Efficacy and Safety of Bowel Cleansing with Mini S-Oral Sulfate Tablet versus the Conventional Oral Sulfate Tablet: A Prospective, Randomized, Investigator-Blinded, Multicenter, Noninferior, Phase 3 Trial.

BACKGROUND/AIMS: Conventional oral sulfate tablets (OSTs) and mini-OSTs have gained popularity for bowel preparation in South Korea. This study aimed to evaluate the efficacy, tolerability, and safety of mini S-OSTs, which have fewer tablets and include simethicone compared to the mini-OSTs. METHODS: This was a prospective, randomized, investigator-blinded, multicenter, and noninferior phase 3 trial conducted between August 2023 and January 2024. The efficacy, safety, and tolerability were compared among a mini S-OST split dose group, mini S-OST non-split dose group and conventional OST group

Subhepatic Appendicitis: A Systematic Review of Clinical Presentation, Diagnostic Challenges, and Surgical Management.

Subhepatic appendicitis (SHA) is a rare positional variant of appendicitis that frequently mimics hepatobiliary disease, leading to diagnostic confusion and delay. As published evidence is limited to case reports and small series, this systematic review aims to synthesize the clinical presentation, diagnostic pathways, management, and outcomes of reported SHA cases. Following an International Prospective Register of Systematic Reviews (PROSPERO)-registered protocol, PubMed, Scopus, and Google Scholar were searched up to 6 May 2024, with additional eligible reports identified through manual sea

Invasive Gastrointestinal Mucormycosis Presenting as Neutropenic Enterocolitis: A Case Report.

BACKGROUND Gastrointestinal mucormycosis is an underrecognized and underreported fungal infection with a high mortality rate. Diagnosis is often confounded by a non-specific constellation of signs and symptoms. We present a case of neutropenic colitis and ileocecal perforation secondary to gastrointestinal mucormycosis. CASE REPORT The patient was a 19-year-old man recently diagnosed with B-cell acute lymphoblastic leukemia, on active chemotherapy. He presented with abdominal pain and tenderness, and a contrast-enhanced CT revealed neutropenic colitis. Despite broad-spectrum antibiotics, his c

Short- and long-term outcomes of laparoscopic right hemicolectomy with D3 resection for right colon cancer in elderly patients.

BACKGROUND: Laparoscopic right hemicolectomy (RHC) with D3 resection, similar to complete mesocolic excision, is an oncologically satisfying procedure; however, it remains controversial in elderly patients. There are no reports of the procedure for tumors fed by middle colic vessels because it is a difficult procedure. We evaluated the feasibility and oncological outcomes of the procedure in elderly patients. METHODS: We retrospectively evaluated 336 consecutive patients undergoing laparoscopic right hemicolectomy with D3 resection for Stage I-III ascending and transverse colon cancer between

Magnesium biology.

Magnesium (Mg2+) is essential for energy metabolism, muscle contraction and neurotransmission. As part of the Mg-ATP complex, it is involved in over 600 enzymatic reactions. Serum Mg2+ levels are tightly regulated between 0.7 and 1.1 mmol/L by interplay of intestinal absorption and renal excretion. In the small intestine, Mg2+ is absorbed paracellularly via claudin-2 and -12. In the colon, transcellular absorption of Mg2+ is facilitated by TRPM6/7 and CNNM4. In the kidney, the proximal tubule reabsorbs only 20% of the filtered Mg2+. The majority of the filtered Mg2+ is reabsorbed in the thick

Malignant Small Bowel Obstruction from Hernia Mesh Invasion by Jejunal Adenocarcinoma: A Report of a Rare Case.

BACKGROUND Small bowel obstructions (SBO) are common and can be caused by various pathologies including intra-abdominal adhesions and hernias. Less frequently, these obstructions are caused by malignancy. The following article will review the etiology and treatment of SBOs, discuss complications of hernia repair with mesh, and examine if there is an association between mesh and cancer. CASE REPORT We present the case of a man who was over 89 years old who presented with an SBO that failed non-operative management. He previously had bilateral inguinal hernia repairs with mesh and pelvic radiati

Evaluation of the short-term efficacy of local analgesic (lidocaine) and opioid analgesic (sufentanil) on patients with centrally mediated abdominal pain syndrome: a randomized controlled trial.

