Defining the reliability of bladder scan in patients with obesity with postoperative urinary retention.

BACKGROUND: Postoperative urinary retention warrants catheterization. A bladder scan helps check for postoperative urinary retention before catheterization. There is a paucity of data about its reliability in postoperative obese patients. Our study assesses its reliability in this patient population. METHODS: All patients in the postoperative ward of one hospital who underwent surgery during their current admission were included in our prospective cohort study. Patients were catheterized on the basis of the clinical judgment of the treating physician. The nurses collected bladder scan reading

Indications for and outcomes of clean intermittent catheterization in children with normal genital sensation.

PURPOSE: Clean intermittent catheterization is generally accepted by parents and children with neurogenic bladder. We analyzed the indications for and feasibility of clean intermittent catheterization in children with normal genital sensation. MATERIALS AND METHODS: We reviewed the records of 2 girls and 15 boys 15 months to 15 years old (mean age 8 years) on clean intermittent catheterization with normal genital sensation. Indications for clean intermittent catheterization were urinary retention and poor urodynamic compliance with or without hydronephrosis. Parents evaluated the protocol via

Prevention of postherniorrhaphy urinary retention with prazosin.

Urinary retention that necessitates catheterization after herniorrhaphy is a well known, but usually ignored, situation. Increased sympathetic activity resulting from surgery may be the contributing factor. Blockade of alpha receptors in the bladder neck and urethral sphincter may prevent postoperative urinary retention. In this prospective placebo-controlled study, the efficacy of prazosin in preventing postoperative urinary retention after herniorrhaphy was investigated in 156 patients. Patients were randomized into two groups. Patients in Group I (control) were given placebo orally 12 hours

Labioplasty for hypertrophic labia minora contributing to recurrent urinary tract infections.

INTRODUCTION: Reduction labioplasty for hypertrophic labia minora is often performed for aesthetic reasons by cosmetic vaginal surgeons. However, hypertrophy of the labia minora can also result in significant morbidity because it causes physical discomfort during activity and interferes with the ability to maintain local hygiene and perform intermittent self-catheterization in certain patients. OBJECTIVES: We describe a case in which a commonly used technique for reduction labioplasty was performed on a patient who had difficulty performing intermittent catheterization and report on the aesth

Systematic review and meta-analysis: reminder systems to reduce catheter-associated urinary tract infections and urinary catheter use in hospitalized patients.

BACKGROUND: Prolonged catheterization is the primary risk factor for catheter-associated urinary tract infection (CAUTI). Reminder systems are interventions used to prompt the removal of unnecessary urinary catheters. To summarize the effect of urinary catheter reminder systems on the rate of CAUTI, urinary catheter use, and the need for recatheterization, we performed a systematic review and meta-analysis. METHODS: Studies were identified in MEDLINE, the Cochrane Library, Biosis, the Web of Science, EMBASE, and CINAHL through August 2008. Only interventional studies that used reminders to ph

Retrospective Multicenter Observational Study of Immediate Voiding at End of Urinary Sphincter Surgery (REMOVE).

PURPOSE: Patients may remain catheterized after artificial urinary sphincter surgery to prevent urinary retention, despite a lack of evidence to support this practice. Our study aims to evaluate the feasibility of outpatient, catheter-free continence surgery using a multi-institutional database. We hypothesize that between catheterized controls and patients without a catheter, there would be no difference in the rate of urinary retention or postoperative complications. MATERIALS AND METHODS: We conducted a retrospective review of patients undergoing first-time artificial urinary sphincter pla

Urinary infection after orthopedic procedures.

We have investigated prospectively the incidence of urinary tract infection (UTI) in 5320 orthopaedic patients. There were 74 UTIs (1.39%). Enterobacteriaceae was the most frequent etiological agent. Each infection increased the length of stay in hospital by more than 8 days. Statistically independent risk factors for the development of urinary infection were a preoperative stay of more than 4 days, inadequate preoperative preventive measures, central venous catheterization and urinary catheterization. Sex, age, or type of surgery had no statistical influence on the development of infection.

Avoiding Bladder Catheters During Atrial Fibrillation Ablation.

OBJECTIVES: This study sought to determine if atrial fibrillation (AF) ablation can be performed safely without bladder catheterization. BACKGROUND: Patients undergoing AF ablation often receive bladder catheters. Catheterization is associated with potential complications. The ABCD-AF (Avoiding Bladder Catheters During Atrial Fibrillation) ablation study evaluates the advantages of performing AF ablation without routine catheterization. METHODS: In this single-center, prospective, randomized controlled trial, 80 patients received bladder catheterization (group A), and 80 patients received on

Shortening Indwelling Catheterization After Vaginal Surgery for Pelvic Organ Prolapse: Results from a Prospective Randomized Trial.

Background/Objectives: Pelvic organ prolapse (POP) is a common condition affecting women. When conservative treatment fails, surgical correction is indicated. Anterior colporrhaphy (AC) is a standard procedure for anterior vaginal wall prolapse repair. Postoperatively, an indwelling urinary catheter (IUC) is typically inserted to prevent urinary retention; however, prolonged catheterization is a known risk factor for urinary tract infection (UTI). This study aimed to evaluate whether postoperative catheterization can be safely shortened from 4 days to 24 h after vaginal POP surgery, and to com

Optimal duration of urinary drainage after rectal resection: a randomized controlled trial.

