The diagnostic role of ultrasound in cubital tunnel syndrome for patients with a previous cubital tunnel surgery.

BACKGROUND: Ultrasound is widely used in the diagnosis of peripheral nerve compressions. Nevertheless, the role of ultrasound, and in particular the cross-sectional area (CSA) measurements, in the diagnosis of cubital tunnel syndrome (CuTS) is debatable, especially in patients who have had previous surgeries. We evaluated the diagnostic value of ultrasound and CSA measurements in a heterogenous group of CuTS patients suffering from persisting or recurrent CuTS after a previous surgical intervention. METHODS: All patients with persisting or recurrent CuTS after previous surgery, who received a

Anterior interosseous nerve transfer combined with cubital and ulnar tunnel release for severe ulnar nerve compression.

INTRODUCTION: We investigated the outcomes of patients with severe ulnar nerve compression who underwent anterior interosseous nerve transfer combined with cubital and ulnar tunnel release. METHODS: Twenty-eight patients with McGowan IIB or III ulnar neuropathy caused by compression and with compound muscle action potentials of the ulnar intrinsics less than 3 mV were prospectively recruited for outcome analysis. Clinical signs of ulnar palsy, two-point discrimination tests, gripping and pinching force and electrophysiological studies were recorded over a 2 year follow-up. RESULTS: Significa

Ulnar conduction block at the wrist.

Two cases of ulnar nerve lesions at the wrist are reported. The lesions had an acute onset and exclusively impaired the ulnar motor deep branch. The coexistence of carpal tunnel syndrome in each case allowed an early diagnosis but was somewhat misleading. In both cases, the use of classic motor and sensory conduction studies did not provide clear abnormalities that would have precisely determined the site of the nerve lesion. In both cases, only palmar stimulation of the ulnar motor deep branch showed an important conduction block. This electrodiagnostic finding showed definitively the site of

Comparison of hand responses to recipient nerve stimulation in sequential variations of supercharged anterior interosseous nerve transfer to the ulnar nerve - Identifying the golden recipient fascicles.

BACKGROUND: In supercharged end-to-side (SETS) anterior interosseous nerve (AIN) transfer for compressive ulnar neuropathy, selecting the optimal recipient fascicle remains challenging owing to weak or absent motor responses. We hypothesized that modifying the procedural sequence would improve intraoperative nerve responsiveness, thereby enhancing the identification of recipient fascicles in the thumb, 4 digits, and hypothenar muscles (3 target muscle groups). METHODS: We retrospectively reviewed 52 patients who underwent SETS AIN transfer from 2019 to 2024. Group 1 (n = 25) followed this seq

Causes, symptoms, and treatments of nerve entrapments around the elbow: Current concepts.

The elbow is a joint extremely susceptible to stiffness, even after a trivial trauma. As for other joints, several factors can generate stiffness such as immobilisation, joint incongruity, heterotopic ossification, adhesions, or pain. Prolonged joint immobilisation, pursued to assure bony and ligamentous healing, represents the most acknowledged risk factor for joint stiffness. The elbow is a common site of nerve entrapment syndromes. The reasons are multifactorial, but peculiar elbow anatomy and biomechanics play a role. Passing from the arm into the forearm, the ulnar, median, and radial ner

Diagnosis of ulnar neuropathy: a new approach.

Conventional electrodiagnosis used to detect an ulnar neuropathy at the elbow depends on accurate determination of ulnar nerve length across this segment. We present a new approach, using the difference in latency of the compound nerve action potentials (CNAPs) of the ulnar and median nerves elicited by stimulation at the wrist and recorded 10 cm above the elbow. Sixty normal controls were examined in order to determine the normal upper limit (1.4 ms) of the difference in CNAP latency of the ulnar and the median nerves (Dlat index). Values obtained in 10 patients with ulnar nerve lesions are d

Sleeve-Shaped Neurothekeoma of the Ulnar Nerve: A Unique Case of a Still Unclear Pathological Entity.

Background: Neurothekeomas are slow-growing, well-circumscribed benign neoplasms. They usually involve cutaneous or subcutaneous tissues. Although originally described as myxomas deriving from nerve sheath cells, their exact histological classification is still uncertain. Peripheral nerve localization is rarely reported. Here, we describe a unique case of sleeve-shaped neurothekeoma of the ulnar nerve, which was incidentally discovered during a cubital tunnel release surgery. Methods: A 57-year-old man was admitted at our institution with clinical, ultrasonographic, and electromyographic findi

[Should we divide Osborn's ligament during epicondylectomy and in situ decompression of the ulnar nerve?].

INTRODUCTION: Two groups of patients with cubital tunnel syndrome were treated by neurolysis and medial epicondylectomy. In the first group, the operative procedure consisted solely of dividing Osborn's ligament and fascia but in the second group Osborn's ligament was reinserted after epicondylectomy to avoid dislocation of the nerve. The aim of this retrospective study was to compare the level of complete recovery after surgery and the frequency of dislocation of the nerve. MATERIAL AND METHOD: Group one: Nineteen patients, with a mean age of 47.7 (15-65), and 52% female, with the dominant h

Osborne's ligament: Anatomical study with application to better understanding ulnar nerve compression at the elbow.

Objective Almost 70 years ago, Osborne described a fibrous band, Osborne’s ligament, stretching between the humeral and ulnar heads of the flexor carpi ulnaris, which forms the cubital tunnel’s roof. Prior to this account, this ligament had received limited attention in the anatomical literature and had been poorly studied. Therefore, the present anatomical study was performed to better elucidate this structure. Methods thirty adult anatomical donors (60 sides) underwent dissection of the medial aspect of the elbow with a focus on Osborne’s ligament. Identified ligaments were documented, class

Comparative study of different surgical transposition methods for ulnar nerve entrapment at the elbow.

