The serodiagnosis of tuberculosis was evaluated by means of enzyme-linked immunosorbent assay of IgG antibody to mycobacterial antigen 60. The study population consisted of 63 patients with active tuberculosis comprising 43 pulmonary tuberculosis, 15 tuberculosis pleurisy, four miliary tuberculosis, and one tuberculous lymphadenitis. The control group consisted of 48 patients embracing 8 cases of inactive pulmonary tuberculosis, 31 cases of pulmonary disorders other than tuberculosis, and nine normal persons. There was a significant difference in antibody titers of sera between patients of act

Haptoglobin (Hp) serum concentration is a highly sensitive, non-specific indicator of inflammation. In the present study, following the example of studies conducted on humans, we analysed whether haptoglobin concentrations in pleural fluids and sera in tuberculin-reactor cattle could be an additional biomarker of Mycobacterium bovis infection. The clinical material (serum, n = 60, pleural fluid, n = 80) was collected from 140 dairy cattle (Bos taurus) shortly before culling at slaughter. During postmortem examination, organs were assessed for tuberculosis-like lesions and tissue samples were c

Selective recruitment of IFN-γ biased Th1 effector cells at the pathologic site(s) determines the local immunity of tuberculosis (TB). We observed the enrichment of CXCR3, CCR5 and CD11a(high) T cells in the peripheral blood, pleural fluid and bronchoalveolar lavage of TB pleural effusion (TB-PE) and miliary tuberculosis (MTB) patients respectively. CXCR3(+)CCR5(+) T cells were significantly high at the local disease site(s) in both the forms of TB and their frequency was highest among activated lymphocytes in TB-PE. Interestingly, all CCR5(+) cells were invariably positive for CXCR3 but all C

Tuberculosis (TB) is among the most predominant infectious illnesses in developing areas around the globe. As stated by the World Health Organization (WHO), the number of instances of drug-resistant tuberculosis (DR-TB) has increased lately. This case report describes the effective diagnosis and customized treatment for primary extra-pulmonary multidrug-resistant tubercular pleural effusion, a disease which is difficult to identify due to relatively low bacterial count as well as frequently negative staining on Ziehl Neelsen (ZN) for acid-fast bacilli (AFB). The bacteria causing multidrug-resi

INTRODUCTION: Tuberculosis remains one of the major infectious diseases. Its incidence has grown in immigrants and in immunosuppressed patients, and, moreover, cases of drug resistance are on the increase. The goal of this study was to analyze the implications of these new developments and surgical experience in our patients.
METHODS: The cases of 104 patients with tuberculosis hospitalized between 1975 and 1995 were reviewed retrospectively.
RESULTS: 22% of the patients (23/104) were immigrants. No HIV-positive patients were described but association with debilitating diseases was frequent.

TNF and IFN-gamma are thought to be involved in the immune response to mycobacterial infection because they exhibit antimycobacterial effects in vitro. To investigate the roles of these cytokines in vivo at the site of disease activity in human tuberculosis, we evaluated local cytokine production in patients with tuberculous pleuritis. Both TNF and IFN-gamma were selectively concentrated 5- to 30-fold in pleural fluid, compared to blood of the same patients. Messenger RNA for both cytokines was detected in pleural tissue by in situ hybridization, suggesting that selective cytokine concentratio

High-mobility group box 1 (HMGB1) has been implicated in matrix remodeling and fibrotic disorders; however, its role in tuberculosis (TB)-associated pleural fibrosis remains unknown. Pleural fluid levels of HMGB1 and matrix metalloproteinase-1 (MMP-1) in patients with TB pleural effusion (TBPE, n = 36) or transudative pleural effusion (TPE, n = 14) were measured. The effects of Mycobacterium tuberculosis H37Ra (MTBRa) on HMGB1 and MMP-1 expression and their effects on mesothelial-mesenchymal transition (MMT) in human pleural mesothelial cells (PMCs) were assessed. The levels of HMGB1 and MMP-1

BACKGROUND: Disseminated tuberculosis is a rare case that causes high mortality and morbidity.
CASE PRESENTATION: A 59-year-old man with a glasgow coma scale of 12, cerebrospinal fluid was found to have dominant mononuclear, high protein level, low glucose level and shortness of breath. A chest X-ray revealed a right pleural effusion with infiltrates in both lung parenchyma and a pleural fluid adenosine deaminase (ADA) test showed 66.1 U/L. Thoracolumbar MRI revealed a compression fracture in the 6th thoracic vertebral body. The patient was given category 1 anti-tuberculosis drug (ATD) therap

BACKGROUND: There are two main causes of exudative effusion including malignancy-induced effusion and tuberculosis. Considering that in reactive ejections, such as tuberculosis-induced effusion, the role of B lymphocytes and in the malignant effusion, the role of T lymphocytes are more important, in this study we analyzed the frequency of CD4, CD8, CD19, CD56-16, CD64, QuantiFERON in the pleural and serum samples of patients with exudative lymphocytic-dominant effusion.
METHODS: In total, 73 patients were enrolled in the study by exudative lymphocyte effusion, and finally, 63 patients had def

