Irrational Beliefs and Personality Traits as Psychological Mechanisms Underlying the Adolescents' Extremist Mind-Set.

The tripartite model of militant extremist mind-set proposed by Stankov et al. (2010b) includes three components: War (justification of violent acts as war); God (violence is seeing extremist acts as moral because they are done in the name of God or Allah); and West (violence against Western countries is justified because they are perceived as evil and/or immoral). There is a lack of conceptual framework regarding psychological mechanism that underlie radicalization and extremism, and there is little evidence regarding risk factors for radicalization in the scientific literature. Care of the mother-infant dyad: a novel approach to conducting and evaluating neonatal resuscitation simulation training in Bihar, India.

BACKGROUND: As the global under-five mortality rate declines, an increasing percentage is attributable to early neonatal mortality. A quarter of early neonatal deaths are due to perinatal asphyxia. However, neonatal resuscitation (NR) simulation training in low-resource settings, where the majority of neonatal deaths occur, has achieved variable success. In Bihar, India, the poorest region in South Asia, there is tremendous need for a new approach to reducing neonatal morality. METHODS: This analysis aims to assess the impact of a novel in-situ simulation training program, developed by PRONTO

Fertility care interventions should be provided as the first line options for HIV+ serodiscordant couples who desire children in settings with affordable access to care, regardless of their fertility status.

INTRODUCTION: With increasing life expectancy, couples with at least one partner afflicted with HIV are more commonly pursuing the opportunity to have biologic offspring. Currently, there are no universally accepted recommendations regarding first line reproductive treatments for HIV serodiscordant couples lacking a history of infertility. We strongly believe that fertility care intervention should be the first line treatment, when affordably accessible, over natural conception for HIV serodiscordant couples to achieve pregnancy in a safe and efficacious manner. DISCUSSION: In the era of high

Why do marital partners of people living with HIV not test for HIV? A qualitative study in Lusaka, Zambia.

BACKGROUND: Knowledge of HIV status is crucial for HIV prevention and management in marital relationships. Yet some marital partners of people living with HIV decline HIV testing despite knowing the HIV-positive status of their partners. To date, little research has explored the reasons for this. METHODS: An exploratory qualitative study was undertaken in Lusaka, Zambia, between March 2010 and September 2011, nested within a larger ethnographic study. In-depth interviews were held with individuals who knew the HIV-positive status of their marital partners but never sought HIV testing (n = 30)

From disease to desire, pleasure to the pill: A qualitative study of adolescent learning about sexual health and sexuality in Chile.

BACKGROUND: Sexual and reproductive rights include access to accurate and appropriate information in order to make informed decisions. In the current age of media globalization and Internet, adolescents are exposed to information about sexual health and sexuality from a myriad of sources. The objective of this study was to explore sources of information and adolescent learning about sexual health and sexuality in Santiago, Chile. METHODS: Data collection included four focus group discussions with a total of 24 adolescents 18-19 years old, 20 semi-structured interviews with adolescents 16-19 y

Should general surgery residents be taught laparoscopic pyloromyotomies? An ethical perspective.

OBJECTIVES: The authors examine the ethical implications of teaching general surgery residents laparoscopic pyloromyotomy. DESIGN/PARTICIPANTS: Using the authors' previously presented ethical framework, and examining survey data of pediatric surgeons in the United States and Canada, a rigorous ethical argument is constructed to examine the question: should general surgery residents be taught laparoscopic pyloromyotomies? RESULTS: A survey was constructed that contained 24 multiple-choice questions. The survey included questions pertaining to surgeon demographics, if pyloromyotomy was taught

Evolutions in both co-payment and generic market share for common medication in the Belgian reference pricing system.

BACKGROUND: In Belgium, a co-insurance system is applied in which patients pay a portion of the cost for medicines, called co-payment. Co-payment is intended to make pharmaceutical consumers more responsible, increase solidarity, and avoid or reduce moral hazards. OBJECTIVE: Our objective was to study the possible influence of co-payment on sales volume and generic market share in two commonly used medicine groups: cholesterol-lowering medication [statins (HMG-CoA reductase inhibitors) and fibrates] and acid-blocking agents (proton pump inhibitors and histamine H2 receptor antagonists). METH

Impact of the Screening Abdominal Aortic Aneurysms Very Efficiently (SAAAVE) Act on abdominal ultrasonography use among Medicare beneficiaries.

BACKGROUND: Since January 1, 2007, Medicare has covered abdominal aortic aneurysm (AAA) screening for new male enrollees with a history of smoking under the Screening Abdominal Aortic Aneurysms Very Efficiently (SAAAVE) Act. We examined the association between this program and abdominal ultrasonography for AAA screening, elective AAA repair, hospitalization for AAA rupture, and all-cause mortality. METHODS: We used a 20% sample of traditional Medicare enrollees from 2004 to 2008 to identify 65-year-old men eligible for screening and 3 control groups not eligible for screening (70-year-old men

Pregnancy loss and role of infant HIV status on perinatal mortality among HIV-infected women.

BACKGROUND: HIV-infected women, particularly those with advanced disease, may have higher rates of pregnancy loss (miscarriage and stillbirth) and neonatal mortality than uninfected women. Here we examine risk factors for these adverse pregnancy outcomes in a cohort of HIV-infected women in Zambia considering the impact of infant HIV status. METHODS: A total of 1229 HIV-infected pregnant women were enrolled (2001-2004) in Lusaka, Zambia and followed to pregnancy outcome. Live-born infants were tested for HIV by PCR at birth, 1 week and 5 weeks. Obstetric and neonatal data were collected after

Should we treat depression with drugs or psychological interventions? A reply to Ioannidis.

