The results of truncal vagotomy and pyloroplasty for gastric, prepyloric and combined ulcers in 215 patients were evaluated 9-15 years after operation. The operative mortality was 1.9 per cent, and the recurrence rates in gastric, prepyloric and combined ulcer patients were 8, 13 and 0 per cent respectively. This paper presents the results obtained in 15 patients with duodenal ulcer and stenosis who were treated more than 3 years ago by highly selective vagotomy (HSV) and dilatation of the stenosis without a gastric drainage procedure. Patients were taking solid food 3--6 days postoperatively and were discharged after 7--14 days. There have been no recurrent stenoses. Fourteen of the 15 patients were Visick grade 1 or 2 at their last visit. One patient has a recurrent ulcer, but no restenosis. Possible promotion of MNNG-induced gastrointestinal carcinogenicity was evaluated in male Wistar rats exposed to unconjugated bile acid given as gavage or as obtained through truncal vagotomy plus pyloroplasty. No significant difference was found compared with the relevant control groups. After 138 selective proximal vagotomies--130 of which were performed because of chronic duodenal ulcer--the rate of the postoperative complications was 3,8%, which is very low; the primary lethality was 0,76%. At re-examination 68% of the patients were without complaints; 8% had a recurrent ulcer. Epigastric fulness (6%), dumping (1%) and diarrhea (1%) were rare. Because of our results we believe, that selective proximal vagotomy is the optimal method for the surgical treatment of duodenal ulcer.

Intravenous administration of chlordiazepoxide (CDP) caused respiratory depression in both rats and cats. The maximally tolerated dose of CDP was found to be 165 +/- 15 mg/kg, i.v., in rats. Pretreatment with picrotoxin (1.5 mg/kg) or naloxone (20 mg/kg, i.v.) significantly increased the maximally tolerated dose of CDP to 330 +/- 40 mg/kg, i.v., and 270 +/- 35 mg/kg, i.v., respectively. The protective effects of both naloxone and picrotoxin were absent in bilaterally vagotomized rats. The initial phase of gastric emptying of solid food was monitored with the use of a gamma camera and on-line computer facilities. In normal subjects a consistent pattern was seen comprising an early delay in the onset of emptying ("plateau"), followed by an exponential decay curve. After vagotomy the plateau was lost, and the exponential pattern of emptying commenced once the stomach was full. There was no evidence of sudden "dumping" of contents from the stomach. The overall rates of emptying as measured by the half emptying time (T 1/2) were the same in normal and postvagotomy subjects.

Gastric resection or vagotomy plus drainage are not easy alternatives for the treatment of duodenal ulcer. More complications are met after resection, more relapses after vagotomy, whereas the postoperative sequelae are the same for both operations. Vagotomy of the parietal cells is designed to prevent all such sequelae by leaving the innervation of the antrum intact so that drainage can be omitted. The myoelectrical activity of the gastric antrum was measured in 13 patients using intraluminal suction electrodes. Exocrine pancreatic function was studied after meal stimulation and intravenous hormone stimulation with secretin and cholecystokinin in 10 patients with duodenal ulcers before, and 3 to 4 months after, highly selective vagotomy. An instance of volvulus of the transverse colon in the postoperative period following truncal vagotomy and anterior gastrojejunostomy is reported. The clinical features of volvulus of the transverse colon are sudden onset of abdominal colic, distension and vomiting. A plain X-ray film of the abdomen is diagnostic. Laparotomy is recommended, followed by untwisting alone, or untwisting and colostomy when the bowel is viable, or resection with a transverse colostomy and mucous fistula as a first stage when the bowel is gangrenous.

It is the authors' opinion that low values of acid production do not exclude the importance of the acid-peptic factor in formation of duodenal ulcers. Vagotomy and pyloroplasty in most patients showing duodenal ulcer with reduced posthistamine secretion provide for good results.

Recently cholinergic stimulation of the lower esophageal sphincter (LES) with bethanechol has been shown to be effective in the treatment of chronic gastroesophageal reflux.
