Differential effects of stent-graft fabrics on arterial stiffness in patients undergoing endovascular aneurysm repair.

PURPOSE: To evaluate the differential effects of endograft fabric types on pulse wave velocity (PWV), a valid, clinically feasible marker of arterial stiffness, in patients undergoing endovascular repair (EVAR) of abdominal aortic aneurysms (AAAs). METHODS: As part of a registered study (ClinicalTrials.gov identifier NCT00636766), 118 consecutive men (mean age 71 ± 8 years) with AAA undergoing elective EVAR were divided into groups according to the type of fabric in the implanted endografts: 46 had polytetrafluoroethylene (PTFE)-covered stent-grafts and 72 received stent-grafts covered with p

Type II endoleaks after endovascular repair of abdominal aortic aneurysm are not always a benign condition.

OBJECTIVE: The aim of the study was to determine whether type II endoleak (T2E) after endovascular repair of abdominal aorta (EVAR) is a benign condition (ie, not associated with growth, reintervention, rupture, or death). METHODS: Data from patients who underwent EVAR for atherosclerotic infrarenal aortic aneurysms between June 1995 and May 2010 in the Vascular Surgery Department of Henri Mondor Hospital were prospectively collected. Data from patients presenting with at least one T2E on computed tomography scan during their follow-up were compared with those with no T2E. Three subcategories

Coarctation-associated aneurysms: a localized disease or diffuse aortopathy.

BACKGROUND: We evaluated the occurrence and treatment of aortic aneurysms in coarctation patients. METHODS: During 1962 to 2011, 943 cases of coarctation were repaired. Aortic aneurysms were identified in 55 patients (5.8%). Forty-eight had prior coarctation repair (median 23 years earlier, interquartile range 18 to 26 years). Forty-two aneurysms were found in the descending thoracic aorta (76.4%), 18 in the ascending aorta (32.7%), 8 in the left subclavian artery (14.5%), and 1 each (1.8%) in the abdominal aorta, iliac artery, and innominate artery. Twenty-three patients (41.8%) had multiple

Blunt abdominal aortic injury.

BACKGROUND: Blunt abdominal aortic injury (BAAI) is a rare injury with less than 200 cases in the current reported world literature, mostly in case report format. We sought to describe the experience of a high-volume trauma center and to provide a contemporary review of the literature to better understand the natural history and management of this injury. METHODS: This was a retrospective review of patients with BAAI between 1996 and 2010. Data collected included demographics, mechanism of injury, associated injuries, type of intervention, subsequent imaging, and follow-up. BAAI was classifie

Sex-based outcomes after endovascular repair of thoracic aortic aneurysms.

INTRODUCTION: Unlike with abdominal aortic aneurysms (AAA), women appear to have an almost comparable incidence as men for thoracic aortic aneurysms (TAA). However, the extent to which a patient's sex influences endograft treatment of TAA has not been reported. The current study analyzes the influence of sex on the endovascular management of TAAs. METHODS: A total of 421 patients (265 men and 156 women) were identified as part of the TAG (W. L. Gore and Associates, Flagstaff, Ariz) thoracic stent graft trials. Preoperative risk factors, intraoperative events, and 365-day follow-up data were a

Predictors of creatinine rise post-endovascular abdominal aortic aneurysm repair.

BACKGROUND: Endovascular abdominal aortic aneurysm repair involves manipulation of the aorta around the renal arteries. Fenestrated grafts involve the direct cannulation, stenting and injecting of contrast into the renal arteries. These procedures may be associated with an acute post-operative creatinine rise. METHODS: We retrospectively examined data from all endovascular aortic repairs at our institution from 2005 to 2009, where contrast dosage had been recorded. Renal impairment was defined as a 25% increase in creatinine during the 5-day postoperative period. Univariable analysis was unde

Acute aortic syndromes.

Acute aortic syndromes (AAS) comprise a group of potentially lethal conditions that require prompt recognition, diagnosis as well as acute medical stabilization and surgical intervention. The purpose of this article is to review the relevant variants of AAS presentation, as well as diagnostic and management issues, including adequate long-term medical therapy and follow-up imaging. In this context, the American College of Cardiology and the American Heart Association recently published guidelines on the management of thoracic aortic disease, drawing greater attention to these processes.

Outcomes and timing of aortic surgery in renal transplant patients.

BACKGROUND: Solid organ transplantation in elderly patients has become more common in recent years. An increasing number of patients present with renal failure requiring transplantation and comorbid occlusive or aneurysmal aortic pathology. The optimal strategy for the timing and management of the aortic disease and renal transplantation in these patients is unknown. Before the availability of endovascular therapies, our policy was to provide open repair of aortic disease before cadaveric transplantation, or by simultaneous aortic reconstruction with renal allotransplantation if a living donor

The limitations of thoracic endovascular aortic repair in altering the natural history of blunt aortic injury.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) is accepted treatment for blunt aortic injury (BAI). We hypothesized that immediate TEVAR would reduce deaths from aortic rupture in patients with BAI. METHODS: Review of 81 patients with BAI who arrived alive at a level I trauma center over a 10-year period. RESULTS: Twenty-three patients (28%) died within 4 hours of admission, including 12 who died of aortic rupture. Fifty-eight patients (72%) survived beyond 4 hours, and 8 (14%) ultimately died of associated injuries. Forty patients (69%) underwent aortic repair (30 open repair, 10 T

Outcomes of the endovascular management of aortic arch aneurysm: implications for management of the left subclavian artery.

BACKGROUND: This study was conducted to define the outcomes of treating aortic aneurysms involving the arch vessels with a hybrid approach using extra-anatomic reconstruction and endovascular repair with nonfenestrated stents. METHODS: A single-center review was done of arch aneurysm endovascular repairs during an 8-year period. Data were collected for patient demographics and aneurysm pathology. Any revascularization procedures performed were analyzed to detect differences between groups and through fitting a logistic regression model. The outcome measures were postoperative death, stroke, a

Single-stage repair of acute type A aortic dissection associated with aortic coarctation, perimembranous ventricular septal defect, and bicuspid aortic valve.

A 17-year-old man with coarctation of the thoracic aorta, bicuspid aortic valve, perimembranous ventricular septal defect, and acute type A dissection of the ascending aorta underwent emergency repair for dissection by means of single-stage replacement of not only the aortic valve and ascending aorta, but also simultaneous repair of the coarctation by graft interposition. These combined conditions comprised some difficulties that included decisions on the optimal timing and sequence of surgical repair, technique, and circulatory support.

Preservation of pelvic circulation with hypogastric artery bypass in endovascular repair of abdominal aortic aneurysm with bilateral iliac artery aneurysms.

PURPOSE: The endovascular repair (EVAR) of an abdominal aortic aneurysm (AAA) with a bilateral common iliac artery aneurysm (CIAA) often requires exclusion of the bilateral hypogastric artery (HA), which can be associated with pelvic ischemic complications such as erectile dysfunction and buttock claudication. This study assessed the effect of HA bypass on improving pelvic circulation. METHODS: Five patients who underwent endovascular repair with HA bypass for an AAA with bilateral CIAA were evaluated. In all patients, the patency of the inferior mesenteric artery and bilateral HAs arteries w
