Delayed-onset vocal cord palsy after thyroidectomy occurring despite normal initial post-operative endoscopy.

BACKGROUND: Recurrent laryngeal nerve (RLN) injury is a known complication of thyroidectomy. Most cases manifest immediately and are promptly recognized. Rarely, some patients experience delayed-onset vocal cord palsy. This can occur despite normal findings on intra-operative nerve monitoring and initial post-operative endoscopy. This can cause considerable distress for patients, and its incidence and prognosis should be recognized. METHODOLOGY: We report seven patients experiencing delayed-onset RLN palsy (RLNP) after thyroidectomy. All had normal findings on pre-operative flexible nasoendos

Comparative analysis of clinical profile: Chronic cough vs paradoxical vocal fold motion.

BACKGROUND: Ongoing contention surrounding typical demographic and clinical attributes of chronic cough (CC) and paradoxical vocal fold motion (PVFM) impedes timely diagnosis and optimum patient care. Designed to reduce preventable patient morbidity through improved recognition and differentiation, the current study aimed to determine representative clinical profiles for CC and PVFM, with identification of distinctive attributes from the general population and risk factors associated with each diagnosis. METHODS: Self-reported medical questionnaires, demographic and lifestyle attributes of CC

Using SMS Texting for Adverse Drug Reaction Surveillance: Evaluation of New Medication Prescriptions in Primary Care.

BACKGROUND: Adverse drug reactions (ADRs) and their timely recognition are major patient safety concerns. Earlier detection of ADRs can improve outcomes through more timely medication management thereby minimizing harm. The research team has conducted studies to proactively monitor patients prescribed medications to identify potential ADRs, through pharmacist phone calls, interactive voice response (IVR) calls, and patient portal messaging. Because many patients regularly use SMS texting, the authors sought to leverage this technology to improve convenience and response rates. METHODS: The te

Primary Care Recognition of Rabson-Mendenhall Syndrome Despite Absence of Classical Diabetic Symptoms.

BACKGROUND Rabson-Mendenhall syndrome (RMS) is an extremely rare autosomal recessive disorder caused by pathogenic variants in the insulin receptor gene, leading to severe insulin resistance and compensatory hyperinsulinemia. Classical features include acanthosis nigricans, non-obese or underweight body habitus, hirsutism, dental abnormalities, dysmorphic features, and variable growth abnormalities. Early recognition may be difficult when the initial presentation is dominated by non-specific symptoms rather than classical metabolic complaints. CASE REPORT A 10-year-old Saudi girl presented to

Machine Learning for Differentiating Essential Tremor: A Scoping Review.

BackgroundEssential tremor (ET) is the most common movement disorder, affecting ~6% of adults over 65 [1]. Differentiating ET from other tremors remains clinically challenging due to overlapping features and variable presentation. Artificial intelligence (AI), particularly machine learning (ML), has emerged a potential tool to support neurologists by enhancing pattern recognition and complementing traditional assessments in complex cases. This is the first scoping review examining ML's potential role in distinguishing essential tremor from other tremor types.MethodsA systematic, scoping search

Speech and language therapists' perceptions of corporate practice in South Africa.

BACKGROUND: Corporate speech and language therapy (CSALT) is an emerging field that extends the role of speech and language therapists (SALTs) into corporate environments, enhancing workplace communication and interpersonal skills. While internationally recognised, CSALT remains underexplored in South Africa, where linguistic and cultural diversity present challenges and opportunities for its integration. OBJECTIVES: This study aimed to explore South African SALTs' perceptions of CSALT. METHOD: A qualitative research approach was employed, grounded in a constructivist paradigm. Semi-structur

Collet-Sicard syndrome due to cervical artery dissection disclosed by high-resolution magnetic resonance imaging.

BACKGROUND AND PURPOSE: Cervical artery dissection (CAD) represents a leading cause of unilateral lower cranial nerve IX-XII palsy, known as Collet-Sicard syndrome (CSS). High-resolution magnetic resonance imaging (HR-MRI) is widely used in the evaluation of patients with CAD, providing information regarding vessel wall abnormalities and intraluminal thrombus. METHODS: We present a patient with palsy of multiple lower cranial nerves in the context of CSS, attributed to unilateral spontaneous internal carotid artery dissection. RESULTS: We describe a 68-year-old man with unremarkable previous

A Functional MRI Paradigm for Efficient Mapping of Memory Encoding Across Sensory Conditions.

We introduce a new and time-efficient memory-encoding paradigm for functional magnetic resonance imaging (fMRI). This paradigm is optimized for mapping multiple contrasts using a mixed design, using auditory (environmental/vocal) and visual (scene/face) stimuli. We demonstrate that the paradigm evokes robust neuronal activity in typical sensory and memory networks. We were able to detect auditory and visual sensory-specific encoding activities in auditory and visual cortices. Also, we detected stimulus-selective activation in environmental-, voice-, scene-, and face-selective brain regions (pa

Hearing loss in older adults.

