Mature ovarian dermoid cyst invading the urinary bladder.

Mature ovarian dermoid cysts are common lesions, accounting for up to 10-25% of all ovarian neoplasms. Uncomplicated dermoid cysts are often asymptomatic and are relatively easy to diagnose on imaging and to treat. Symptoms develop once complications set in and these may cause diagnostic dilemmas. Torsion (16%) is the most common complication, while rupture, suppuration and malignant transformation are relatively uncommon. Of all these complications, spontaneous rupture into the urinary bladder is least common. The diagnosis of this condition has been through the use of cystoscopy or laparotom…

[Disorders in urinary bladder micturition after rectal operations].

AIM: Functional disorders of micturition after rectal resection were investigated on an in-house patient collective. Because the neuro-anatomy of the small pelvis is very complex, the aim of this study was to evaluate the incidence and form of disorders in micturition. MATERIALS AND METHODS: A total of 536 patients were operated on for a rectal carcinoma during the period 2000 to 2004 at the Surgical University Clinic in Erlangen. Patients with a recurrent tumor and patients who died during the study were excluded (140 patients). All patients were retrospectively questioned on the pre-operati

Benign nerve sheath tumors on urinary bladder biopsy.

Benign nerve sheath tumors (schwannoma and neurofibroma) involving the urinary bladders are rare with only case reports and limited series. We identified 6 neurofibromas and 2 schwannomas involving the urinary bladder. Of the 8 cases, 7 were sent to one of the authors in consultation and the last came for treatment to The Johns Hopkins Hospital. Patients (3 males, 5 females) ranged in age from 3 to 69 years old (average 36 y old). Presenting symptoms included recurrent infection in 6 patients, hematuria in 3 patients, and irritative symptoms in 3 patients (some patients with 2 symptoms). Three…

Bladder pain from indwelling urinary catheterization: case study.

Indwelling urinary catheters can cause severe pain and discomfort and can impair a person's quality of life. Constipation can cause bladder emptying problems and pain if a catheter is present. If the person is constipated this should be treated. If there are no clinical indications for urinary catheterization the catheter should be removed. If the catheter is clinically necessary it may be possible for the person to use intermittent self-catheterization. If this is not possible the nurse should adopt a problem-solving approach to determine the reasons why pain is occurring. Ensuring that the c

Childhood urinary symptoms predict adult overactive bladder symptoms.

PURPOSE: A relationship between childhood urinary symptoms and adult lower urinary tract symptoms in women is often clinically suspected. In this analysis we investigated the relationship between childhood and adult urinary symptoms in middle-aged women. MATERIALS AND METHODS: A population based cohort of 2,109 women 40 to 69 years old who were members of a large health maintenance organization was randomly selected from age and race strata. Through self-reported questionnaires, women recalled a childhood history of and current urinary lower urinary tract symptoms, including frequent daytime

Urinary catheter policies for long-term bladder drainage.

BACKGROUND: People requiring long-term bladder draining commonly experience catheter-associated urinary tract infection and other problems. OBJECTIVES: To determine if certain catheter policies are better than others in terms of effectiveness, complications, quality of life and cost-effectiveness in long-term catheterised adults and children. SEARCH STRATEGY: We searched the Cochrane Incontinence Group specialised trials register (searched 9 June 2003). Additionally, we examined all reference lists of identified trials. SELECTION CRITERIA: All randomised and quasi-randomised trials comparin

Endogenous factors modulating urinary bladder compliance in mice.

The application of repeated cystometric measurements in longitudinal long-term studies both in patients and experimental animals requires detailed knowledge of factors causing intraindividual variability. In the present study, the effect of age, body weight, estrus cycle phase, rectal temperature and premeasurement bladder volume on the results of cystometry in mice have been assessed. No systematic changes in bladder compliance parameters were observed with age (75 to 344 days) or body weight (18 to 38 gm.). Also, the animal's position in the estrus cycle had no effect on bladder capacity. Th

Abscess of urachal remnant mimicking urinary bladder neoplasm.

