Surgical fixation methods for tibial plateau fractures.

BACKGROUND: Fractures of the tibial plateau, which are intra-articular injuries of the knee joint, are often difficult to treat and have a high complication rate, including early-onset osteoarthritis. Surgical fixation is usually used for more complex tibial plateau fractures. Additionally, bone void fillers are often used to address bone defects caused by the injury. Currently there is no consensus on either the best method of fixation or bone void filler. OBJECTIVES: To assess the effects (benefits and harms) of different surgical interventions, and the use of bone void fillers, for treatin

Evaluating One-Week Post-Casting Alignment Checks and Surgical Intervention Rates in Pediatric Type IIA Supracondylar Humeral Fractures.

BACKGROUND: This study aims to determine the rate of surgical intervention in children with type IIA supracondylar humerus fractures (SCHF) following routine post-casting radiographic assessment. We hypothesized that no cases would convert to operative management following one-week post-casting alignment assessments. METHODS: This single-center retrospective study focused on pediatric patients diagnosed with type IIA SCHF from 2019 to 2022. Patients were treated with initial long arm cast immobilization, followed by a one-week post-casting radiographic alignment check in cast. Fractures were

Wound drainage after proximal femoral nail antirotation (PFNA) fixation may negatively affect the patients with intertrochanteric fractures: A prospective randomized controlled trial.

INTRODUCTION: The effect of using closed suction drainage system with the proximal femoral nail antirotation (PFNA) system fixation on outcomes in treating intertrochanteric fractures (ITFs) is still unknown. This prospective randomized controlled trial aimed to examine whether routine drainage is useful for PFNA fixation in ITFs. METHODS: A total of 80 patients with acute ITFs were treated with closed or mini-open reduction with PFNA fixation at the National Cheng Kung University Hospital and 60 eligible patients were randomized for whether to receive suction drainage. In clinical outcomes,

Association between surgical delays for femoral neck fractures and early postoperative complications in young and middle-aged adults: A study based on the national inpatient sample database.

BACKGROUND: Femoral neck fractures (FNF) in young and middle-aged adults are primarily caused by high-energy injuries in traffic accidents. Surgical delays often occur due to transportation issues, preoperative evaluations, and economic burdens. METHODS: A retrospective analysis was conducted on young and middle-aged FNF patients undergoing reduction and internal fixation surgeries from 2010 to 2019 with the use of the National Inpatient Sample database. Logistic regression analysis was used to assess the relationship between surgical delays and complications, and the independent risk factors

Unravelling long-term source removal effects and chlorinated methanes natural attenuation processes by C and Cl stable isotopic patterns at a complex field site.

The effects of contaminant sources removal in 2005 (i.e. barrels, tank, pit and wastewater pipe sources) on carbon tetrachloride (CT) and chloroform (CF) concentration in groundwater were assessed at several areas of a fractured multi-contaminant aquifer (Òdena, Spain) over a long-term period (2010-2014). Changes in redox conditions, in these chlorinated methanes (CMs) concentration and in their carbon isotopic compositions (δ13C) were monitored in multilevel wells. δ13C values from these wells were compared to those obtained from sources (barrels, tank and pit before their removal, 2002-2005)

Open versus minimally invasive fixation of a simulated syndesmotic injury in a cadaver model.

BACKGROUND: Malreduction of unstable syndesmotic ankle fractures is common. This study compared the reduction quality of an anterolateral open technique (OT) versus a conventional minimally invasive technique (MIT). METHODS: Fourteen fresh-frozen lower torso specimens with 28 matched lower extremities underwent computed tomography (CT) to measure syndesmosis position before dissection. Reduction was performed using direct visualization and fluoroscopy for the OT group (right-sided specimens) and fluoroscopy only for the MIT group (left-sided specimens). Fixation was achieved with 2 cortical s

Nursemaid's Elbow With Transient Wrist Drop in a Toddler: An Atypical Presentation.