We reply to the Ioannidis's paper "Effectiveness of antidepressants; an evidence based myth constructed from a thousand controlled trials." We disagree that antidepressants have no greater efficacy than placebo. We present the efficacy from hundreds of trials in terms of the percentage of patients with a substantial clinical response (a 50% improvement or more symptomatic reduction). This meta-analysis finds that 42-70% of depressed patients improve with drug and 21%-39% improve with placebo. The response benefit of antidepressant treatment is 33%-11% greater than placebo. Ioannidis argues tha

Attitudes about honour killing among men and women--perspective from Islamabad.

BACKGROUND: The concept of honour has cultural, social and moral underpinnings that determine its expression and perseveration. Women are viewed as the bearers of family honour with chastity equated with abstinence from premarital or extramarital relationships and obeying norms determined and dictated by traditions and societies. The objective of this study was to determine the opinions of men and women pertaining to killing in the name of and saving one's honour. METHODS: A cross-sectional survey with convenience sampling was conducted among the 18 years and older (range 18-71) men and women

[Forms of custody of children placed in Switzerland: ambiguous policies, resistance, and conflicting objectives (1850-1950).].

In the past, many European countries were faced with the problem of providing care for boarded-out children. And very often the policies implemented up to the middle of the twentieth century were essentially similar and thus similarly inadequate. The problem with boarding out is that it was a measure in response to widely varying situations, not only in respect of the illegitimate as well as the legitimate children concerned, but also with regard to the reasons which led to boarding out. Orphans after the First World War with no relatives who could take them in formed a minority, and in severa

Defective maintenance of T cell tolerance to a superantigen in MRL-lpr/lpr mice.

In normal mice neonatal injection of staphylococcal enterotoxin B (SEB) induces tolerance in T cells that express reactive T cell receptor (TCR) V beta regions. To determine if a T cell neonatal defect was present in MRL-lpr/lpr mice, 20 micrograms of SEB was injected intraperitoneally every other day into V beta 8.2 TCR transgenic and nontransgenic MRL(-)+/+ and MRL-lpr/lpr mice from birth to 2 wk of age. At 2 wk of age, V beta 8+ T cells were depleted, and SEB reactivity was lost, in spleen, lymph node, and thymus. These effects were equivalent in +/+ and lpr/lpr SEB-tolerized mice. However,

Clinical implications of respect for autonomy in the psychiatric treatment of pregnant patients with depression.

Major depression, as well as depressive symptoms that do not meet the full diagnostic criteria for a diagnosis of depression, can chronically and variably affect a woman patient's decisions about the management of pregnancy, including the decision about whether to continue a pregnancy. Depression also has potential adverse consequences for the pregnant woman and her pregnancy. However, little attention has been given to the ethical challenges posed by the psychiatric management of depression during pregnancy. The psychiatrist should balance respect for the autonomy of the depressed woman with

How do social norms influence sexual and gender-based violence and access to care and support services among youth? A qualitative study in Busia and Mbale districts, Eastern Uganda.

BACKGROUND: Harmful social norms underpin much of the sexual and gender-based violence (SGBV) experienced globally, influencing both its perpetration and survivors’ access to care. In Uganda, these norms rooted in gender inequality, male authority and survivor stigma remain pervasive, particularly in rural districts with limited youth-friendly services. Few studies have explored how such norms shape help-seeking behaviour among young survivors. This study examined the social norms surrounding SGBV and their impact on access to care and support in Busia and Mbale districts. METHODS: This quali

Socio-Cultural Experiences of Exclusive Breastfeeding Among Banjar Mothers in Indonesia.

Background: Exclusive breastfeeding is recognized as an effective public health intervention; however, its practice is shaped by complex social, emotional, and cultural dynamics, particularly within collectivist societies. This study aims to explore in depth the meaning of exclusive breastfeeding in the early postpartum period within the socio-cultural context of the Banjar community with a focus on postpartum stress and the dynamics of social support. Methods: This qualitative study, grounded in a constructivist paradigm and designed as a case study, was conducted among 15 Banjar mothers aged

Emergency department culture and the care of older patients with frailty: a focused ethnography.

BACKGROUND: Culture shapes care delivery, professional communication, and the development of trust between patients and providers. Culture encompasses identifiable elements, shared values and beliefs, and underlying assumptions that shape behaviour. The Emergency Department (ED) subculture arises from its complex, high-pressure focus on acute illness. ED professionals increasingly encounter older patients with frailty, whose complex but often non-urgent needs require more holistic, individualised care. However, the ED subculture may hinder the provision of appropriate geriatric emergency care.

Caring, pragmatic and discriminatory: how private paediatric outpatient providers normalise unequal care for Roma-ethnographic findings from affluent Slovakia.

BACKGROUND: Healthcare providers may enact discriminatory practices while perceiving themselves as caring and pragmatic. Whilst anti-Roma discrimination in Central and Eastern European healthcare has been documented in hospital and emergency settings in poorer regions, less is known about routine outpatient care in affluent areas. This ethnographic study examines how private paediatric outpatient providers in Slovakia’s most affluent region normalise unequal care for Roma families through everyday practices rationalised as contextually appropriate responses. METHODS: The study was conducted be