Three cases of urachal abscess with extensive interstitial inflammation and fibrosis are reported. Clinically, all patients had a lower abdominal mass and CT scan and ultrasonic examination revealed a large tumour located anterior or superior to the bladder. They all underwent laparotomy and had either a partial cystectomy or an en bloc resection of the bladder and adjacent organs due to severe adhesions. Microscopic examination of the specimens revealed no evidence of carcinoma and only abscess formation with extensive chronic interstitial inflammation and fibrosis were seen. In one case, res

Malakoplakia of urinary bladder following cadaveric renal transplantation.

Malakoplakia of the urinary bladder following cadaveric renal transplantation in a twenty-two-year-old woman is reported. Urinary tract infection with Escherichia coli persisted postoperatively. Three years later, gross hematuria and fever occurred. Yellow-tan mucosal plaques or nodules were observed cystoscopically, and histologic examination revealed malakoplakia of the urinary bladder with characteristic foamy histiocytes containing Michaelis-Gutmann bodies.

External sphincterotomy in functional urinary bladder outlet obstruction.

Transurethral external sphincterotomy was performed in 36 male and 2 female patients with obstructive symptoms. The patients were studied with synchronous pressure-flow-electromyography investigations. They all had hypoactive bladders without organic obstruction, as determined by urethroscopy, micturition urethrocystography, urethral pressure profile and sphincterometry. 11 patients had urinary retention. Sphincterotomy was done at the 12 o'clock position. Voiding improved in 33 (91%) of the 36 male patients. During the follow-up time, 7 patients were reoperated with altogether 15 sphincteroto

[A new pathology: unstable urinary bladder (author's transl)].

Thirty-five cases of incontinence of urine due to hyperactivity of the detrusor as a result of unstable urinary bladder were found among 100 patients with incontinence. The cause of the hyperactivity was established after a thorough urological and neurological screening with intravenous urography, seriated mictional cystoureterography, cystoscopy, and urine culture. The diagnosis was considered definite if uninhibited contractions of the detrusor were detected by electrocystomanometry. The primary clinical symptom was an imperious urge to mi-turate, with or without leakage of urine. Diurnal an…

Urinary fibrinogen degradation products (FDP) in bladder cancer.

The value of urinary fibrinogen degradation products (EPD) as a biological marker of bladder cancer has been studied. An increase in FDP was found in 75% of bladder cancer patients, independent of the fact that they had active tumours or that they were disease free. A high correlation between the amount of the increase and the grade, local invasiveness, and risk of recurrence of the tumour has been established. The assessment of urinary FDP is considered as a valuable screening and follow-up test in patients with bladder neoplasms.

Nomograms of total renal volume, urinary bladder volume and bladder wall thickness index in 3,376 children with a normal urinary tract.

BACKGROUND: We have previously shown that urinary bladder volume index (BVI = length x width x depth of bladder) and bladder volume wall thickness index (BVWI = BVI at full bladder/average bladder wall thickness) are useful indicators of bladder dysfunction in children with enuresis and urinary tract infection. These indices show a good correlation with urodynamic studies. We have expanded the study to include normal paediatric subjects with a wide age range. We illustrate a simple sonography protocol with nomograms of different parameters, which provide useful references for functional assess…

[Urinary incontinence in the neuropathic bladder in children. Treatment using an artificial urinary sphincter].

The neurogenic urinary incontinence can be successfully managed by implantation of an artificial urinary sphincter. The most important requirement for implantation is the recognition that the patient has failed to respond to well-accepted conservative measures or is an unsuitable candidate for such conservative measures. It is necessary to rule out the presence of detrusor hyperreflexia or to show that, if it exists, hyperreflexia can be easily pharmacologically controlled. At the same time the bladder's capability for storage of urine must be "adequate" and the vesical compliance normal or on
