The expanding domain of implantable hearing devices: an update on current status in the United States.

Several implantable devices are now approved in the United States for rehabilitation of conductive, mixed, and sensorineural hearing loss. This paper will review current rehabilitative options including middle ear implants, used for mild to severe sloping sensorineural loss, and bone-anchored hearing aids, for conductive or mixed hearing loss. The indications for these devices differ, as do the bases for exclusion. Audiologic evaluation parameters will be reviewed as a guideline to selection, and as a baseline to determine potential rehabilitative impact. We will also describe emerging applica

The role of stability measurements of the Baha® system in children.

OBJECTIVE: Percutaneous bone conduction hearing aids are an established treatment for selected children unable to use conventional hearing aids. Currently in children, loading the implant is delayed for 3-6 months following fixture placement, due to concerns regarding bone quality, bone thickness and subsequent implant stability. Traditionally, such concerns led to children undergoing 2-stage Baha(®) surgery, with a second operation to attach the abutment after 3-6 months. Bone conduction implant stability can be objectively measured using resonance-frequency analysis (RFA) to generate Implant

Sound Level Changes the Auditory Cortical Activation Detected with Functional Near-Infrared Spectroscopy.

BACKGROUND: Functional near-infrared spectroscopy (fNIRS) is a viable non-invasive technique for functional neuroimaging in the cochlear implant (CI) population; however, the effects of acoustic stimulus features on the fNIRS signal have not been thoroughly examined. This study examined the effect of stimulus level on fNIRS responses in adults with normal hearing or bilateral CIs. We hypothesized that fNIRS responses would correlate with both stimulus level and subjective loudness ratings, but that the correlation would be weaker with CIs due to the compression of acoustic input to electric ou

Confirmatory Auditory Brainstem Responses Testing Results in Discordant Outcomes: Implications for Timely Care.

OBJECTIVES: The purpose of this study is to evaluate for discrepancies in diagnostic auditory brainstem responses (ABR) between Children's National Hospital (CNH), a pediatric medical center, and outside facilities (OSF) that referred patients to CNH for confirmatory evaluation. Such discrepancies impact early hearing detection and intervention (EHDI) timelines. METHODS: A retrospective chart review was conducted from an internal database of patients who underwent diagnostic ABR from 2017 to 2021. Only patients with ABR results from both CNH and OSF were analyzed. Demographic data, external a

Sensitivity to Pulse Rate and Amplitude Modulation in an Animal Model of the Auditory Brainstem Implant (ABI).

The auditory brainstem implant (ABI) is an auditory neuroprosthesis that provides hearing by electrically stimulating the cochlear nucleus (CN) of the brainstem. Our previous study (McInturff et al., 2022) showed that single-pulse stimulation of the dorsal (D)CN subdivision with low levels of current evokes responses that have early latencies, different than the late response patterns observed from stimulation of the ventral (V)CN. How these differing responses encode more complex stimuli, such as pulse trains and amplitude modulated (AM) pulses, has not been explored. Here, we compare respons

Attentional modulation of auditory cortical activity in individuals with single-sided deafness.

Persons with single-sided deafness (SSD) typically complain about the impaired ability to locate sounds and to understand speech within background noise. However, the findings from previous studies suggest that paying attention to sounds could mitigate the degraded spatial and speech-in-noise perception. In the present study, we characterize the pattern of cortical activation depending on the side of deafness, and attentional modulation of neural responses to determine if it can assist better sound processing in people with SSD. For the active listening condition, adult subjects with SSD perfo

Understanding Listening-Related Fatigue: Perspectives of Adults with Hearing Loss.

OBJECTIVE: Adults with hearing loss (AHL) often report feeling fatigued after being in situations that require prolonged listening, an experience referred to as listening-related fatigue. We conducted focus groups to identify key domains and constructs of listening-related fatigue. Our goal was to create a theoretical framework for understanding listening-related fatigue that could guide the development of a reliable and valid assessment tool. DESIGN: Eight focus group discussions were conducted using a moderator's guide. Discussions were recorded, transcribed, coded, and analysed to identify

First experience in Italy with a new transcutaneous bone conduction implant.

Since 2011, transcutaneous bone-anchored auditory implants have been an alternative to the classic percutaneous implant (Baha) for bilateral conductive/mixed hearing loss that cannot be corrected by surgery. Recently, a new transcutaneous device has been approved for clinical use. Its internal component is made of the classic titanium Baha fixture, coupled to a 27 mm diameter subcutaneous circular magnet. The external component includes a second circular magnet 29 mm in diameter and a digital sound processor. To date, there are no reports describing the results of the application of this devic

Peripheral auditory processing in normal and abnormal ears: physiological considerations for attempts to compensate for auditory deficits by acoustic and electrical prostheses.

The processing of complex acoustic signals, such as speech, by the normal peripheral auditory system is described in terms of the properties of individual cochlear nerve fibres and the effects of lateral inhibition in the cochlear nucleus. The implications of the peripheral filtering of signals is indicated in psychophysical terms. In cochlear pathology, these filtering characteristics deteriorate, and the implications arising from the consequent deterioration in auditory frequency selectivity are outlined for the coding of loudness and for the analysis of speech. These considerations suggest

Design of Piezoelectric Dual-Bandwidth Accelerometers for Completely Implantable Auditory Prostheses.

For the last 20 years, researchers have developed accelerometers to function as ossicular vibration sensors in order to eliminate the external components of hearing aid and cochlear implant systems. To date, no accelerometer has met all of the stringent performance requirements necessary to function in this capacity. In this work, we present an accelerometer design with an equivalent noise floor less than 20 phon equal-loudness-level over a 0.1-8 kHz bandwidth in a package small enough to be implanted in the middle ear. Our approach uses a dual-bandwidth (two sensing elements) microelectromech

Utility of 3D printed temporal bones in pre-surgical planning for complex BoneBridge cases.

With the advent of single-sided hearing loss increasingly being treated with cochlear implantation, bone conduction implants are reserved for cases of conductive and mixed hearing loss with greater complexity. The BoneBridge (BB, MED-EL, Innsbruck, Austria) is an active fully implantable device with no attenuation of sound energy through soft tissue. However, the floating mass transducer (FMT) part of the device is very bulky, which limits insertion in complicated ears. In this study, 3D printed temporal bones of patients were used to study its utility in preoperative planning on complicated c…

An overview of different systems: the bone-anchored hearing aid.

In the past 30 years, a large amount of clinical and audiological research on bone conduction hearing devices has been performed. In this review, we give a brief history of the developments in indications, surgical techniques and sound processors with respect to implantable bone conduction devices like the bone-anchored hearing aid or Baha. Starting with the use of Baha in patients with bilateral conductive or mixed hearing loss (HL), the indications for such devices have been extended to patients with unilateral HL, children and moderate mentally retarded patients. Bilateral fitting has been