BACKGROUND: Eosinophilic pleural effusions are defined by an eosinophil count ≥10% in pleural fluid and represent approximately 10% of exudative pleural effusions.
OBSERVATION: We report the first case of eosinophilic pleural effusion occurring due to lisinopril treatment. Improvement after drug discontinuation and recurrence after reintroduction indicated that lisinopril was responsible for the effusion.
CONCLUSION: The main causes of eosinophilic pleural effusions are infections including tuberculosis, and malignancies. Drug-induced eosinophilic pleural effusions have only rarely been desc

Hydatid cysts, caused by Echinococcus granulosis, most commonly occur in the liver as a space-occupying lesion; the second most common are lung cysts, whereas rupture into the pleural space occurs very rarely. A case of intra-thoracic hydatid cysts with invasion of the pleural space and rupture into the thoracic muscle is described in a young immigrant from Morocco to Europe (Denmark). Hydatid cyst is an important differential diagnosis from tuberculosis in immigrants from areas with endemic echinococcosis and tuberculosis, such as North Africa and the Middle East.

We analyzed hospital files of 614 pleurisy patients at the Cocody University Hospital over a 5 year period (1990-1994). We searched for the main etiologies of pleural effusion in these patients in order to propose an empirical treatment. Pleurisy is involved in 23% of all respiratory patients seen each year. Pleural effusions are observed in 91% of the cases. Clear fluid was observed most frequently (65%), followed by purulent (31%) and hemorrhagic effusion (4%). The dominant etiology was tuberculosis (66%). Non-tuberculosis infections were found in 5% of the cases. Tumoral diseases were invol

BACKGROUND: IFN-γ is presently the only soluble immunological marker used to help diagnose latent Mycobacterium tuberculosis (M.tb) infection. However, IFN-γ is not available to distinguish latent from active TB infection. Moreover, extrapulmonary tuberculosis, such as tuberculous pleurisy, cannot be properly diagnosed by IFN-γ release assay. As a result, other disease- or infection-related immunological biomarkers that would be more effective need to be screened and identified.
METHODOLOGY: A panel of 41 soluble immunological molecules (17 cytokines and 24 chemokines) was tested using Lumine

Herein, a 12-year-old Afghan boy with chronic cutaneous leishmaniasis on the face and verrucous lesions on the body and pleural effusion suspected of having co-existent tuberculosis has been presented. The cutaneous lesions were appeared for five years before his admission. Leishman-Donovan bodies were seen in H&E (Hematoxylin and eosin) slide of skin lesion specimens. The pathogenic species was proved to be Leishmania tropica using Polymerase Chain Reaction (PCR) method. Purified Protein Derivative (PPD) and Leishmanin Skin Test (LST) were strongly positive. The patient was treated with syste

Pleural tuberculosis (TB) is a form of extra-pulmonary TB observed in patients infected with Mycobacterium tuberculosis. Accumulation of myeloid-derived suppressor cells (MDSC) has been observed in animal models of TB and in human patients but their role remains to be fully elucidated. In this study, we analyzed the role of transmembrane TNF (tmTNF) in the accumulation and function of MDSC in the pleural cavity during an acute mycobacterial infection. Mycobacterium bovis BCG-induced pleurisy was resolved in mice expressing tmTNF, but lethal in the absence of tumor necrosis factor. Pleural infe

Mononuclear cells (MNC) generate cell-bound procoagulant activity (PCA) which shortens recalcification time after incubation with an antigen to which the donor has been sensitized. PCA has been demonstrated in various lung diseases, including exudative pleural effusions. To determine the value of measuring cell-bound PCA in the diagnosis of tuberculous pleural effusions we examined pleural effusion MNC of patients with tuberculosis (n = 19), congestive heart failure (n = 7), and carcinoma (n = 7). MNC were isolated, incubated in 0 or 10 micrograms/ml purified protein derivative (PPD) for 15 mi

Tuberculous pleurisy is extra-pulmonary tuberculosis caused by Mycobacterium tuberculosis (MTB), which is one of the main cause of pleural effusions in developing countries. Intercostal chest catheter is useful for drainage of infected pleural fluid and facilitates sepsis control. However, management might be challenging in complex tuberculous pleural effusion as the septations within the effusion hinder pleural drainage. Intrapleural fibrinolysis therapy improved infected fluid drainage and septic parameter in parapneumonic effusions; however, there seems to be little data on its use in tuber

A 46-year-old man was admitted to Shin-Kokura Hospital because of fever and right chest pain. Laboratory studies showed mild leukocytosis, an increased erythrocyte sediment rate, positive C-reactive protein, and abnormal liver function. The tuberculin skin test was positive. A chest X-ray showed massive right pleural effusion, and exudative pleural effusion fluid was obtained by thoracocentesis. The pleural fluid revealed an increased adenosine deaminase concentration and cultures were negative for mycobacteria.
