Defining the criteria for intubation of the patient with thermal burns.

OBJECTIVES: Recent studies demonstrate that burn patients are undergoing unnecessary intubations. We sought to determine the clinical criteria that predict intubations with benefit. METHODS: This was a retrospective review of intubated adults admitted to our center with thermal burns 2008-2013. Criteria for intubation were defined as traditional criteria (suspected smoke inhalation, oropharynx soot, hoarseness, dysphagia, singed facial hair, oral edema, oral burn, non-full thickness facial burns), or ABA criteria as defined by the 2011 ABA guidelines (full thickness facial burns, stridor, res

Prenatal and perinatal morbidity in children with Tourette syndrome and attention-deficit hyperactivity disorder.

OBJECTIVE: Tourette syndrome (TS) and attention-deficit hyperactivity disorder (ADHD) are frequently seen in combination, though the cause of comorbidity is uncertain. Low birth weight is a known risk factor for ADHD. The objective of the study was to assess the association between pre- and perinatal morbidity and the comorbid diagnosis of ADHD in children with TS. METHOD: A nested case-control study of children evaluated for TS at a subspecialty clinic was performed. Cases were defined as children with TS and ADHD; controls had TS without ADHD. Exposure to pre- and perinatal morbidity was as

Interobserver Variability of Both Glasgow Coma Scale and Full Outline of Unresponsiveness Scores in Forecasting the Results of Critically Ill Patients with Altered Sensorium.

BACKGROUND: Altered mental status (AMS) refers to changes in cognitive function or consciousness, encompassing cognitive, attention, arousal, and consciousness disorders. The Glasgow Coma Scale (GCS) and full outline of unresponsiveness (FOUR) score are tools used to evaluate patients with altered consciousness. Few studies have compared the interobserver reliability of these scales. This study aimed to assess interobserver variability between GCS and FOUR scores in predicting outcomes of critically ill patients with altered sensorium. METHODOLOGY: This hospital-based forecasting experimental

Sleep-Disordered Breathing and Central Respiratory Control in Children: A Comprehensive Review.

Background/Objectives: Sleep-disordered breathing (SDB) is a primary concern in children's health. Research suggests that repeated oxygen drops during sleep-common in SDB-may harm the brainstem's breathing control centres. This damage likely occurs through oxidative stress, inflammation, and cell death, which weaken the brain's ability to regulate breathing. Over time, these effects could lead to functional changes (e.g., disrupted chemical signalling) and physical damage in critical brain regions, creating a cycle of unstable breathing. However, much of this evidence comes from animal or lab

Managing xylazine-involved overdoses in a community harm reduction setting: lessons from Tijuana, Mexico.

BACKGROUND: Xylazine is a α2-adrenergic receptor agonist, used for sedation in veterinary contexts. Although it is increasingly found in overdose deaths across North America, the clinical management of xylazine-involved overdoses has not been extensively studied, especially in community-based harm reduction settings. Here we present a clinical series of xylazine-involved overdose and share the clinical approach and lessons learned by a community overdose response team in Tijuana, Mexico amidst the arrival of xylazine. CASE PRESENTATION: We present three cases describing the clinical managemen

Cardiac expression and location of hexokinase changes in a mouse model of pure creatine deficiency.

Creatine kinase (CK) is considered the main phosphotransfer system in the heart, important for overcoming diffusion restrictions and regulating mitochondrial respiration. It is substrate limited in creatine-deficient mice lacking l-arginine:glycine amidinotransferase (AGAT) or guanidinoacetate N-methyltranferase (GAMT). Our aim was to determine the expression, activity, and mitochondrial coupling of hexokinase (HK) and adenylate kinase (AK), as these represent alternative energy transfer systems. In permeabilized cardiomyocytes, we assessed how much endogenous ADP generated by HK, AK, or CK st

Predictors of subjective and physiological craving during VR-cue exposure in alcohol use disorder: an explorative single-arm clinical study.

BACKGROUND: Craving is a diagnostic criterion and predictive factor in alcohol use disorder (AUD). In Virtual Reality Cue Exposure (VR-CE), it is intentionally induced as a basis for Virtual Reality Cue Exposure Therapy. As the underlying subjective and physiological factors and predictors of craving are still incompletely understood, we aim to identify factors associated with craving responses during VR-CE in patients with AUD. METHODS: Craving responses of 61 patients with AUD during VR-CE were measured using subjective ratings (visual analogue scale, VAS) and physiological parameters, incl

An Unusual Presentation of Respiratory Dysfunction in Parkinson's Disease: A Case Study.

