Predicting and preventing root resorption: Part I. Diagnostic factors.

The purpose of this study was to determine whether it would be possible to identify pretreatment factors that will allow the clinician to predict the incidence, location, and severity of root resorption before the commencement of orthodontic treatment. The records of 868 patients who were treated with full, fixed edgewise appliances were obtained from 6 private offices; full-mouth periapical radiographs were used to accurately assess apical root resorption from first molar to first molar in both arches. The results showed that resorption occurs primarily in the maxillary anterior teeth, averag

Predicting and preventing root resorption: Part II. Treatment factors.

The main objective of this study was to determine which treatment factors are most clearly identified with external apical root resorption that is detectable on periapical radiographs at the end of orthodontic treatment. The records of 868 patients who completed fixed, edgewise treatment from experienced clinicians in private practice were examined. The horizontal and vertical displacement of the root apex of the maxillary central incisor was measured on cephalometric radiographs. Patients who underwent first premolar extraction therapy had more resorption than those patients who had no extrac

Pharyngeal airway space changes after counterclockwise rotation of the maxillomandibular complex.

The purpose of this study was to evaluate the effects of double-jaw surgery with counterclockwise rotation of the maxillomandibular complex on the pharyngeal airway space and velopharyngeal anatomy in patients with high occlusal plane facial morphology. Fifty patients (22 men, 28 women) with high occlusal plane facial morphology underwent double-jaw surgery with counterclockwise rotation of the maxillomandibular complex. The patients were divided into 2 groups: group 1, 30 patients (8 men, 22 women) who underwent maxillary and mandibular advancement and group 2, 20 patients (14 men, 6 women) w

Eruption of third permanent molars after the extraction of second permanent molars. Part 1: Assessment of third molar position and size.

The eruptive path of third molars after extraction of second molars was examined in 63 patients. Panoramic radiographs from the start and the end of active treatment and 3 or more years after treatment were assessed. Study models were used to compare the size of the second and third molars and to assess the final position of the third molars. All third molars erupted; none became impacted. During eruption, maxillary third molar crowns uprighted and maintained their angulation as they came into occlusion. Mandibular third molar crowns continued to upright significantly mesiodistally after activ

A unique treatment approach for maxillary canine-lateral incisor transposition.

Maxillary canine-lateral incisor transposition [Mx3-2] is a relatively rare anomaly, with both dental and facial esthetic implications. A challenging treatment approach for selected cases is described in detail, including the mechanics and sequencing used to align the completely transposed teeth to their normal anatomic position in the arch. The long-term esthetic outcome warrants the longer treatment time and greater efforts required in the successful treatment of this unusual abnormality.

Intrusion of overerupted molars by corticotomy and magnets.

Although posterior tooth intrusion in an adult patient is a difficult procedure, it can be achieved without extruding the adjacent teeth by performing a corticotomy and using magnets. In carrying out this procedure on 2 adult patients whose molars had overerupted due to the early loss of antagonists, tooth movement was rapidly achieved without discomfort or side effects.

A mechanism of noncontinuous supraosseous tooth eruption.

Numerous theories have been propounded to explain the various aspects of tooth eruption, but no general consensus exists about some of the underlying mechanisms that govern these aspects. An important unresolved issue is the source of the motive forces that displace teeth. We proposed that supraosseous eruptive forces are generated by tooth socket distortions caused by functional jaw deformations. Previous studies used basic equations of static equilibrium to demonstrate that the concomitant stretching of the oblique periodontal ligament (PDL) fibers give rise to forces that may cause supraoss

Deciduous canine and permanent lateral incisor differential root resorption.

When a permanent maxillary canine erupts apical to the permanent lateral incisor and the deciduous canine, resorption typically takes place only on the deciduous canine root. An understanding of this differential resorption could provide insight into the reasons for excessive iatrogenic root resorption during orthodontic tooth movement. The purpose of the present study was to examine the response of roots of permanent lateral incisors and deciduous canines to simulated resorption, and to acid and enzyme attack, reflecting the physiologic environment of an erupting permanent canine. Groups of m

Periodontal status of mandibular incisors after pronounced orthodontic advancement during adolescence: a follow-up evaluation.

The purpose of this study was to analyze whether pronounced orthodontic advancement of the mandibular incisors during Class II correction in the mixed dentition results in gingival recession. Through mandibular superimposition of the pretreatment and posttreatment cephalograms of 67 Class II patients who were treated with reverse headgear to the mandibular dentition, 45 patients with a minimum of a 1-mm advancement of the cementoenamel junction (CEJ; mean, 2.18 +/- 0.87) and a minimum of a 2-mm advancement of the incisal edge (mean, 3.87 +/- 1.34) were identified. Eruption of third permanent molars after the extraction of second permanent molars. Part 2: Functional occlusion and periodontal status.

Functional occlusion and periodontal health were investigated after orthodontic treatment that involved extraction of second molars and after eruption of the third molars in 37 patients (25 female, 12 male). The mean age was 21 years 9 months (range, 16 years 1 month-30 years 5 months). The third permanent molars invariably erupted into a position that maintained good functional occlusion. Sixty-three percent of patients had canine guidance in lateral excursion; the remainder of patients had satisfactory group function. There were only 2 non-working side interferences in the sample, 1 from a r

Maxillary tooth transpositions: characteristic features and accompanying dental anomalies.

Transposition is a dental anomaly manifested by a positional interchange of 2 permanent teeth. The maxillary permanent canine usually transposes with the first premolar and occasionally with the lateral incisor. The records of 65 orthodontically treated individuals with maxillary tooth transpositions (40 females and 25 males) with an age range of 9 to 25 years (mean age, 13.4 years) were studied to determine the distribution of transposition and to evaluate the accompanying dental anomalies. Thirty-six individuals (55%) had a transposition of the maxillary canines and first premolars, 27 (42%)

Incorporating retained deciduous teeth in orthodontic therapy.

While patients with hypodontia of the permanent dentition constitute a relatively small portion of the general population, a significant percentage of these patients will require orthodontic therapy. This is a case report in which the family opted to maintain the retained deciduous teeth rather than seek combined orthodontic-prosthodontic treatment.

The effect of preemptive and/or postoperative ibuprofen therapy for orthodontic pain.

The control of pain during orthodontic treatment is of vital interest to both clinicians and patients. Surprisingly, there has been limited research into the control of orthodontic pain, and there is no standard of care for controlling this discomfort. The purpose of this study was to compare the effectiveness of preemptive ibuprofen therapy, postoperative ibuprofen therapy, and a combination of the 2 therapies. Forty-one orthodontic patients aged 9 years 3 months to 16 years 11 months who were to undergo separator placement were enrolled in this prospective study. Patients were randomly assig

Changes in the masticatory cycle following treatment of posterior unilateral crossbite in children.

In a previous study, we established that young children with unilateral posterior crossbite have a longer mandibular ramus and more superiorly and posteriorly positioned condyles on the crossbite side. In this study, we evaluated chewing cycle shape and duration in 14 of the patients before treatment, and we looked for changes in cycle shape and duration 6 months after treatment with rapid palatal expansion. Mandibular kinematics was recorded while chewing gum using an optoelectric recording system at 100 Hz. Subjects were asked to chew normally for 20 cycles, chew on the crossbite side only f
