Shared Decision-Making with Palliative Care Specialist.

Shared decision-making (SDM) represents a central framework for achieving person-centered, value-concordant care in palliative and end-of-life settings. This review synthesizes current SDM concepts, practical models, and future directions relevant to palliative care specialists. Foundational models, such as the three-talk framework and the National Institute for Health and Care Excellence guidelines, emphasize iterative dialogue, informed preferences, and relational trust. In clinical practice, SDM is integrated with adaptive interdisciplinary approaches that align immediate treatment choices

'Genuine doctor care': Perspectives on general practice and community-based care of Australian men experiencing homelessness.

People with complex health and social needs, including tri-morbidity and homelessness, are challenging for modern healthcare systems. These clients have poor health and social outcomes. They tend to use available health resources inefficiently, with fragmented, uncoordinated use of multiple health and social care services. Increasing access for these clients to well-supported general practice care may be an effective response to these challenges. The aim of this study was to explore client experiences of, and attitudes to, community-based healthcare, and general practice in particular, to iden

Family physicians' perspectives on outcomes, processes, and policies in dementia care.

The rapid rise in numbers of people living with Alzheimer's disease and related disorders (ADRD) poses major challenges to health systems and policy. Although primary care clinicians provide ongoing medical care for 80% of affected individuals, they face persistent barriers to providing high-quality dementia care. We conducted qualitative interviews with family physicians (n = 20) to understand what core outcomes they consider most important and what care processes and systems and policy strategies they propose to achieve them. Participants identified processes and outcomes pertaining to their

Health-State Utility Values in CP Patients Following Deformity Surgery: Are We Now Ready for Cost-Utility Analysis in This Patient Population?

Background: Cost-utility analysis (CUA) is frequently used by reimbursement agencies and national advisory bodies to make informed decisions on whether or not to reimburse surgical interventions. Health state preferences (utilities) are a key component in valuing health outcomes in that they are used in calculating quality-adjusted life-years (QALY). Unfortunately, disease-specific HRQoL measures commonly lack the preference weights necessary to produce health-state utility values for use in CUA. A solution to this problem is to map a disease-specific quality-of-life measure to a generic prefe

Opportunities for better value wound care: a multiservice, cross-sectional survey of complex wounds and their care in a UK community population.

BACKGROUND: Complex wounds impose a substantial health economic burden worldwide. As wound care is managed across multiple settings by a range of healthcare professionals with varying levels of expertise, the actual care delivered can vary considerably and result in the underuse of evidence-based interventions, the overuse of interventions supported by limited evidence and low value healthcare. OBJECTIVES: To quantify the number, type and management of complex wounds being treated over a two-week period and to explore variations in care by comparing current practices in wound assessment, prev

Is there value in volume? An assessment of liver transplant practices in the United States since the inception of MELD.

BACKGROUND: Liver transplantation has increased in volume and provides substantial survival benefit. However, there remains a need for value-based assessment of this costly procedure. METHODS: Model for end stage liver disease era adult recipients were identified using United Network for Organ Sharing Standard Transplant Analysis file data (n = 75,988) and compared across time periods (period A: February 2002 to January 2007; B: February 2007 to January 2013; C: February 2013 to January 2019). Liver centers were divided into volume tertiles for each period (small, medium, large). Value for th

Development and Validation of a Natural Language Processing Algorithm to Identify Social Isolation in Older Surgical Patients.

BACKGROUND: Social isolation is increasingly prevalent among older adults and is associated with adverse health outcomes, yet its impact in surgical populations remains poorly defined. Lack of systematic measurement during surgical care has limited large-scale investigation of social isolation and its associations with postoperative outcomes. METHODS: We conducted a prospective validation study of adults ≥ 65 years old admitted to inpatient surgical services at an academic medical center to evaluate the accuracy of a semi-automated, rule-based natural language processing (NLP) algorithm for i

A pharmacist-overseen, artificial intelligence-enabled model for provider-side prior authorization: From burden to opportunity (PAVE-1).

Prior authorization (PA) imposes substantial administrative burdens on clinicians, contributing to burnout, delayed care, and excess health care spending. Artificial intelligence (AI) is emerging as a tool to automate PA tasks in managed care pharmacy, yet concerns persist regarding transparency, bias, and overreliance on autonomous systems. This is particularly true in payer-deployed AI systems that may deny claims without adequate clinical review. This viewpoint proposes a pharmacist-overseen, AI-enabled system designed for provider organizations and health systems as an optimal model, integ

X-Linked Retinitis Pigmentosa (XLRP): An Investigation of the Impact of XLRP on Health-Related Quality of Life of Carers in Great Britain.

INTRODUCTION: X-linked retinitis pigmentosa (XLRP) is one of the most severe forms of retinitis pigmentosa, representing 5-15% of all cases. There is a notable gap in understanding the health-related quality of life (HRQoL) impact on carers of people with XLRP, essential for holistic health economic evaluations. The objective of the study is to estimate health state utilities of informal carers for a person with XLRP which can be used in economic modelling. METHOD: Four carer descriptions or "vignettes" were developed for mild, moderate, severe, and completely blind XLRP patient care. The vig

Comparing different forms of structured follow-up support in the treatment of multiple health risks for chronic disease: a randomized controlled trial protocol.

BACKGROUND: Primary care clinics need effective, low-cost tools to screen for and address multiple health risks - and technology can provide important efficiencies. My Own Health Report (MOHR) is a web-based intervention with 3 core functions: (1) risk flagging; (2) goal-setting for patient-prioritized risks; and (3) providing resources. A prior randomized trial in primary care settings showed that MOHR was feasible and effective. However, we identified two key gaps: a need for a flexible menu of delivery options to adopt and sustain MOHR and a pragmatic way to provide structured follow-up. M

Value-based Clinical Quality Improvement (CQI) for Patients Undergoing Abdominal Wall Reconstruction.

Patients with complex ventral/incisional hernias often undergo an abdominal wall reconstruction (AWR). These operations have a high cost of care and often result in a long hospital stay and high complication rates. Using the principles of clinical quality improvement (CQI), several attempts at process improvement were implemented in one hernia program over a 3-year period. For consecutive cases of patients undergoing abdominal wall reconstruction, process improvement attempts included the use of a long-term resorbable synthetic mesh (TIGR® Resorbable Matrix, Novus Scientific, Uppsala, Sweden)