Radial head subluxation, commonly known as nursemaid's elbow, is a frequent upper extremity injury in young children and typically presents with refusal to use the affected arm without focal distal motor dysfunction. We report an atypical presentation in a previously healthy two-year-and-10-month-old girl who was brought to the emergency department after an unwitnessed event with no clearly identified mechanism of trauma. She held her right elbow flexed against her chest in a posture suggestive of radial head subluxation, and additionally refused to let go of her wrist and demonstrated a wrist

Hexaxial external fixator versus intramedullary nail in treating segmental tibial fractures: a retrospective study.

BACKGROUND: It's difficult to treat segmental tibial fractures (STFs), which are intricate injuries associated with significant soft tissue damage. The aim of this study was to compare the clinical effect of hexaxial external fixator (HEF) and intramedullary nail (IMN) in treatment of STFs. METHODS: A total of 42 patients with STFs were finally recruited between January 2018 and June 2022. There were 25 males and 17 females with age range of 20 to 60 years. All fractures were classified as type 42C2 using the Arbeitsgemeinschaftfür Osteosythese/Orthopaedic Trauma Association (AO/OTA) classifi

Ultrasound in the Diagnostics of Metaphyseal Forearm Fractures in Children: A Systematic Review and Cost Calculation.

OBJECTIVES: Metaphyseal forearm fractures are a common occurrence in childhood accounting up to 20% of all pediatric fractures. The standard diagnostic procedure is an x-ray scan. Sonographic examinations could be an alternative that avoids exposition to ionizing radiation and possibly reduces pain, time, and costs. This is a systematic review of clinical studies evaluating ultrasound as a possible alternative to radiographs in diagnosing metaphyseal forearm fractures in children. METHODS: A systematic literature research for diagnostic studies and reviews was conducted in EMBASE, MEDLINE, an

Acute Traumatic Pseudo-pointing Index Finger Due to an Isolated Second Metacarpal-Trapezoid Dislocation: A Case Report.

INTRODUCTION: An isolated second metacarpal dislocation is one of the least reported injuries of the hand. This particular injury assumes clinical significance as they occur subsequent to high energy direct trauma and are usually missed with associated carpal and metacarpal bone fractures. We describe this unique case of an isolated metacarpal-trapezoidal disruption with clinical presentation as the pseudo-pointing index finger and also describe a novel radiological sign to ascertain the rotational deformity of metacarpals on plain radiographs. CASE REPORT: A 32-year male information technolo

A Novel Physeal-Sparing Fixation Technique for Tibial Eminence Fractures in Paediatric Patients.

Introduction Tibial eminence fractures are common knee injuries in paediatric patients. They are an uncommon cause of knee injury that is associated with effusion, and most frequently present in the paediatric population. These injuries affect knee stability. Surgical management is challenging and associated with risks and complications due to the direct relation to the proximal tibial growth plate. We developed a new physeal-sparing technique to fix these fractures while avoiding any violation of the proximal tibial growth plate. Materials and methods This new technique involves transfixing t

The epidemiology of closed reduction for simple elbow dislocations and the incidence of early subsequent open reduction.

BACKGROUND: Simple elbow dislocations are often treated with closed reduction (CR); however, the rate of CR failure and factors that may predict failure have been largely underinvestigated. The objectives of this study were (1) to determine the incidence of elbow dislocations treated by CR in a universal health care system and (2) to identify patient characteristics associated with failed CR, defined as the subsequent need for open reduction. METHODS: Patients ≥16 years old who underwent elbow CR by a physician between 1994 and 2010 were identified from administrative databases. Concurrent el

Iliosacral screw fixation of the unstable pelvic ring injuries.

