Medical devices of the head, neck, and spine.

There are many medical devices used for head, neck, and spinal diseases and injuries, and new devices are constantly being introduced. Many of the newest devices are variations on a previous theme. Knowing the specific name of a device is not important. It is important to recognize the presence of a device and to have an understanding of its function as well as to be able to recognize the complications associated with its use. The article discusses the most common and important devices of the head, neck, and spine, including cerebrospinal fluid shunts and the Codman Hakim programmable valve; s

Morphological changes in the normal and neomycin-perfused guinea pig cochlea following chronic prosthetic implantation.

The effects of chronic prosthetic implantation and interval electrical stimulation were studied in the normal and neomycin-perfused cochlea of the guinea pig. One group of guinea pigs was implanted with a multiple-electrode prosthesis in the scala tympani. During a 4-week period, the device was stimulated for 3 hours weekly with a continuous, 1 kHz sinusoidal current of constant intensity. A second group of guinea pigs underwent identical implantation and stimulation except that cochlear perfusion with .1 M neomycin was performed at the time of implantation. Current intensities ranged from .1

Combining Multi-Canal Vestibular Implant Stimulation to Restore the Vestibulo-Ocular Reflex in 3D.

INTRODUCTION: Vestibular implants targeting the semi-circular canals might offer a potential treatment approach for vestibulopathy patients. Electrical stimulation is provided to all three canals to restore gaze stabilization in all three planes of space. This study evaluated the effect of electrically stimulating multiple canals at the same time. It was hypothesized that eye responses of single-canal stimulation could predict eye responses of multi-canal stimulation, using vector summation. This might provide valuable information for future vestibular implant fitting strategies. METHODS: Nin

Accuracy and precision of direct bone conduction measurements.

OBJECTIVES: Variability in Baha® sound processor fittings arise from several sources including the nature of the implant-to-bone transmission and transcranial attenuation in patients with single-sided sensorineural deafness. One method of improving the predictability of Baha fittings is to measure the individual patient's actual bone conduction thresholds via the implant, thereby removing the influence of skin thickness and/or implant location site. METHODOLOGY: One hundred thirty eight adult wearers of the Baha System participated in the study. Direct bone conduction thresholds were obtained

Assessment of a direct acoustic cochlear stimulator.

This study aimed to assess the functional results of a new, active, acoustic-mechanical hearing implant, the Direct Acoustic Cochlear Stimulation Partial Implant (DACS PI), in a preclinical study. The DACS PI is an electromagnetic device fixed to the mastoid by screws and coupled to a standard stapes prosthesis by an artificial incus (AI). The function of the DACS PI-aided reconstruction was assessed by determining: (1) the maximum equivalent sound pressure level (SPL) of the implant, which was obtained from measurements of the volume displacement at the round window in normal and implanted ea

Adaptation to noise in amplitude modulation detection without the medial olivocochlear reflex.

The detection of amplitude modulation (AM) in quiet or in noise improves when the AM carrier is preceded by noise, an effect that has been attributed to the medial olivocochlear reflex (MOCR). We investigate whether this improvement can occur without the MOCR by measuring AM sensitivity for cochlear implant (CI) users, whose MOCR effects are circumvented as a result of the electrical stimulation provided by the CI. AM detection thresholds were measured monaurally for short (50 ms) AM probes presented at the onset (early condition) or delayed by 300 ms (late condition) from the onset of a broad

AI-driven neural implants for vision and hearing: a qualitative study of user perspectives.

Neural implants are being developed to treat various conditions, including sensory impairments such as blindness and deafness. In these technologies there is a growing role for artificial intelligence (AI) to enable interpretation of complex data input. Current users of cochlear implants (CIs) face challenges in noisy environments, prompting the development of AI-driven software for personalized and context-aware noise suppression and speech enhancement. For blindness, an AI-driven cortical visual neural implant (cVNI) for artificial visual perception is under development. Here, AI-driven soft

High Riding Jugular Bulb Protruding Into Tympanic Cavity: Longitudinal Radiologic Study in a Deaf Child.

BACKGROUND: High riding jugular bulb (HRJB) develops after 2 years and is rare at younger ages. High riding jugular bulb sometimes protrudes into the tympanic cavity, which can cause hemorrhagic complications during otologic surgery. CASE PRESENTATION: We describe a congenitally deaf child with bilateral inner ear malformations and a right-sided HRJB on CT at 9 months. This child had undergone left cochlear implantation (CI) at 19 months, and right CI was planned at 6 years. However, we decided not to perform the right CI because preoperative CT images revealed that the right jugular bulb (JB…

MRI of the Internal Auditory Canal, Labyrinth, and Middle Ear: How We Do It.

MRI is firmly established as an essential modality in the imaging of the temporal bone and lateral skull base. It is used to evaluate normal anatomic structures, evaluate for vestibular schwannomas, assess for inflammatory and/or infectious processes, and detect residual and/or recurrent cholesteatoma. It is also extensively used in pre- and postoperative evaluations, particularly in patients with vestibular schwannomas and candidates for cochlear implantation. Nevertheless, despite the widespread use of MRI for these purposes, many radiologists remain unfamiliar with the complex anatomy and e…
