The purpose of this study was to assess the incidence of medical illness among members of trekking groups in the Nepal Himalaya. The design was a cohort study using interview and clinical examination by a single physician. The setting was the Manaslu area in the central Nepal Himalaya along a 22-day trekking route with elevations ranging from 487 m to 5100 m. We report a case of a trekker who ascended to Tilicho Lake (4919 m) from Kathmandu (1400 m) in 3 days. On day 3, he descended to Shree Kharka (4045 m). The following day, he trekked to Yak Kharka (4025 m) where, although he had no symptoms, he measured his own oxygen saturation (SpO2) at 72% in ambient air. Wolf spiders are widespread globally, but there are limited reports of wolf spider envenomation in the United States. Wolf spider bites are generally considered to be minimally consequential, with most reported bites causing only mild local symptoms.
