An electronic regulatory document management system for a clinical trial network.

A computerized regulatory document management system has been developed as a module in a comprehensive Clinical Trial Management System (CTMS) designed for an NIH-funded clinical trial network in order to more efficiently manage and track regulatory compliance. Within the network, several institutions and investigators are involved in multiple trials, and each trial has regulatory document requirements. Some of these documents are trial specific while others apply across multiple trials. The latter causes a possible redundancy in document collection and management. To address these and other r

Case Report: Recurrent anti-glutamic acid decarboxylase 65 antibody-associated encephalitis in a child.

OBJECTIVE: To explore the clinical characteristics of recurrent pediatric anti-glutamic acid decarboxylase 65 (GAD65) antibody-associated au … d occipital cortical regions. Electroencephalogram (EEG) revealed focal epileptiform discharges and status epilepticus. His clinical condition improved following treatment with methylprednisolone and intravenous immunoglobulin. In November 2024, the child presented with a decline in mental status, generalized weakness, increased somnolence, reduced mobility and soci …

Epilepsy and neuroprotection: an illustrated review.

Multiple types of insults, such as status epilepticus, hypoxia and trauma, may alter the central nervous system. Strategies to protect the brain against insults remain a very difficult and challenging problem. Damage to the central nervous system can be modulated via excessive excitatory and reduced inhibitory neurotransmission. In addition, increased sodium and calcium loading through impaired voltage-sensitive channels, as well as alterations in the acid-base balance can contribute to both excitotoxic and apoptotic cell death. Epilepsy treatment has always been related to neuroprotection, si…

Two patients were presented with status epilepticus. Interictal EEG in the patients with seizures revealed theta to delta slowing in all patients and epileptiform discharges in four patients only. Management of Urgent Medical Conditions at the End of Life.

Medical emergencies at the end of life require recognition of patients at risk, so that a comprehensive assessment and plan of care can be put in place. Frequently, the interventions depend on the patient's underlying prognosis, location of care, and goals of care. The mere presence of a medical emergency often rapidly changes an estimated prognosis. Education of the patient and family may help empower them to adequately handle many situations when clinicians are not available.

Galanin contributes to monoaminergic dysfunction and to dependent neurobehavioral comorbidities of epilepsy.

Status epilepticus (SE) in rats, along with chronic epilepsy, leads to the development of behavioral impairments resembling depressive disorder and/or attention deficit/hyperactivity disorder (ADHD), thus reflecting respective comorbidities in epilepsy patients. Suppressed neurotransmitter tone in the raphe nucleus (RN)-prefrontal cortex (PFC) serotonergic pathway and in the locus coeruleus (LC)-PFC noradrenergic pathway underlies depressive- and impulsive-like behavioral deficits respectively. We examined possible mechanisms leading to the monoamine dysfunction in brainstem efferents, namely …

G2677T polymorphism can predict treatment outcome of Malaysians with complex partial seizures being treated with Carbamazepine.

Carbamazepine (CBZ) is used as the first line of treatment of complex partial seizures (CPS) in Malaysia. While this drug is known to be effective for the treatment of CPS, more than 30% of patients remain drug resistant to CBZ mono-therapy. We examined a possible relationship between patients' response to CBZ mono-therapy and the G2677T SNP of the ABCB1 gene. Three hundred and fourteen patients with CPS were recruited from the Neurology Department of the Kuala Lumpur Hospital, of whom 152 were responders and the other 162 were non-responders to CBZ mono-therapy. DNA was extracted from blood s

Occlusion of modified Blalock-Taussig shunt after clopidogrel cessation.

It has been suggested previously that rebound hypercoagulability may be responsible for morbidity and mortality following clopidogrel cessation in adults with acute coronary syndrome. We report a case of acute occlusion of a modified Blalock-Taussig shunt in an infant after clopidogrel discontinuation.

Respiratory chain deficiency in Alpers syndrome.

Alpers syndrome is a progressive encephalopathy of early onset, characterized by rapid and severe developmental delay, intractable seizures and liver involvement in a previously healthy child. Here, we report on respiratory chain enzyme deficiency in the liver of four unrelated children presenting with epileptic encephalopathy and liver involvement diagnosed as Alpers syndrome. Interestingly, oxidative phosphorylation in skeletal muscle was normal in 4/4 and blood and CSF lactate in 3/4 patients. Liver involvement had a late clinical onset in patients with previously isolated epileptic encepha

Rapid Cycle Deliberate Practice of Pediatric Emergencies Education for Pharmacists (PEEPs).

PURPOSE: To describe the development and implementation of Pediatric Emergencies Education for Pharmacists (PEEPs), a rapid-cycle deliberate practice curriculum designed to prepare inpatient pharmacists for medication-related responsibilities during pediatric emergency responses. SUMMARY: PEEPs is a two-hour, in-person program delivered to pairs or triads of pharmacists. After a prebrief, orientation, and warm-up scenario, participants complete approximately 15 rapid practice cycles involving septic shock, severe hypoglycemia, status epilepticus, and supraventricular tachycardia. Prevalence and Associations of Abnormal Levels of Vitamin B6 in People with Parkinson's Disease.

BACKGROUND: In people with Parkinson's disease, abnormal levels of vitamin B6 may be associated with polyneuropathy and epileptic seizures, including medication-refractory status epilepticus. METHODS: In this cross-sectional study, fasting serum levels of vitamin B6 and related metabolites were measured in 144 people with Parkinson's disease randomly recruited from eligible clinic patients, compared with 144 historical controls matched for age, sex, diet, and v…