BACKGROUND: Centrally mediated abdominal pain syndrome (CAPS) is characterized by continuous or frequently recurring abdominal pain and can result in functional loss across several life domains. The efficacy of the present management methods has not been established yet. We performed a prospective randomized controlled trial to explore the short-term efficacy of local analgesic (lidocaine) and opioid analgesic (sufentanil) in patients with CAPS. METHODS: We consecutively enrolled 130 patients who met the Rome IV CAPS criteria and divided them into the sufentanil + lidocaine (S + L) group and

Age-Related Colonic Mucosal Microbiome Community Shifts in Monkeys.

Age-related changes in gut microbiome impact host health. The interactive relationship between the microbiome and physiological systems in an aged body system remains to be clearly defined, particularly in the context of inflammation. Therefore, we aimed to evaluate systemic inflammation, microbial translocation (MT), and differences between fecal and mucosal microbiomes. Ascending colon mucosal biopsies, fecal samples, and blood samples from healthy young and old female vervet monkeys were collected for 16S rRNA gene sequencing, MT, and cytokine analyses, respectively. To demonstrate microbia

Laparoscopic distal pancreatectomy for intraductal papillary mucinous neoplasm-associated pancreatic cancer: A case report.

INTRODUCTION AND IMPORTANCE: Intraductal papillary mucinous neoplasm (IPMN) of the pancreas is often found incidentally during examination for other diseases. In addition to the risk of malignant transformation, patients with IPMN are at risk of developing pancreatic cancer. We report a case of pancreatic tail cancer that developed separately from a preexisting IPMN after minimally invasive esophagectomy for cancer of the esophagogastric junction and was resected successfully by laparoscopic distal pancreatectomy. CASE PRESENTATION: A 72-year-old man underwent thoracoscopic and laparoscopic e

Cancer in adult patients with inflammatory arthritis is associated with high ascending aortic stiffness and left ventricular hypertrophy and diastolic dysfunction.

Inflammatory arthritis, including rheumatoid arthritis (RA), psoriatic arthritis (PsA), and ankylosing spondylitis (AS), are associated with both cancer and cardiovascular (CV) adverse events. Cancer and CV abnormalities have coincident etiologic and pathophysiologic pathways in RA/PsA/AS patients. However, a comprehensive evaluation of CV system has never been performed in these patients in relation to the presence of cancer. This study was designed to assess the possible relationships between CV abnormalities and cancer among RA/PsA/AS patients. Between March 2014 and March 2015, 414 patient

Association between KRAS G13D mutations and anastomotic recurrence in colorectal cancer: Two case reports.

RATIONALE: The prevalence of anastomotic recurrence (AR) in colorectal cancer (CRC) after resection of the primary tumor (PT) is 5% to 14%. However, no association has been observed between specific somatic genetic alterations and AR. Such associations may shed light on the mechanism of AR. PATIENT CONCERNS: We experienced 2 patients with AR of CRC. The first patient was a 42-year-old woman who underwent resection of an AR of rectal cancer 2 times within 19 months after resection of the PT. The second patient was a 77-year-old woman who underwent resection of an AR of ascending colon cancer t

Gastrointestinal digestion of food-use silver nanoparticles in the dynamic SIMulator of the GastroIntestinal tract (simgi®). Impact on human gut microbiota.

The increasing use of silver nanoparticles (AgNPs) in consumer products has led to concern about their impact on human health. This paper aims to provide new scientific evidence about the modifications and potential effects of AgNPs with food applications during their passage through the digestive tract. For that, two types of AgNPs [solid polyethylene glycol-stabilised silver nanoparticles (PEG-AgNPs 20) and liquid glutathione-stabilised silver nanoparticles (GSH-AgNPs)] were initially subjected to gut-microbial digestion simulation in an in vitro static model. Based on these experiments, dig

Assessment of colorectal length using the electromagnetic capsule tracking system: a comparative validation study in healthy subjects.

AIM: We aimed to determine colorectal length with the 3D-Transit system by describing a 'centreline' of capsule movement and comparing it with known anatomy, as determined by magnetic resonance imaging (MRI). Further, we aimed to test the day-to-day variation of colorectal length assessed with the system. METHOD: The 3D-Transit system consists of electromagnetic capsules that can be tracked as they traverse the gastrointestinal tract. Twenty-five healthy subjects were examined with both 3D-Transit and MRI. Another 21 healthy subjects were examined with 3D-Transit on two consecutive days. RES
