Direct laser and laser-thermal irradiation of normal canine coronary arteries: implications for laser delivery methods.

Clinical use of laser recanalization has been mostly limited by arterial perforation. Two modifications of the optical fiber were used to decrease the perforation rate: (1) a guidewire to align the fiber in a coaxial position with the vascular lumen and (2) encapsulation of the optical fiber tip with a metal cap. Fourteen dogs were studied. Argon laser radiation was delivered through optical fibers, 9 with a 1.7-mm metal encapsulated fiber (Laserprobe-PLR,¿ Trimedyne, Santa Ana, CA) and 5 with a 2-mm metal encapsulated fiber with a window at the tip (Spectraprobe-PLR). Three dogs served as con

Surgical reconstruction of iliofemoral veins and the inferior vena cava for nonmalignant occlusive disease.

PURPOSE: Venous reconstructions are rarely performed, and factors affecting long-term results of bypass grafts implanted in the venous system are not well defined. In this report we updated our experience. METHODS: The clinical data of all patients who underwent venous reconstruction for iliofemoral or inferior vena caval (IVC) occlusion due to nonmalignant disease between January 1985 and June 1999 were retrospectively reviewed. Patients were classified, and outcomes were compared according to the guidelines of the Joint Vascular Societies. RESULTS: Forty-two patients, 23 males and 19 femal

Palmar arch identification during evaluation for radial artery harvest.

Noninvasive testing for patency and completeness of the palmar arch prior to radial artery (RA) harvesting for coronary revascularization is a frequently requested test in the Vascular Laboratory (VL). The sonographer and interpreting physician must be aware of palmar arch anatomy and common variations in order to provide accurate information and avoid hand ischemia. The RA is a favored conduit for coronary revascularization due to good early and mid-term patency rates, which are superior to vein grafts.(1) Cardiac surgeons need precise information on adequate ulnar artery (UA) collateral circ

[Treatment of Budd-Chiari syndrome with occlusion of both inferior vena cava and hepatic veins lesions by interposition of graft meso-caval-atrial shunt].

OBJECTIVE: To evaluate the therapeutic efficacy of interposition of graft meso-cavo-atri shunt (MCAS) for the treatment of Budd-Chiari syndrome (BCS) with occlusion of both inferior cava vena (ICV) and hepatic veins (HVs). METHODS: 51 BCS patients with combined occlusion of ICV and HVs, 30 males and 20 females, aged 18-45, underwent MCAS. A 16-18 mm ring-reinforced graft (main graft) was anastomosed firstly to the side of ICV with continuous 5/0 vascular suture, then to the side of the right atrium with continuous 5/0 vascular suture. A 10-12 mm graft (secondary graft) was anastomosed to the

A quantitative approach to lower extremity vein repair.

OBJECTIVE: We prospectively investigated the patency of venous repair in a quantitative fashion with measurement of vein blood flow velocities after lower extremity injuries caused by either military or civilian trauma. MATERIAL AND METHODS: During a 10-year study period (March 1990 to December 2000), surgical intervention was performed after lower extremity vascular injuries in 130 patients. Most of these patients were men (n = 125), with a mean age of 23 +/- 5 years (range, 17 to 44 years). One hundred ten direct venous injuries were identified in 97 patients, involving the common femoral v

Utilizing allogeneic venous grafts to solve the problem of vascular defects and reconstruction in pancreatic surgery: a retrospective cohort study.

BACKGROUND: Extensive vascular invasion or defects after resection significantly limit the feasibility of radical pancreatic surgery, underscoring the value of effective vascular reconstruction techniques. This study evaluates a novel approach to vascular reconstruction using allogeneic venous grafts (AVGs) in pancreaticoduodenectomy and pancreatectomy. METHODS: This retrospective cohort study included 187 patients with biliary or pancreatic malignancies or lesions presenting with various types of venous invasion who underwent pancreatic resection with vascular reconstruction using AVGs. The

Hybrid electrospun rapamycin-loaded small-diameter decellularized vascular grafts effectively inhibit intimal hyperplasia.

For the surgical treatment of coronary artery disease, renal artery stenosis and other peripheral vascular diseases, there is significant demand for small diameter (inner diameter <6 mm) vascular grafts. However, autologous grafts are not always available when the substitute vascular grafts are severely diseased. In our previous work, hybrid small-diameter vascular grafts were successfully fabricated by combining electrospun polycaprolactone (PCL) and decellularized rat aorta (DRA). However, histological assessments of these grafts revealed the development of intimal hyperplasia, indicating po

[Catheter treatment of arterial occlusive disease of supraaortic vessels].

Catheter dilatation can be applied in occlusive vascular disease affecting the proximal and distal subclavian, vertebral, and innominate arteries and common internal and external carotid arteries. The treatment of 102 patients is reported to demonstrate the efficacy of percutaneous transluminal angioplasty in this vascular region with a technical success rate of more than 90% and a patency rate of 75% after a follow-up of more than 3 years. The complication rate is low, transient ischemic attacks having occurred in only 2 of these cases.

