Occipital nerve stimulation for chronic migraine.

Occipital nerve stimulation may be effective in treating chronic migraine. Six studies, including three double-blind studies, were performed, with five showing evidence of benefit. However, of the three randomized, controlled trials, none has met a primary endpoint successfully. A separate study suggested a benefit for combined supraorbital and greater occipital nerve stimulation.

Massage therapy as an effective treatment for carpal tunnel syndrome.

Carpal tunnel syndrome is a common peripheral entrapment that causes neuralgia in the median nerve distribution of the hand. The primary aim of this study was to evaluate the efficacy of massage therapy as a treatment for carpal tunnel syndrome. Within this process, the locations of trigger-points that refer neuropathy to the hand were identified. The creation of massage pressure tables provides a means of treatment reproducibility. Twenty-one participants received 30 min of massage, twice a week, for six weeks. Carpal tunnel questionnaires, the Phalen, Tinel, and two-point discrimination test

Moyamoya disease emerging as an immune-related angiopathy.

Moyamoya disease (MMD) is a rare cerebrovascular disorder with unknown etiology. MMD is characterized by progressive narrowing of arteries of the brain and the formation of a compensatory network of fragile vessels. Genetic studies have identified RNF213, also known as mysterin, as a susceptibility gene for MMD, but the low penetrance in genetically susceptible individuals suggests that a second hit is necessary to trigger disease onset. Recently, several molecular studies uncovered RNF213 as a key antimicrobial protein with important functions in the immune system. In addition, an increasing

The impact of endometrial fluid on single euploid frozen embryo transfers, to cancel or not?

STUDY QUESTION: What is the impact of endometrial fluid (EF) on single, euploid frozen embryo transfer (FET) cycles on live birth rate (LBR) and is cycle cancellation for EF a worthwhile intervention? SUMMARY ANSWER: The LBR of single euploid FETs was significantly lower by 20.2 percentage points when EF was persistent on the day of decision for progesterone start/trigger, but despite the lower LBR, cycle cancellation may not confer an improved chance at live birth. WHAT IS KNOWN ALREADY: The incidence of EF in cycles ranges from 3% to 8%, thus, only a few small studies have been performed t

Comparison of extracorporeal shock wave therapy targeting the posterior myofascial chain and plantar tenderness points versus plantar tenderness points alone in patients with plantar fasciitis: a randomized controlled trial.

AIM: Plantar fasciitis is a common cause of heel pain. Although extracorporeal shock wave therapy (ESWT) targeting plantar tenderness points is widely used, outcomes remain unsatisfactory in some patients. This study aimed to compare the clinical efficacy of ESWT targeting both posterior myofascial-chain trigger points and plantar tenderness points with that of conventional ESWT targeting plantar tenderness points alone in patients with plantar fasciitis. METHODS: Eighty patients with plantar fasciitis were randomly assigned to a control or treatment group (n = 40 per group). The control grou…

Altering the gag reflex via a palm pressure point.

BACKGROUND: The gag reflex regularly interferes with dental procedures. The authors hypothesize that applying pressure to a specific point on the palm alters the gag reflex and that hypersensitive gag reflexes may be categorized according to oropharyngeal landmarks. METHODS: Thirty-six neurologically intact subjects underwent a series of gag reflex trials (baseline, sham and treatment). The authors developed a hand pressure device for subjects to wear, which provided a consistent force, and they described a gag trigger point index (GTPI) scale. On the basis of the GTPI, they divided subjects

Clinical performance of Botulinum Toxin injections in the treatment of bruxism: A retrospective study.

CONTEXT: The effectiveness of botulinum toxin (BTX) in treating bruxism is recognised and well-established. However, it is often recommended in cases of severe bruxism. OBJECTIVES: This study aims to highlight the performance of trigger points (TrPs) released with BTX injections at an early stage of bruxism while providing an overview of its analgesic effect. METHODS: 67 patients who suffered from bruxism between 2009 and 2015 and who recovered completely after injection sessions. Patients were divided into two groups based on bruxism duration. The main outcomes were pain relief, injection s…

Dry needling in the management of chronic tension-type headache associated with levator scapulae syndrome: A case report.

KEY CLINICAL MESSAGE: The use of DN to the muscular trigger points and distal periosteal enthesis of the levator scapulae may be a useful adjunct intervention within a multi-modal plan of care for the management of work-related chronic tension-type headaches associated with LSS. ABSTRACT: Chronic tension-type headaches (CTTH) have a lifetime prevalence of 42% and account for more lost workdays than migraine headaches. Dry needling (DN) is being increasingly used by physical therapists in the management of CTTH; however, to date, the supporting evidence is limited. The purpose of this case rep…

Myofascial Pelvic Pain: An Overlooked and Treatable Cause of Chronic Pelvic Pain.

Myofascial pelvic pain is a common, nonarticular musculoskeletal disorder characterized by the presence of myofascial trigger points in the lower abdominal wall and/or pelvic floor muscles. Myofascial pelvic pain is involved in an estimated 22% to 94% of cases of chronic pelvic pain, which is one of the most common gynecologic conditions in the United States. Myofascial pelvic pain may exist independently or in conjunction with disorders such as vaginismus, dysmenorrhea, and endometriosis and is frequently a causative factor in sexual pain or dyspareunia. This article reviews the pathophysiolo…
