This paper provides an analysis of the last 50 years of uterus transplantation (UTx). Animal research on UTx began in the 1960s, aiming to solve tubal factor infertility. Despite awareness of obstetricians to the constant increase in the number of caesarean sections in recent years, certain dogmas concerning uterine scar still persist in our practices and influence clinical decisions. A spontaneous uterus rupture occurred during the labour of a 37-week-pregnant woman showing a precocious rupture of membranes. It appeared enlarged and the cut surface of the myometrium showed coarse trabeculations. The histological examination showed a hypertrophic gravidic myometrium with heterogeneous areas of fibrosis and adenomyosis, necrotic decidual foci and hyperplastic cervical canal glands. Our experience suggests that a silent and spontaneous uterine rupture, is possible even in the absence of the principal risk factors.

Two cases with rare location of ectopic accessory ostium ureteris in the cervix uteri are described. This abnormality can be combined with a vaginal and/or uterine developmental defect, leading to female sterility. Acute inversion of the uterus is a rare complication during cesarean section. We describe one such case in which diagnosis was made immediately and reversion was performed within few minutes. A high index of suspicion and prompt management can prevent further complications.

In the period between 1990-1995, six women with complete septate uterus presented to our clinic and underwent uterine reconstruction using our own unique procedure, a method we independently developed using the Jones & Jones procedure and the Thompkins procedure for reference. Five of these 6 cases later became pregnant, and to date, they have given birth to 7 newborns. Because of a very high rate of successful postoperative pregnancy, we were convinced that uterine reconstruction is useful for complete septate uterus.