BACKGROUND: Voiding dysfunction is frequently observed after rectal resection and justifies urinary drainage. However, there is no agreement about the optimal duration of this postoperative drainage. The aim of this controlled trial was to compare 1 versus 5 days of transurethral catheterization after rectal resection, with special reference to urinary tract infection and bladder retention. METHODS: One hundred twenty-six patients undergoing rectal resection were included in a prospective randomized study designed to compare the results for patients undergoing 1 day of transurethral catheteri

Urine leakage from the umbilicus in a child with achondroplasia and tetraplegia (due to cervical stenosis): a safety vent for the obstructed neuropathic bladder.

Urine leakage from the umbilicus was observed while expressing urine by the Crede manoeuvre in a three-year-old tetraplegic girl with a chronically distended urinary bladder. Intravenous urography (IVU) revealed bilateral hydroureteronephrosis with markedly distended urinary bladder. Regular three hourly intermittent catheterization was advised, and the parents and carers of this child agreed to perform catheterization. There was cessation of urine leak within 48 hours of urethral drainage. Cystography performed two weeks later showed no vesicoureteric reflux; vesicoumbilical fistula was no lo

The Efficacy of Intraoperative Ketamine-Haloperidol for Prevention of Catheter-related Bladder Discomfort After Lumbar Spinal Stenosis Surgery.

OBJECTIVES: Catheterization of urinary bladder during surgery frequently leads to agitation of the patient in the recovery room, especially in those patients who remain catheterized after gaining consciousness. We hypothesized that administration of a combination of ketamine-haloperidol (KH) before urinary catheterization would reduce the incidence of catheter-related bladder discomfort (CRBD) while reducing some adverse effects of ketamine in the postoperative period. METHODS: A total of 119 male patients who underwent lumbar spinal stenosis surgery were randomized into three groups. The KH …

Catheter knots in the urinary tract.

The use of catheters in the urinary tract, both in children and adults, has not been without complications. Many procedures performed in the urinary tract, including urethral or suprapubic catheterization of the bladder, urethrography, and ureteral catheterization, have showed complications, one of which is the formation of catheter knots. This complication, although very rare, yet it is very annoying. In this study, we searched (in winter 2002, employing Medline) and reviewed published reports of urinary tract catheter knots, and present measures for prophylaxis, diagnosis and proper manageme

Factors Affecting Difficulties Faced by Women During their First Year of Clean Intermittent Self-Catheterization.

INTRODUCTION AND HYPOTHESIS: Clean intermittent self-catheterization represents a cornerstone in the management of lower urinary tract dysfunction in women. Patients may be confronted with many difficulties during their early experience of self-catheterization, which may lead to discontinuation. The purpose was to identify the factors affecting difficulties that women with lower urinary tract dysfunction face during their first year of clean intermittent self-catheterization. METHODS: Females older than 18 years who were on clean intermittent self-catheterization for less than 1 year responde

Suprapubic Versus Transurethral Catheterization: Perioperative Outcomes After Colpocleisis.

OBJECTIVES: Transient postoperative urinary retention occurs in approximately half of women after colpocleisis; however, the optimal strategy for postoperative catheter management is unclear. This study compared length of catheterization and postoperative complications after colpocleisis between planned suprapubic catheter placement and transurethral catheterization. METHODS: This is a retrospective cohort study including all women undergoing colpocleisis from January 2015 to December 2019 in a large female pelvic medicine and reconstructive surgery practice. Women undergoing planned placemen

Epidural anaesthesia and urinary dysfunction: the risks in total hip replacement.

Epidural anaesthesia in total hip replacement is an established and safe practice. It may be used alone or in combination with general anaesthesia for analgesia. Urethral catheterization in the perioperative period is known to greatly increase the complication of deep sepsis following total hip replacement. We assessed the effect on the incidence of urinary catheterization of using bupivocaine epidural anaesthesia in addition to general anaesthesia in total hip replacement. A prospective study was made of 113 total hip replacements. The incidence of catheterization in male patients who receive…

Unusual complication of prolonged indwelling urinary catheter - iatrogenic hypospadias.

Long-time urethral catheterization may be responsible for various complications such as urethral stricture, urethral fracture, urinary tract infections, and hypospadias. Hypospadias is the most common congenital anomaly of male external genitalia. However, urethral catheter-induced iatrogenic hypospadias is a rare entity. In this article, we describe a case of an elderly male who was found to have iatrogenic hypospadias 2 months after urinary catheterization.

Portable ultrasound determination of urinary residual volume.

A portable ultrasound to determine bladder volume, the BladderScan 2500, was compared to transurethral catheterization in 72 women. Most measurements were made after voiding. Volumes measured by catheterization were 17 ml greater than those measured by ultrasound, which was not clinically significant. The BladderScan technique does not carry the risk of urinary tract infection or patient trauma, and patient dignity is preserved, while accurately determining urinary volume.

Urinary Tract Infections: 2021 Update.

Catheter-associated urinary tract infection (CAUTI) remains one of the most prevalent, but preventable, health care-associated infections and predominantly occurs in patients with indwelling urinary catheters. Duration of urinary catheterization is the most important modifiable risk factor for development of CAUTI. Alternatives to indwelling catheters should be considered in appropriate patients. If indwelling catheterization is necessary, proper aseptic practices for catheter insertion and maintenance and use of a closed catheter collection system are essential for preventing CAUTI. The use o…