PURPOSE OF THE STUDY: To report on the early results and possible complications of iliosacral screw fixation in the management of unstable pelvic ring injuries. MATERIAL AND METHODS: One hundred and two unstable pelvic ring injuries were treated using iliosacral screw fixation for posterior pelvic ring disruptions. Closed manipulative reductions of the posterior lesion were attempted for all patients. Open reductions were used in the minority of patients with unsatisfactory closed reductions as assessed fluoroscopically at the time of surgery. Anterior fixations were by means of open reductio

[The Anterior "Triple-/Quadruple" Technique for C1/C2 Trauma in the Elderly: First Experience with 16 Patients].

INTRODUCTION: In geriatric patients the management of odontoid type II fractures is complicated by osteoporosis and atlantoaxial arthritis (spondylarthritis C1/C2) with an increased lever arm. Furthermore, a few of the odontoid fractures are accompanied by an atlas fracture resulting in the "atlantoaxial unhappy triad". Posterior C1/C2 spondylodesis with bilateral Magerl screws and C1 hooks is a strong biomechanical construct, however, the posterior approach is associated with several drawbacks such as increased risk of infection and increased blood loss. In contrast, the anterior bilateral C1

Open fracture of the tibia in children.

The records of eighty-three children who had had an open fracture of the tibial metaphysis or diaphysis between January 1983 and July, 1993 were studied retrospectively. The average duration of follow-up was fourteen months (range, two to seventy-five months). There were twenty-four grade-I, forty grade-II, thirteen grade-IIIA, six grade-IIIB, and no grade-IIIC fractures, according to the classification scheme of Gustilo et al. Sixty patients (72 per cent) had sustained the fracture when they were struck by an automobile, and forty-eight patients (58 per cent) had other associated major injuri

Diaphyseal fractures of the humerus with radial nerve injury: how to manage both fracture and the nerve injury.

In this article, data of nerve and bone recovery were reported in a series of patients treated with open reduction and internal fixation (ORIF) with plate in humeral shaft fractures (HSF) associated with radial nerve palsy (RNP). The authors highlight the role of early surgical intervention for optimal nerve repair and patient's recovery, and to avoid subsequent and more complex surgery to patients. This is a retrospective study of 24 of 31 patients with HSF with RNP (10,1%) out of 308 patients with HSF, treated surgically between 2012 and 2023 in a level I Trauma Center. The mean age was 57 y

Utilization of Surgical Fixation for Distal Radius Fractures in the United States: A Population-Based Cohort Study.

PURPOSE: In contrast to younger adult populations, clinical practice guidelines state that there is no difference in long-term functional outcomes in surgically versus nonoperatively treated distal radius fractures in patients ≥65 years old. In this study, we asked the following questions: (1) has treatment for distal radius fractures in younger and older adult populations changed over time? (2) What patient and surgeon factors are associated with treatment trends? METHODS: Using a national administrative claims database, we identified patients aged ≥18 years with isolated, closed distal radi

Retrospective surgical outcomes of Gartland type III supracondylar humeral fractures with absent pulse in children.

INTRODUCTION: Supracondylar humeral fractures are the most common elbow injuries in children. Gartland type III fractures require urgent treatment due to the high risk of neurovascular injury. Vascular compromise occurs in 5%-15% of cases, most commonly involving the brachial artery, and clinical presentation ranges from complete ischemia ("cold, pale hand") to the "pink pulseless hand" (PPH). MATERIALS AND METHODS: Twelve children (9 males, 3 females) with Gartland type III supracondylar humeral fractures and absence of a palpable radial pulse, treated between April 2023 and September 2025,

Effectiveness of straight arm traction versus operative treatment for displaced paediatric supracondylar humerus fractures: a randomised single-blind controlled, a non-inferiority trial-the STOPUS trial.

BACKGROUND: Supracondylar humeral fractures are common injuries in children and can be associated with high morbidity and lead to lifelong disability. The method of treatment affects the risk of complications and potentially the functional outcome. Closed reduction and percutaneous pinning CRPP has become the most widely used treatment method in high-income countries. In the current literature, both CRPP and lateral straight arm traction have been shown to give good results and reasonable levels of complications. The two methods have never been tested against each other in a randomised trial.
