation for discharge is complex and multidimensional, involving aspects that have to do with the knowledge, skills for caring, security, and trust to transit and take care of the children at home. This process is conducted in the neonatal unit gradually, in function of the clinic

Transition from intensive care unit to a portable ventilator in children with severe bronchopulmonary dysplasia.

As the population of ventilator-dependent children (VDC) with tracheostomies due to underlying severe bronchopulmonary dysplasia grows, there is an increasing need to shift the care of these children from hospital to home. Transitioning the ventilator-dependent child from the hospital to home is a complex process that requires coordination between the medical team and the family. One crucial step in the process is transitioning from an Intensive care unit (ICU) ventilator to a portable home ventilator (PHV). The Clinical team needs to understand the nuances in transitioning to PHV, including a

Pre-decision regret before transition of dependents with severe dementia to long-term care.

BACKGROUND: To place a dependent with severe dementia in a nursing home is a painful and difficult decision to make. In collectivistic oriented societies or families, children tend to be socialised to care for ageing parents and to experience guilt and shame if they violate this principle. Leaving the care to professional caregivers does not conform with the cultural expectations of many ethnic groups and becomes a sign of the family's moral failure. RESEARCH DESIGN: Qualitative design with individual in-depth interviews with nurses, family members and dementia care coordinators in Norway, Mo

Retention in care for persons with opioid use disorder transitioning from sublingual to injectable buprenorphine.

INTRODUCTION: In the current overdose epidemic, effective treatments for opioid use disorders (OUD), including innovations in medication delivery such as extended-release formulations, have the potential to improve treatment access and reduce treatment discontinuation. This study assessed treatment retention in a primary care-based, extended-release buprenorphine program. METHODS: The study recruited individuals (n = 92) who transitioned from sublingual buprenorphine to extended-release buprenorphine (BUP-XR) in 2018-2019. The study defined the primary outcome, treatment retention, as three o

The transition experiences of adolescents with type 1 diabetes from paediatric to adult care providers.

PURPOSE: Emerging adults with type 1 diabetes (T1D) are at increased risk for poor health outcomes as they transition from paediatric to adult healthcare providers. This is in part due to the complexities of young adult life as individuals with T1D enter the workforce, leave home or start college while learning to manage the disease on their own. We sought to identify the barriers and facilitators adolescents face during their emerging adult years with T1D. METHODS: Young adults, aged 24-35, who lived with T1D during their adolescent years were recruited online to complete a survey regarding