This study compared the therapeutic effects of two techniques for surgical decompression treatment for ulnar nerve entrapment at the elbow: subcutaneous transposition and modified submuscular transposition with Z-lengthening of the pronator teres origin. A total of 278 patients with ulnar nerve entrapment (McGowan grades I - III) were randomly assigned to undergo one of these techniques. All patients were followed-up for 2 years. The effects were assessed by preoperative and postoperative cross-sectional area, motor conduction velocity, sensory conduction velocity and nerve action potential. A

[How to avoid operative injury to Martin-Grüber communicating branch in the transmuscular medial approach of the elbow?].

The incidence of Martin-Grüber medio-ulnar communicating branch (MUCB), from median nerve to ulnar nerve, is about 20%. To avoid operative injury to MUCB, the authors suggest some modifications to the medial approach of the elbow. They suggest medial aponeurotomy of the anterior compartment of the forearm, disinsertion of the medial epicondylar muscles with dissection between the flexor carpi ulnaris (FCU) and the other medial epicondylar muscles, not exceeding 3 cm distally. This allows a valuable exposure of the elbow joint through a medial approach, with FCU disinsertion and ulnar nerve tra

[Carpal tunnel syndrome. Current approaches].

A clinical, epidemiological and nerve conduction studies report on carpal tunnel syndrome was done after electrophysiological author's experience on 668 cases and literature review. The median nerve underwent focal (nodal) or segmental demyelination after compression on carpal tunnel, 3-4 distal to wrist fold. The symptomatic complex includes nocturnal hands numbness and paraesthesia, mostly bilateral and between 40-60 years old. Familial cases are described and the gene could encode thick transverse carpal ligament. Anthropomorphic findings could also bring about an additional risk, but with

Management of Ulnar Nerve Irritation by Pisiformectomy: A Clinical Anatomical Study.

PURPOSE: The goal of this anatomical clinical study is to clarify whether a simple pisiformectomy can provide sufficient decompression of the ulnar nerve along its entire course within the ulnar canal. METHODS: A simple pisiformectomy was performed in 20 cadaveric hands, followed by complete release of the ulnar nerve. The anatomical findings were subsequently correlated with clinical outcomes in 17 patients presenting with ulnar nerve irritation and pisotriquetral joint pain. RESULTS: Our anatomical results demonstrated that additional division of fibrous structures was not necessary in any

Anconeus Epitrochlearis Muscle Causing Ulnar Neuropathy at the Elbow: Clinical and Neurophysiological Differential Diagnosis.

Ulnar neuropathy at the elbow is the second most frequent entrapment neuropathy and is considered idiopathic in most patients. However, several anatomic variations, including the anconeus epitrochlearis muscle, have been reported to cause ulnar nerve compression. The anconeus epitrochlearis muscle is a common anatomic variation, with a prevalence of up to 34%, but the clinical diagnosis of ulnar neuropathy of the elbow as a result of this variation is rare, with an unknown prevalence. It is a congenital accessory muscle between the medial humeral epicondyle and the olecranon that covers the po

Outcome of simple decompression of the compressed ulnar nerve at the elbow - influence of smoking, gender, and electrophysiological findings.

BACKGROUND: Compression of the ulnar nerve at elbow is frequently treated with simple decompression. Knowledge about factors influencing results of surgery of the nerve is limited and contradictory. The primary aim was to evaluate outcome of simple decompression of the nerve using a QuickDASH questionnaire, and to investigate any influence of smoking, gender, and preoperative electrophysiological findings. A second aim was to estimate the relation between QuickDASH score and a clinical assessment of outcome by the surgeon. METHODS: Patients who were operated on with simple decompression of th

Ulnar nerve transposition at the elbow under local anesthesia: a patient satisfaction study.

BACKGROUND: Ulnar nerve decompression at the elbow traditionally requires regional or general anesthesia. We wished to assess the feasibility of performing ulnar nerve decompression and transposition at the elbow under local anesthesia. METHODS: We examined retrospectively the charts of 50 consecutive patients having undergone ulnar nerve entrapment surgery either under general or local anesthesia. Patients were asked to estimate pain on postoperative days 1 and 7 and satisfaction was assessed at 1 year. RESULTS: On day 1, pain was comparable among all groups. On day 7, pain scores were twic

[Tendon entrapment after forearm distal bones fracture, case report].

In the aftermath of a forearm trauma, tendon contractures are difficult to diagnose and evoke nerve compression or muscle ischemia (Volkmann's syndrome). One rarely thinks of tendon incarceration within the fracture and the diagnosis is often made long after. During claw fingers retraction, it is known as "false Volkmann's syndrome" (Baudet and Lafond, 1979) or "pseudo Volkmann's syndrome" (Deeney et al., 1998). The authors report the case of ulnar claw fingers retraction, one year after a fracture of both bones of the forearm, treated surgically with recuperation of normal mobility immediatel

Cubital tunnel perfusion in different postures-An anatomical investigation.

INTRODUCTION/AIMS: For cubital tunnel syndrome, the avoidance of predisposing arm positions and the use of elbow splints are common conservative treatment options. The rationale is to prevent excessive stretching and compression of the nerve in the cubital tunnel, as this mechanical stress impedes intraneural perfusion. Data regarding those upper extremity postures to avoid, or whether elbow flexion alone is detrimental, are inconsistent. This study aimed to assess perfusion and size changes of the cubital tunnel during different postures in an experimental cadaver setup. METHODS: Axillary ar
