Coronary stenting with M-Guard: feasibility and safety porcine trial.

UNLABELLED: M-Guard is an ultra-thin polymer mesh sleeve attached to the external stent surface. It is designed to minimize distal embolization during coronary, renal, carotid and vein graft stenting. The polymer net could also serve as a platform for more uniform drug delivery. AIM: To evaluate coronary M-Guard stent deliverability and safety (stent thrombosis and restenosis) in comparison to bare-metal stents (BMS) in a porcine model of percutaneous coronary interventions (PCIs). METHODS: Under general anesthesia using percutaneous technique, 6 swine received a total of 18 coronary stents:

Paraoxonase genotype and carotid intima-media thickness in children with familial hypercholesterolemia.

BACKGROUND: Paraoxonase (PON) 1 is a high-density lipoprotein-associated enzyme that may protect against cardiovascular disease. METHOD: We have studied the contribution of PON-1 and PON-2 single nucleotide polymorphisms (SNP; L55M, Q192R and T-107C, S311C) to the intima-media thickness of the common carotid artery in a population of children with classic familial hypercholesterolaemia. RESULTS: The L-variant of the L55M SNP was associated with increased common carotid artery intima-media thickness when compared with the M-variant (P value for trend 0.03). No significant relationship was obs

Venous arterialization in extralobar pulmonary sequestration associated with fetal hydrops.

BACKGROUND/PURPOSE: Fetal extralobar pulmonary sequestration (EPS) is sometimes complicated by massive pleural effusion (PE) leading to fetal hydrops. The underlying mechanisms as well as the origin of the fluid are not well understood. This study was performed to find a histological hallmark of an EPS with massive PE. We hypothesized that venous obstruction has a role to play in the mechanisms of fluid production by EPS. METHODS: We recently experienced 3 cases of fetal EPS complicated by fetal hydrothorax requiring thoracentesis and eventually thoracoamniotic shunt placement. Total protein

Circumferential dissection with intimal intussusception: an unusual case of acute aortic type A dissection.

An unusual case of a type-A aortic dissection with complete circumferential dissection just above the aortic valve with intimointimal intussuseption just distal to the left subclavian artery partially obstructing the descending aorta is described. CT-scan and transesophageal echocardiography together with clinical suspicion led to the correct diagnosis. Intimointimal intussusception is an unusual type of aortic dissection in which a proximal circumferential tear causes dissection with intussusception of the torn intima downstream, which could cause confusion about the appropriate diagnosis ([1

Intravascular ultrasound imaging before and after balloon angioplasty for pulmonary artery stenosis.

Previous reports regarding intravascular ultrasound (IVUS) imaging of the pulmonary arteries in children and its application to balloon pulmonary angioplasty are limited. This study was designed to compare findings of IVUS imaging and those of angiography of the pulmonary artery before and after the balloon angioplasty procedure. Thirty patients had significant pulmonary artery stenosis and underwent balloon angioplasty. In all, of 34 branch pulmonary arteries were dilated. All patients underwent both angiography and IVUS imaging at the time of balloon angioplasty. The mean age at balloon angi

CT diagnosis of an unusual aortic dissection with intimointimal intussusception: the wind sock sign.

A unique combination of CT findings is reported in a rare case of aortic dissection with intimointimal intussusception. The CT showed a wind sock-like appearance in the contrast column of the aortic arch, which was felt to be characteristic of the intussuceptum. Complementary CT findings, including proximal flap in the dilated root of the aorta, no mid-ascending aortic flap, a descending aortic flap, and pericardial effusion, enabled establishment of the preoperative diagnosis.

Conjugated equine estrogens inhibit progression of atherosclerosis but have no effect on intimal hyperplasia or arterial remodeling induced by balloon catheter injury in monkeys.

OBJECTIVES: This study sought to determine the effects of estrogen treatment on atherosclerosis progression and the proliferative and structural responses of the atherosclerotic arteries to injury. BACKGROUND: Estrogen treatment suppresses the intimal response to arterial injury in nonatherosclerotic rodents and rabbits and inhibits the in vitro proliferation of smooth muscle cells. However, the effect of estrogen on the response of atherosclerotic arteries to transmural injury, as occurs in balloon catheter angioplasty in humans, is unknown. METHODS: Forty-six ovariectomized cynomolgus monk

Vascular remodeling in balloon injured rabbit iliac arteries.

To characterize long-term vascular remodeling associated with neointimal formation in vivo, we established a model of balloon injury in normal chow fed rabbits. The iliac artery was injured by denudation using a 2F embolectomy catheter. Injured vessels were removed after perfusion fixation (90 mm Hg) in situ at 2, 4, 6, and 12 weeks post-injury; control vessels were obtained from 2- and 12-week age-matched, uninjured animals. Intimal growth was observed in all animals post-injury. Intimal area averaged 0.13 +/- 0.02 mm2 2 weeks post-injury and continued to increase at 4 and 6 weeks post-injury

The morphology of venovenous bypass graft endothelium.

