Vasodilators and primary pulmonary hypertension. Variability of long-term response.

Long-term response to vasodilator therapy was assessed in six patients with PPH. Following an acute trial, each patient was restudied after (1) two months of drug administration, (2) one month of abstinence from therapy, and (3) an additional two months of therapy. Three of six patients had no long-term reduction of PVR after treatment; one showed a progressive increase. Of the three patients whose PVR was still reduced at the end of the second therapy period none showed a return of PVR to baseline during the abstinence phase, which may suggest that long-term reduction of pulmonary vascular to

Vasodilator responses to dopamine in rat perfused mesentery are age-dependent.

1. Dose-dependent vasodilator responses to dopamine, isoprenaline, noradrenaline, 3-isobutyl-1-methylxanthine (IBMX) and sodium nitroprusside were obtained in isolated perfused mesentery preparations, taken from reserpine-treated rats of different ages. The preparations were pretreated with phenoxybenzamine (1 microM) and perfused with physiological salt solution containing cocaine (10 microM), additional KCl (20 mM) and vasopressin (0.1 microM). 2. Vasodilator responses to dopamine were abolished by the dopamine1 (DA1)-selective antagonist SCH 23390 (10 nM) and those to isoprenaline by propra

[Pharmacologic effects of peripheral vasodilators in patients with secondary pulmonary hypertension].

A single dose of capoten (25 mg) and prazosin (I mg) was given to 82 patients with stable pulmonary hypertension and chronic non-specific pulmonary diseases and to 34 patients with the above condition and arterial hypertension. Systolic and diastolic pressure in the system of the pulmonary artery and total pulmonary and total peripheral resistance were decreased; capoten exhibited more pronounced hypotensive effect. Unidirectional changes were revealed in the course treatment. Combined use of capoten, prazosin and euphilline produced the additional hypotensive effect on pulmonary circulation.

Use of the novel cardiotonic and vasodilator agent pimobendan in traumatic shock.

The effect of 4,5-dihydro-6-[2-(4-methoxyphenyl)-1H-benzimidazol-5-yl]-5-methyl- 3(2H)- pyridazinone (pimobendan, UD-CG 115 BS), a novel positive inotropic and vasodilator agent, was studied in a severe model of murine traumatic shock. Noble-Collip drum trauma produced a shock state characterized by a significantly reduced mean arterial blood pressure (MABP), a 5-fold increase in plasma cathepsin D and myocardial depressant factor (MDF) activities, and a survival time of 80 +/- 12 min. Administration of pimobendan (100 micrograms/kg, i.v. bolus) significantly prolonged the survival time to 175

Effect of a new coronary vasodilator, nicorandil, on variant angina pectoris.

The effect of nicorandil, a new coronary vasodilator, was evaluated in 32 patients with variant angina pectoris in a single-blind trial. The study was comprised of a pretreatment period of 2 days with a placebo, a 3-day nicorandil medication period (20 mg/day), and a 2-day posttreatment period with the placebo. Anginal attacks disappeared completely in 24 of the 32 patients. The number of attacks during the pretreatment period, 3.6 +/- 0.4 per day, became significantly reduced to 0.7 +/- 0.2 per day during nicorandil therapy (P less than 0.001) and significantly increased to 1.3 +/- 0.3 per da

The coronary vasodilator effect of neurotensin in the guinea pig isolated heart.

Neurotensin (NT) infusions into isolated, electrically-driven hearts of guinea pigs, elicited concentration-dependent reductions of myocardial perfusion pressure accompanied by proportional increases of myocardial tension. The decrease of myocardial perfusion pressure caused by NT (attributed to the coronary vasodilator effect of NT) was highly dependent on basal (pre-NT infusions) levels of perfusion pressure, being larger at high perfusion pressure (e.g., 75 mmHg) values than at lower ones (e.g., 50 and 25 mmHg). The perfusion pressure-lowering effect on NT was potentiated and inhibited by n

[Classification and principles of the use of vasodilators in the treatment of left ventricular insufficiency].

Vasodilators have been widely used in recent years for the treatment of severe left ventricular failure (LVF). However, amongst these drugs, not all have the same therapeutic effects, but they differ from one another in molecular structures, cellular mechanisms, haemodynamic actions and modes of administration. A three tier classification is proposed with the single objective of facilitating the therapeutic choice: mechanism of action: this property distinguishes the alpha-blockers from the beta-2 sympathomimetics, calcium antagonists, angiotensin converting enzyme inhibitors and prostacyclin

Sodium channel blockers are vasodilator as well as natriuretic and diuretic agents.

Amiloride (100-400 micrograms) injected intra-arterially into the dog forelimb perfused at constant flow produced a prompt but transient dose-dependent decrease in perfusion pressure. Intravenous injection lowered systemic arterial pressure, but effects were small and transient except in doses exceeding 10 mg. We tested 11 analogues of amiloride, 3 other diuretics, and a hypotensive imidazopyrazine for vasodilator activity in the dog forelimb and found one analogue, 6-iodo-amiloride, with twice the activity of amiloride. Intravenous injection of 3 mg of 6-iodo-amiloride promptly decreased syst

Action of renal vasodilators in dogs following acute biliary obstruction.

The renal vasodilator properties of six endogenous substances were tested before and 4 hr after ligation of the common bile duct. Two substances, acetylcholine and a glucocorticoid, retained their vasodilator properties at a fixed dose following acute biliary obstruction. Dopamine was still able to increase glomerular filtration rate and renal blood flow, but demonstrated an attenuated response. Several other agents, glucagon, glycine, and bradykinin, lost their renal vasodilator actions at the dosages employed. For these latter three compounds, control studies using sham-obstructed dogs and i

Hemodynamic effects of vasodilators and long-term response in heart failure.

Hemodynamic responses to vasodilators are commonly assessed when starting long-term vasodilator treatment in patients with chronic left ventricular failure, although the relation between short- and long-term responses is not established. Thus, short- and long-term hemodynamic responses to placebo and vasodilators (isosorbide dinitrate, minoxidil and enalapril or captopril) were measured and long-term clinical efficacy was assessed by changes in exercise capacity after 1 to 5 months of vasodilator administration (plus digitalis and diuretic agents) in 46 patients with New York Heart Association

Antihypertensive effects of a novel peripheral vasodilator, Ro 12-4713.

The effects of Ro 12-4713, a vasodilator, on blood pressure and heart rate, were tested in 13 ambulant patients with mild to severe hypertension. The patients received Ro 12-4713 in total oral doses ranging from 1.5 to 4 mg/kg over a 3-5 day period. Standard laboratory tests were done 24 h before and after the study. Standing and supine blood pressure and heart rate were determined before, and every hour for 6-8 h after drug administration. ECGs were recorded before, during and at the end of each study day. Ro 12-4713 reduced the pretreatment standing and supine-blood pressure from 213 +/- 34/

[Prazosine: a new vasodilator used for treatment of hypertension (author's transl)].

Prazosine, a derivative of quinazoline, acts by relaxing the smooth vascular muscles and blocking postsynaptic alpha-adrenoreceptors. A special protocol was used to treat arterial hypertension in 21 subjects. A small dose (0.5 mg) was given the first day to avoid orthostatic hypotension, then 0.5 mg x 3 on days 2, 3 and 4, followed by 1 mg x 3 on subsequent days. Dosage can be progressively increased up to 30 mg/day. During the first 36 days of treatment, prazosine was given alone. A significant drop in systolic and diastolic arterial pressure was observed in the reclining subject. The effect