Parkinson's disease (PD) is a progressive neurodegenerative disorder affecting around 10 million people worldwide. It is primarily associated with the loss of dopaminergic neurons in the substantia nigra pars compacta, though the exact mechanism is unclear. PD is characterized by resting tremor, muscular rigidity, bradykinesia, and postural instability. These motor symptoms, resulting from impaired muscle function, can lead to dysphagia and respiratory dysfunction. It is suggested that the muscular rigidity and bradykinesia in PD may impair repetitive motor actions, negatively impacting respir

The phase coherence of the neurovascular unit is reduced in Huntington's disease.

Huntington's disease is a neurodegenerative disorder in which neuronal death leads to chorea and cognitive decline. Individuals with ≥40 cytosine-adenine-guanine repeats on the interesting transcript 15 gene develop Huntington's disease due to a mutated huntingtin protein. While the associated structural and molecular changes are well characterized, the alterations in neurovascular function that lead to the symptoms are not yet fully understood. Recently, the neurovascular unit has gained attention as a key player in neurodegenerative diseases. The mutant huntingtin protein is known to be pres

[Results of neonatal screening for spinal muscular atrophy in Hungary in 2023].

Introduction: Optimal health care for patients born with spinal muscular atrophy can only be achieved through neonatal screening. Neonatal screening for this incurable, progressive genetic disease that most often causes death in childhood has been introduced in many countries, and its usefulness has been proven with the outstanding results of early diagnosis and initiation of therapy. Objective: To evaluate the neonatal screening research program in Hungary, to examine the reliability, public demand, cost-effectiveness of the chosen screening method, and the health benefits of early treatment;

Calcitonin gene-related peptide protects from soluble fms-like tyrosine kinase-1-induced vascular dysfunction in a preeclampsia mouse model.

Introduction: Preeclampsia (PE) is a hypertensive disorder during pregnancy associated with elevated levels of soluble FMS-like tyrosine kinase (sFLT-1) and increased vascular sensitivity to angiotensin II (ATII). Calcitonin gene-related peptide (CALCA) is a potent vasodilator that inhibits the ATII-induced increase in blood pressure and protects against ATII-induced increases in oxidative stress through a mitochondrial-dependent pathway in male mice. In rodent pregnancy, CALCA facilitates pregnancy-induced vascular adaptation. Most of the vascular effects of CALCA are mediated by vascular smo

Morphological and enzymatical observations in Oncomelania hupensis after molluscicide treatment: implication for future molluscicide development.

A preparation of niclosamide named 50 % wettable powder of niclosamide ethanolamine salt (WPN), the only chemical molluscicide available in China, has been widely used for Oncomelania hupensis control over the past 20 years, but its molluscicidal mechanism has not been elucidated yet. Recently, a derivative of niclosamide, the salt of quinoid-2',5-dichloro-4'-nitro-salicylanilide (Liu Dai Shui Yang An, LDS), has been proven to have equivalent molluscicidal effects as WPN but with lower cost and significantly lower toxicity to fish than WPN. In our previous study, gene expression profiling of O

Adult Quality of Life Post Cleft Palate Repair: A Comparison of Two Techniques.

BACKGROUND AND PURPOSE: In 1989, the Cleft Palate-Craniofacial Journal published the first randomized prospective cleft surgery study, comparing the Kriens intravelar veloplasty (IVV) with a non-IVV 2-flap repair. Results in that and follow-up publications yielded no difference between the 2 groups for need for secondary velopharyngeal management. The subjects have now reached adulthood. This study was designed to ask: Is there any difference between the groups in the outcomes that multidisciplinary team care addresses: speech intelligibility, facial growth, breathing while awake and asleep, a

Performance of published scoring tools for predicting the risk of perioperative respiratory adverse events in children - An evaluation in a large paediatric cohort.

BACKGROUND: Perioperative respiratory adverse events (PRAE) are a main cause of morbidity and mortality in paediatric anaesthesia. Clinicians need to be able to predict their patients' risk of PRAE to plan their care. Clinical risk prediction tools have been developed to assist with pre-operative risk stratification; however, validation outside the contexts of their development is limited. In this study, we test the ability of common risk prediction tools to identify patients at high risk of PRAE in general anaesthesia. METHODS: In this post-hoc secondary analysis, six risk prediction scores

Spiritual care for prevention of psychological disorders in critically ill patients: study protocol of a feasibility randomised controlled pilot trial.

INTRODUCTION: A significant number of critically ill patients who survive their illness will experience new sequelae or a worsening of their baseline health status following their discharge from the hospital. These consequences may be physical, cognitive and/or psychological and have been labelled postintensive care syndrome (PICS). Prior research has demonstrated that spiritual care aligned with a specific creed during hospitalisation in the intensive care unit (ICU), as part of a comprehensive care plan, may be an effective strategy for preventing psychological sequelae in surviving critical