Venovenous bypass grafts are commonly used in the repair of vascular trauma to large- and small-caliber veins. This study examines the morphology of the venous wall in an experimental model of the venovenous bypass graft. The morphology of the venous endothelium from unmanipulated jugular veins and from jugular veins implanted as a venovenous bypass graft in the external jugular venous system for 10 min, 6 h, and 1, 3, 5, 7 and 28 days was examined. Veins and venovenous grafts were pressure fixed in situ at 80 mmHg and were examined by scanning and transmission electron microscopy. The endothe

Mechanisms of neointima formation--lessons from experimental models.

The lamina intima of an artery is the region between the endothelial cell surface and the internal elastic lamina, which forms the luminal margin of the media. In humans the intima of atherosclerosis-prone arteries becomes thicker due to accumulation of smooth muscle cells, which originate from the media. The introduction of percutaneous transluminal coronary angioplasty (PTCA) boosted scientific interest in intimal thickening, because restenosis remains an unresolved problem of this intervention. In order to unravel the mechanisms of intimal thickening there is a need for appropriate animal m

Angiotensin converting enzyme expression in primary pulmonary hypertension.

We compared immunoreactivity for angiotensin converting enzyme (ACE) in pulmonary artery and lung parenchymal tissues (obtained at the time of resection for lung transplantation) from eight patients with Stage IV primary pulmonary hypertension (PPH) with the reactivity in similar tissues from eight normal donors. ACE immunoreactivity was markedly and consistently increased in the endothelium and subendothelial neointimal regions of elastic pulmonary arteries from patients with PPH as compared with normal pulmonary arteries. Immunoreactivity in normal muscular pulmonary arteries was usually les

Role of leukocytes in neointimal formation after balloon angioplasty in the rabbit atherosclerotic model.

BACKGROUND: Adherence and transendothelial migration of circulating leukocytes is one of the initial events after vascular injury. This process is mediated principally by the expression of integrins (CD11/C18) on the cell surface, which interact with their counterparts in the vessel wall cells. In order to determine the role of leukocytes in the development of neointimal thickening after balloon angioplasty, a monoclonal antibody (R15.7) against leukocyte adherence glycoprotein CD18 was used. METHODS: Femoral artery atherosclerotic lesions were induced in 20 New Zealand White rabbits, which w

Long-term oral administration of L-arginine reduces intimal thickening and enhances neoendothelium-dependent acetylcholine-induced relaxation after arterial injury.

BACKGROUND: Nitric oxide (NO), in addition to its potent vasorelaxant properties, may participate in growth regulation of cultured smooth muscle cells. It was recently demonstrated that in vivo endothelial injury induces the production of NO from L-arginine in the arterial wall. METHODS AND RESULTS: We studied the effects of long-term administration of L-arginine, the precursor of NO, on neointimal thickening and on neoendothelium-dependent vasorelaxation 4 weeks after balloon denudation of normocholesterolemic rabbit iliac arteries. Rabbits were fed with either a standard diet or a diet supp

Catheter atherectomy of intimal fibroplasia of the common iliac artery.

An 18-year-old woman presented with renovascular hypertension and left lower extremity claudication. Aorto-iliac angiography showed stenotic lesions in the left renal artery and the left common iliac artery. For uncontrolled hypertension, nephrectomy was performed and histopathology of the renal artery showed intimal fibroplasia, an uncommon type of fibromuscular dysplasia. The left common iliac artery lesions were treated with directional atherectomy, which produced excellent immediate angiographic and symptomatic improvement.

Percutaneous polymeric stents in porcine coronary arteries. Initial experience with polyethylene terephthalate stents.

BACKGROUND: To test the feasibility of percutaneous deployment of intracoronary polymeric stents, a prototype polyethylene terephthalate (PET) stent and a catheter-based delivery system were developed. METHODS AND RESULTS: Polymeric stents were deployed in the coronary arteries of 11 Yucatan swine: six stents were placed in the left anterior descending coronary artery, four stents were placed in the circumflex artery, and one stent was placed in the right coronary artery. Stent deployment was achieved by withdrawal of an outer delivery sheath, thus allowing the PET stent to self-expand to a p

Common carotid intima-media thickness and framingham risk score predict incident carotid atherosclerotic plaque formation: longitudinal results from the study of health in Pomerania.

BACKGROUND AND PURPOSE: Cross-sectional studies describe a positive association between common carotid artery intima-media thickness (CCA-IMT) and carotid plaques (CP). However, longitudinal data on the predictive value of CCA-IMT for occurrence of CP are limited. Therefore, the role of increasing CCA-IMT in the atherosclerotic process is still discussed controversially. METHODS: We investigated the predictive value of CCA-IMT and the Framingham risk score (FRS) for incident CP formation in a population-based longitudinal study of 1922 subjects aged 45 to 81 years who underwent ultrasonograph
