Clinical uses of antiprogestogens.

Antiprogestogens, which block the action of progesterone at the cellular level through binding to the progesterone receptor, are proving to be one of the most significant developments in endocrinology in recent years. Several hundreds of such compounds have been synthesized, but only a few of them have been evaluated to any significant extent in biological screening models and, to our knowledge, only three compounds, namely mifepristone, lilopristone (ZK 98.734) and onapristone (ZK 98.299) have been given to humans. Most of the clinical research to date has focused on the use of mifepristone g

Effects of a combined estrogen-gestagen regimen on serum levels of the carboxy-terminal propeptide of human type I procollagen in osteoporosis.

Estrogen stimulates osteoblastic collagen production in vitro, but whether the same stimulation takes place in vivo is still unknown. To test the stimulatory effects of a combined estrogen-gestagen regimen in vivo we monitored serum levels of the carboxy-terminal propeptide of human type I procollagen (S-PICP) in a group of 12 osteoporotic women over a 150 week treatment period. Spinal bone mineral content (BMC) increased to a maximum of 5% over pretreatment values around week 90. Serum alkaline phosphatase (S-AP) and serum bone gla protein (S-BGP) both fell from initial values of 220 U/liter

Development and Validation of an Endometrial Cancer Algorithm in US Claims Data.

PURPOSE: This study developed and validated algorithms to identify the incidence of endometrial cancer using ICD-9-CM and ICD-10-CM coding systems as part of a post-authorization safety study (EU PAS: EUPAS45602) of vaginal estrogen, estrogen and progestin combination hormone therapy, and non-users of vaginal estrogen. METHODS: Using national claims data in the HealthCare Integrated Research Database, provisional endometrial cancer cases were identified using a screening algorithm comprising at least one endometrial cancer diagnosis code at any location and position between January 1, 2007 an

Hormonal contraceptives and EEG biomarkers for antidepressant treatment response in women.

BACKGROUND: Women are disproportionately affected by major depressive disorder (MDD). Widely used hormonal contraceptives are linked to depression and altered brain function, yet their unstudied impact on EEG-biomarkers of antidepressant response may confound efforts to develop biomarkers for women. METHODS: In 60 unmedicated premenopausal women with MDD: non-users (n = 25), combined oral contraceptive users (COC, n = 19), and progestin-only contraceptive users (POC, n = 16), we assess five EEG-biomarkers before eight weeks of 10-20 mg escitalopram: Alpha peak frequency (APF), Vigilance level

Contraceptive hormone ethinyl estradiol but not levonorgestrel modulates the role of reinforcer-enhancement in nicotine self-administration in ovary-intact female Sprague-Dawley rats.

Hormonal contraceptives containing a synthetic estrogen (e.g., ethinyl estradiol/EE) and/or a progestin (e.g., levonorgestrel/LEVO) are associated with heightened nicotine use. Whether altered intake reflects changes in nicotine reinforcement or nicotine enhancement of co-occurring reinforcers is unknown. Reinforcer-enhancement is evidenced when nicotine self-administration increases when delivered with a reinforcing visual stimulus (VS). We examined EE and LEVO effects on nicotine reinforcement and reinforcer-enhancement in ovary-intact female Sprague-Dawley rats. Rats were implanted with a j

Associations between clinical AUDIT-C screens and HDL cholesterol are observed across primary care patient subgroups.

BACKGROUND: The Alcohol Use Disorders Identification Test-Consumption (AUDIT-C) is a validated, scaled marker of past-year alcohol consumption that is increasingly used in population-based screening and research. Like other screening questionnaires, AUDIT-C scores are influenced by patient and system factors affecting self-report. High-density lipoprotein (HDL) cholesterol increases with alcohol consumption and is routinely measured in primary care. Researchers using AUDIT-C scores as an outcome could potentially use HDL as a population-level check on the performance of alcohol screening, for

Knowledge gaps and information needs and preferences regarding oral contraceptive pills and over-the-counter access: A focus group study with Black and Latinx young people assigned female at birth.

OBJECTIVE: On July 13, 2023, the Food and Drug Administration (FDA) approved a progestin-only oral contraceptive pill (OCP) for over-the-counter (OTC) use without an age restriction. Prior to this approval, we launched a qualitative study with Black and Latinx young people to inform access to an OTC OCP. Here we present findings related to knowledge gaps and information needs and preferences. STUDY DESIGN: From April 26 to July 27, 2023, we held eight in-person focus groups (prior to the FDA approval) and one virtual focus group (after the FDA approval) with 65 pregnancy-capable young people

Oral Contraceptives Use and Colorectal Cancer Risk Among Moroccan Women: A Case-Control Study.

OBJECTIVE: Colorectal cancer (CRC) encompasses both non-modifiable and modifiable risk factors. While diet remains the main factor influencing CRC risk, hormonal and reproductive factors have also demonstrated a potential impact on CRC risk. This study aims to explore the relationship between oral contraceptives (OC) use and Colorectal cancer risk among Moroccan women. METHODS: We conducted a case-control study included 300 participants, divided into 150 cases and 150 controls matched by age (± 3 years). Through a face-to-face interview with trained investigators, we collected data on oral co

Using updated clinical recommendations to support contraceptive decision-making: U.S. medical eligibility criteria for contraceptive use, 2024.

OBJECTIVES: In August 2024, the U.S. Centers for Disease Control and Prevention updated U.S. Medical Eligibility Criteria for Contraceptive Use (U.S. MEC), which provides recommendations for safe use of contraception for women with certain characteristics and medical conditions. This paper provides a summary of the evidence and context for new and updated U.S. MEC recommendations. STUDY DESIGN: The 2024 U.S. MEC was updated through a rigorous, multi-year process of determining priorities that would have high impact on clinical care, conducting systematic reviews of the evidence, and consideri

Hormonal contraception and risk of major adverse cardiovascular events: A nationwide registry study from Finland.

OBJECTIVES: In this study, we assessed the association of modern systemic hormonal contraception (HC), including newly introduced estradiol-containing combined oral contraceptives, with major adverse cardiovascular events (MACE): myocardial infarction [MI] and ischemic stroke [IS], during 2018-2019. STUDY DESIGN: A prospective registry-based cohort study including all 15-49-year-old women residing in Finland in 2017, using HC. We identified women from the national prescription register and incident cases of MACE from the national patient register, yielding 584,236 women and 1167,685 person-ye