Clinical outcomes after vascular reconstruction using synthetic grafts for limb salvage in patients with lower extremity sarcoma: a single-center retrospective experience.

INTRODUCTION: Limb-salvage surgery has become the mainstream approaches for the treatment of sarcoma in the lower extremity. In cases where the sarcoma infiltrates the primary vessel, concurrent resection of the vessels and vascular reconstruction are required to ensure sufficient resection and preservation of limb function. The objective of this study is to assess the clinical outcomes of patients who underwent vascular reconstruction utilizing synthetic grafts for limb salvage, specifically in terms of postoperative complications and limb functional status. METHODS: Between September 2016 a

Experienced operators achieve superior patency and wound complication rates with endoscopic great saphenous vein harvest compared with open harvest in lower extremity bypasses.

OBJECTIVE: Prior studies have suggested improved wound complication rates but decreased primary patency in lower extremity bypasses performed with endoscopic vein harvest (EVH) vs open vein harvest (OVH). We hypothesize that the inferior patency reflects the initial learning curve for EVH and that improved patency can be achieved with experience. METHODS: This was a single-institution review of 113 patients with critical limb ischemia who underwent infrainguinal bypass with a continuous segment of great saphenous vein harvested endoscopically (n = 49) or through a single open incision (n = 64

Evaluation of vascular repair by tissue-engineered human acellular vessels or expanded polytetrafluoroethylene grafts in a porcine model of limb ischemia and reperfusion.

BACKGROUND: This study evaluated performance of a tissue-engineered human acellular vessel (HAV) in a porcine model of acute vascular injury and ischemia. The HAV is an engineered blood vessel consisted of human vascular extracellular matrix proteins. Limb reperfusion and vascular outcomes of the HAV were compared with those from synthetic expanded polytetrafluoroethylene (ePTFE) grafts. METHODS: Thirty-six pigs were randomly assigned to four treatment groups, receiving either the HAV or a PTFE graft following a hind limb ischemia period of either 0 or 6 hours. All grafts were 3-cm-long inter

Laparoscopic pancreaticoduodenectomy with reconstruction of the mesentericoportal vein with the parietal peritoneum and the falciform ligament.

BACKGROUND: With the improvement of the surgical technique of Laparoscopic pancreaticoduodenectomy (LPD), indications will be extended to patients with vascular invasion. With LPD, vascular grafts for reconstruction are more frequently needed because adequate mobilization is not always done and vascular grafts can safely facilitate reconstruction. We describe our experience of reconstruction with the falciform ligament. METHODS: Venous reconstruction is performed after removal of the specimen. The falciform ligament is rapidly harvested within the same surgical field and for any size and used

Patency of direct revascularisation of the hypogastric arteries in patients with aortoiliac occlusive disease.

OBJECTIVES: Various indications for internal iliac artery (IIA) revascularisation have been reported. Revascularisations for gluteal ischaemia and buttock claudication remain controversial and uncommon. The objective of the study was to assess the patency of direct conventional revascularisations (CRs) of the IIA in patients with aortoiliac occlusive disease because few studies have focussed on this specific topic. MATERIALS AND METHODS: The charts of all patients who underwent CR of the IIA, between August 2000 and January 2009, were retrospectively reviewed. We recorded for each patient pre

A multidisciplinary approach for abdominal venous involvement in oncologic resections.

BACKGROUND: An aggressive surgical approach to locally advanced malignancy is being increasingly used in the absence of distant metastatic disease. This includes resection and reconstruction of major venous structures. We investigated the results of using a multidisciplinary surgical approach in these instances. METHODS: The study data were obtained from a university-affiliated hospital from January 1, 2006, to December 31, 2012. All patients who underwent an oncologic resection using a multidisciplinary approach with vascular surgery consultation were included in the analysis. Primary outcom

A Comparative In Vivo Study of Small-Diameter Vascular Grafts Fabricated from Electrospun Tecothane and Tecoflex Polyurethanes.

This study aimed to compare the in vivo performance of small-diameter vascular prostheses (VPs) electrospun from protein-enriched aromatic (Tecothane) and aliphatic (Tecoflex) polyurethanes versus standard expanded polytetrafluoroethylene (ePTFE) grafts in a rat aortic model. The unsatisfying long-term patency and excessive neointimal hyperplasia of clinically used ePTFE small-diameter grafts motivate the search for alternative materials with improved biocompatibility and mechanical compliance. SDVGs were implanted into the infrarenal aorta of Wistar rats for up to 6 months. Evaluations includ

Patency and maturation rates after forearm arteriovenous fistulas: systematic review with meta-analysis.

BACKGROUND: The growing burden of chronic kidney disease has significantly increased the demand for hemodialysis, with reliable vascular access being vital for treatment success. Although the radiocephalic arteriovenous fistula (AVF) remains the first option as a long term hemodialysis access, challenges related to aging patient population and prevalence of comorbidities may compromise maturation and durability. Therefore, this systematic review with meta-analysis aims to determine the patency and maturation rates of radiocephalic AVF, focusing on how different patient characteristics and surg
