With the rising number of vasectomies the demand for refertilization procedures is increasing. Following vasovasostomy patency is achieved in 80 to 90% and fertility in up to 30%. About half of these figures are attained by epididymo-vasostomy. Encouraging experimental studies with an alloplastic sperm-reservoir justify its clinical application. Reasons for failure and for the discrepancy between patency and fertility following refertilization are: surgical technique, autoimmunization, adynamia and hyperactive phagocytosis within the epididymis.

Because of both external and individual factors, nowadays more men seek clarification and consultation about their fertility. This article reviews the current diagnostic work-up of subfertile men and the evidence based treatment options. Also testicular biopsy and cryopreservation of sperm or testicular tissue are addressed. Finally, the issues of vasectomy reversal and aplasia of the vas deferens are discussed. The paper emphasizes practical issues and practicability in office urology.

We report two cases of acute epididymo-orchitis developing 4 to 6 weeks after cryoablation for prostate cancer. One patient required a simple orchiectomy for epididymal abscess; the other responded to treatment with antibiotics. Since the occurrence of these two cases, we routinely perform bilateral vasectomy prior to prostate cryoablation. We suggest that an extended course of prophylactic antibiotics may also be needed in order to avoid this complication.

As far as we know this is the first report on bulking agent injection into intravesical ectopic ejaculatory orifices reported in the English literature. During a follow-up period of 23 months, the child was free of episodes of epididymo-orchitis. Deflux injection in this rare anomaly of intravesical refluxing ducts had prevented irreversible damage to the testes from recurrent EO. Thus, it may be a better option than vasectomy when antibiotic treatment fails.

Duplication of the vas deferens is a rare congenital anomaly that has been encountered during inguinal hernia repair, orchidopexy, varicocelectomy, vasectomy, and radical prostatectomy. Identification of the duplicated vas deferens is crucial intraoperatively to avoid iatrogenic injury to the structure and the risks that come with failure to correctly distinguish the structure. We report a case of duplicated vas deferens and incarcerated urinary bladder during a laparoscopic converted to open left direct inguinal hernia repair.

We studied eight cases of an epididymal lesion resembling vasitis nodosa. Four cases were encountered among a group of patients who underwent epididymectomy for pain following vasectomy. The other four cases occurred in the absence of that history. The lesion probably represents a regenerative effort following rupture of a duct resulting from long-standing obstruction, either locally or at some point distally in the ductal system. Recognition is important to prevent misdiagnosis as a neoplasm.

Scrotal surgery is common in routine veterinary practice. Many techniques are available for commonly performed surgeries, and most can be accomplished easily in private practice and in the field. Procedures are described for surgical and nonsurgical castration, unilateral castration, inguinal herniorrhaphy, cryptorchidectomy, caudal epididymectomy, and vasectomy. Changes in current thinking about the timing of castration and about pain control in cattle surgery likely will alter the way veterinarians perform scrotal surgery in the years to come.