Hearing loss affects approximately one-third of adults 61 to 70 years of age and more than 80 percent of those older than 85 years. Men usually experience greater hearing loss and have earlier onset compared with women. The most common type is age-related hearing loss; however, many conditions can interfere with the conduction of sound vibrations to the inner ear and their conversion to electrical impulses for conduction to the brain. Screening for hearing loss is recommended in adults older than 50 to 60 years. Office screening tests include the whispered voice test and audioscopy. Older pati

Advanced practice nursing, critical care, and Spain: A point of view.

Advanced Practice Nursing (APN) continues to gain recognition. Despite the guidelines published by the International Council of Nurses in 2020, there is still some confusion about this concept. In general, APN is used in three different and not necessarily compatible ways: as an umbrella term that regulates four different roles, as a level of practice, or as a role itself. Specialization in nursing does not always imply advanced practice, although both concepts are not mutually exclusive. The "Acute Care Nurse Practitioners" in the United States can conduct physical examinations and medical hi

Music perception in acquired prosopagnosia.

BACKGROUND: Acquired prosopagnosia is often associated with other deficits such as dyschromatopsia and topographagnosia, from damage to adjacent perceptual networks. A recent study showed that some subjects with developmental prosopagnosia also have congenital amusia, but problems with music perception have not been described with the acquired variant. OBJECTIVE: Our goal was to determine if music perception was also impaired in subjects with acquired prosopagnosia, and if so, its anatomic correlate. METHOD: We studied eight subjects with acquired prosopagnosia, all of whom had extensive neu

A meta-analysis of social cognitive deficits in schizophrenia: Does world region matter?

Individuals with schizophrenia (SCZs) demonstrate social cognitive (SC) deficits across a variety of social cognitive tasks: affect recognition, social perception, Theory-of-Mind (ToM), and attributional style. These tasks all use socially-relevant words, social scenarios, clothing, faces and voices that may make their application to different cultural contexts problematic. However a paucity of research has investigated the cross-cultural validity of SC measures. We meta-analyzed the literature investigating differences in social cognitive skills SCZs and matched healthy controls (NCs) across

Reduced neural sensitivity to emotional faces and voices in preterm five-year-olds: a comparative study.

BACKGROUND: Accurate emotion processing is crucial for social development. Preterm birth is associated with neurodevelopmental alterations and increased vulnerability to subtle socio-emotional difficulties, often described within the preterm behavioral phenotype. Previous studies have reported impaired emotion recognition in preterm populations, but have largely relied on behavioral tasks and primarily focused on facial expressions. Because explicit emotion recognition is still developing in early childhood, objective neural markers of implicit emotion discrimination may be particularly inform

[THE STUDY OF INVOLVEMENT IN THE EDUCATIONAL PROCESS KNOWLEDGE FATIGUE AND THE LEVEL OF EMOTIONAL INTELLIGENCE OF STUDENTS USING NEURAL NETWORKS].

The relevance of the study is as follows. Traditional methods of pedagogy do not allow objective and continuous monitoring of the emotional state of each student. The teacher usually relies on subjective observations - facial expressions behavior in a group - which may be inaccurate. At the same time modern technologies provide new opportunities: cameras and sensors backed by AI algorithms are able to automatically recognize facial expressions voice intonations and other signs of an emotional state. There are already commercial solutions for facial emotion recognition which have been used in m

Burkitt Lymphoma Presentation with Oropharyngeal Mass of Tonsillar Fossa: A Case Report.

INTRODUCTION: Burkitt lymphoma is a highly aggressive subtype of non-Hodgkin lymphoma with varied clinical presentation, including in some cases involvement of the intraoral cavity. Early recognition of this malignancy is critical, as it typically responds well to prompt and intensive treatment. In this case report, we present a rare manifestation of Burkitt lymphoma presenting as an oropharyngeal mass. CASE REPORT: An eight-year-old male presented with tonsillar swelling and new-onset oral bleeding. A month earlier, he had been seen in the emergency department (ED) for similar swelling follo

Perceptions of adolescent pregnancy in the rural context and the Colombian armed conflict: a qualitative approach based on social determination of health.

BACKGROUND: Adolescent pregnancy in rural areas is a persistent health problem that has still not properly been understood. Studies with qualitative perspectives that address this phenomenon as a complex social process, which involves the recognition of the voices of the actors involved and the analysis of the specific context in which it takes place, are limited. OBJECTIVE: This research explored the perceptions of young people and other social actors (municipalities of Palmira and El Cerrito in Colombia) of the social forces and dimensions of the social determination of adolescent pregnancy
