[Therapy of a schwannoma of the vagal nerve--stated at the example of an extensive tumor].

INTRODUCTION: Schwannoma of the vagal nerve are rare and usually accidentally found by exploring a slowly growing and painless neck tumor. CASE: We report on a 49-year-old patient with an extensive schwannoma of the vagal nerve. The tumor grew from the brainstem to the lower neck vessel sheath and destroyed the skull base. The tumor had already involved all nerves of the foramen jugulare. The surgery was performed in cooperation with the neurosurgeons. Initially the cervical part of the tumor was removed by transcervical approach. Then the endocranial part of the tumor was removed by retrosig

[High duplication of an internal jugular vein].

The internal jugular vein is an important landmark in most cervical operations. The surgeon should know the exact position and possible variations to prevent complications. We report on a duplicated internal jugular vein, which is a rare anatomic anomaly and discuss aetiology and appearance of this malformation and the possible affecting of the accessory nerve.

Multiple osteomas in the middle ear.

Osteomas in the middle ear are extremely rare and are generally solitary and small. We present a case of a 28-year-old female patient with multiple middle ear osteomas which arose from the promontory, epitympanum, and lateral semicircular canal. The osteomas were successfully removed by tympanomastoidectomy. This is the first reported case of a patient with middle ear osteomas arising from multiple sites.

Combined approach for otogenic brain abscess.

Otogenic intracranial infections usually require both neurosurgical and otolaryngological surgery. This prospective, non-randomized study investigated the value of combining both surgical procedures. Thirteen patients with otogenic intracranial abscess were treated by mastoidectomy and abscess removal through the same incision between 1993 and 2002. Another 12 patients underwent abscess removal or mastoidectomy followed by the other procedure within 7 days. The clinical features of the patients were compared. Four patients died in our series. All four patients had Glasgow Coma Scale (GCS) scor…

CT detection of facial canal dehiscence and semicircular canal fistula: comparison with surgical findings.

OBJECTIVE: The purpose of this study was to determine the accuracy of high resolution CT (HRCT) in the detection of facial canal dehiscence and semicircular canal fistula, the preoperative evaluation of both of which is clinically very important for ear surgery. MATERIALS AND METHODS: We retrospectively reviewed the HRCT findings in 61 patients who underwent mastoidectomy at Yamagata University between 1989 and 1993. The HRCT images were obtained in the axial and semicoronal planes using 1 mm slice thickness and 1 mm intersection gap. RESULTS: In 46 (75%) of the 61 patients, the HRCT image-b…

An Ameloblastoma in the Middle Ear.

An ameloblastoma is a locally aggressive odontogenic tumour that commonly develops from the odontogenic epithelium within the jawbone. Here we present for the first time a case of a rare primary ameloblastoma in the middle ear cavity, along with some consideration of its treatment and a new classification. A 65-year-old woman presented with a left middle ear cavity tumour. Pathological examination led to the diagnosis of an ameloblastoma. We resected the tumour along with an extensive part of the middle ear mucosa, which made it difficult to have an adequate margin. It is recommended that the

Practicality analysis of the staging system proposed by the Japan Otological Society for acquired middle ear cholesteatoma: A multicenter study of 446 surgical cases in Japan.

OBJECTIVE: The Objective of this study was to assess the practicality of the cholesteatoma staging system proposed by the Japan Otological Society (JOS) for acquired middle ear cholesteatoma (the 2010 JOS staging system). METHODS: Between 2009 and 2010, 446 ears with retraction pocket cholesteatoma underwent primary surgery at 6 institutions in Japan. The extent of cholesteatoma was surgically confirmed, and classified into three stages. RESULTS: The cholesteatoma affected the pars flaccida in 325 ears (73%), the pars tensa in 100 ears (22%), and both regions combined in 21 ears (5%). The he

Postoperative Imaging Findings following Sigmoid Sinus Wall Reconstruction for Pulse Synchronous Tinnitus.

BACKGROUND AND PURPOSE: Transmastoid sigmoid sinus wall reconstruction is a surgical technique increasingly used for the treatment of pulsatile tinnitus arising from sigmoid sinus wall anomalies. The imaging appearance of the temporal bone following this procedure has not been well-characterized. The purpose of this study was to evaluate the postoperative imaging appearance in a group of patients who underwent this procedure. MATERIALS AND METHODS: The medical records of 40 consecutive patients who underwent transmastoid sigmoid sinus wall reconstruction were reviewed. Thirteen of 40 patients…

An assessment of skeletal craniofacial asymmetry in South Indian population.

The present study was undertaken to assess the skeletal craniofacial asymmetry in South Indian population by a posteroanterior cephalometric radiographic method. The skeletal craniofacial structures on one side of the face were compared with that of the other, by drawing various triangles representing different craniofacial regions. The sample consisted of 60 subjects (30 males and 30 females) aged between 18 to 25 years, who were mainly dental college students from South India. Overall 52 X-rays were obtained, with four errors each in the male and the female groups. The results revealed that

Lemierre syndrome: two cases and a review.

OBJECTIVES: Lemierre syndrome is usually caused by an acute oropharyngeal infection in previously healthy young adults, resulting in thrombophlebitis of the internal jugular vein, leading to metastatic septic embolization and bacteraemia. The usual organism is Fusobacterium necrophorum. Lemierre syndrome, not so long ago labeled as the "forgotten disease," is on the rise. Today with increasing antibiotic-resistant organisms, and decreasing awareness of the syndrome, subsequent re-emergence of this "forgotten disease" is becoming more common in clinical settings. Lemierre syndrome has significa

Otomastoiditis caused by Rhodococcus equi in a patient with AIDS.

Rhodococcus equi is a well-recognized pathogen in veterinary medicine and a rare but well-documented cause of cavitary pneumonia in immunocompromised patients. Most cases of Rhodococcus equi infections in these patients involve the lungs. Otomastoiditis due to Rhodococcus equi is rare, and disseminated Rhodococcus equi with otomastoiditis has never been reported. We report a case of otomastoiditis with systemic dissemination due to Rhodococcus equi in a patient with AIDS.

Complications and their management in tympanomastoid surgery.

Chronic diseases of the ear and their surgical treatment can endanger the delicate structures of the temporal bone in close relationship with the middle ear. The experience and expertise of the otologic surgeon, based on perfect knowledge of the complex anatomy of the whole temporal bone, are unfortunately built up partly through the management of complications encountered in tympanomastoid surgery and may require knowledge of basic neuro-otologic procedures. Paralysis and paresis of the facial nerve and labyrinthine, and dural and vascular injuries may have consequences not only in the final …

Systematic approach to electrode insertion in the ossified cochlea.

Ossification of the fluid spaces of the cochlea occurs often in candidates for cochlear implantation, especially children. When noted before surgery on computerized tomography, ossification previously was thought to contraindicate cochlear implantation because of possible mechanical obstruction and uncertainty about the level of function that could be achieved by stimulating an ossified cochlea. However, during the preceding 6 years, techniques have been developed that permit implantation in ossified cochleas. We present a systematic approach that has been developed to treat the three clinical
