Endoscopic management of eroded prosthesis in vertical banded gastroplasty patients.

INTRODUCTION: One of the major complications of applying a prosthetic device to facilitate gastric partitioning has been intraluminal erosion of the prosthesis. Removing an eroded gastric band is fraught with difficulty secondary to the extensive inflammatory response around the proximal stomach and left lobe of the liver. As a result, bariatric clinicians have attempted to apply endoscopic technology to facilitate removal of eroded gastric prostheses. Our study reports on our experience of applying endoscopic scissor transection to remove eroded gastric prostheses in a large tertiary-care med

Residual muscle activity associated with phantom limb movements after transhumeral amputation without surgical reinnervation: a preliminary ultrasound study.

Transhumeral amputations often result in significant functional limitations. Multiarticulated myoelectric prostheses for individuals with transhumeral amputations are often unreliable because of limitations in signal quality and challenges with accurately detecting motion intent from the available musculature in the residual limb, leading to high prosthesis rejection rates. Surgical approaches like targeted muscle reinnervation have been proposed to mitigate these challenges by biologically amplifying motor signals but are not suitable for all individuals. There is intriguing evidence that dis

Botulinum toxin and hyperhidrosis of the amputation stump in war amputees.

INTRODUCTION: Stump hyperhidrosis is a common condition after lower limb amputation. It affects the prosthesis use, and the quality of life of patients. Several case reports tried to prove benefit of using Botulinum toxin in its treatment. AIM: This study was to conduct a larger workforce clinical trial and to demonstrate benefits of botulinum toxin injection in the treatment of stump hyperhidrosis. METHODS: A prospective study was conducted. War amputees who complained of annoying excessive sweating of the stump were included. They received intradermal injection of botulinum toxin A in the

Socket size adjustments in people with transtibial amputation: Effects on residual limb fluid volume and limb-socket distance.

BACKGROUND: Small intermittent adjustments of socket size using adjustable sockets may be a means for people with transtibial amputation to better maintain residual limb fluid volume and limb position while using a prosthesis. METHODS: Socket size, limb fluid volume, and distance from the limb to the socket, termed "sensed distance," were recorded while participants with transtibial amputation walked on a treadmill wearing a motor-driven, cabled-panel, adjustable socket. Researchers made frequent socket size adjustments using a mobile phone app to identify participants' acceptable socket size

Surgical closure of left ventricular outflow tract after left ventricular assist device implantation in patients with aortic valve pathology.

BACKGROUND: In patients with refractory heart failure, aortic valve (AV) insufficiency or the presence of a prosthetic AV has been considered a relative contraindication to left ventricular assist device (LVAD) therapy. Nevertheless, we have successfully implanted LVADs in 5 patients with native AV insufficiency or an AV prosthesis by closing the left ventricular outflow tract (LVOT). METHODS: The method of LVOT closure was tailored to the patients' differing circumstances. Patient 1 had a regurgitant tri-leaflet native AV. Patient 2 had undergone previous aortic root reconstruction with a po

Immersive augmented reality system for the training of pattern classification control with a myoelectric prosthesis.

BACKGROUND: Hand amputation can have a truly debilitating impact on the life of the affected person. A multifunctional myoelectric prosthesis controlled using pattern classification can be used to restore some of the lost motor abilities. However, learning to control an advanced prosthesis can be a challenging task, but virtual and augmented reality (AR) provide means to create an engaging and motivating training. METHODS: In this study, we present a novel training framework that integrates virtual elements within a real scene (AR) while allowing the view from the first-person perspective. Th

Virtual reality-based action observation facilitates the acquisition of body-powered prosthetic control skills.

BACKGROUND: Regular body-powered (BP) prosthesis training facilitates the acquisition of skills through repeated practice but requires adequate time and motivation. Therefore, auxiliary tools such as indirect training may improve the training experience and speed of skill acquisition. In this study, we examined the effects of action observation (AO) using virtual reality (VR) as an auxiliary tool. We used two modalities during AO: three-dimensional (3D) VR and two-dimensional (2D) computer tablet devices (Tablet). Each modality was tested from first- and third-person perspectives. METHODS: We

Bilateral Temporomandibular Joint Ankylosis Treated With Combined Intraoral and Extraoral Gap Arthroplasty: A Case Report.

Temporomandibular joint (TMJ) ankylosis causes severe limitation of mouth opening and functional impairment. Surgical management requires careful selection of approach and interpositional material to prevent recurrence and preserve function. We report a case of post-traumatic bilateral TMJ ankylosis treated by gap arthroplasty using different surgical approaches on each side. Because the patient had a right ocular prosthesis, preservation of the left facial nerve and visual function was considered essential. Therefore, the right side was managed using an extraoral approach with a temporalis mu

Intrascleral anchoring knot on the double suture without peritomy for fixation of intraocular lens.

A new minimally invasive technique for universal fixation of any posterior chamber intraocular lens (IOL), iris prosthesis or capsular tension device (CTD), both for rescuing in case of dislocation and for secondary implantation, is described. It uses intrascleral anchoring knot on the reinforced double 9-0 polypropylene suture without conjunctival opening or scleral dissection. The technique was applied for scleral fixation of the S-shaped monoblock acrylic IOL dislocated into the vitreous cavity in 1 eye, and decentered IOL-CTD-capsular bag complexes in 3 eyes. In none of the cases, complica
