Fifty-four previously reported cases of otogenic pneumocephalus were analyzed in addition to five new cases which are presented in detail. Forty-one males and 18 females were included with 95% of the patients being over 12 years of age. The most common presenting symptom was headache, and the ventricular system was the intracranial space most commonly involved. Tension pneumocephalus was present in 40 (66%) cases. Trauma (36%) was the most common etiologic factor, while otitis media (30%), otologic surgery (30%), and congenital defects (2%) accounted for the rest. The overall mortality was 12%

I have reviewed a group of otologic patients who represent controversial problems in management. Decisions for revisional otologic surgery must be based on each patient's individual findings. Statistical reviews are of little help in deciding which patient will benefit. Physicians managing patients with poor results after surgery for hearing, infection, or vertigo must obtain accurate audiologic, electronystagmographic, and laboratory data before deciding on revisional surgery. The overall interests of the patient are sometimes best served by avoiding further surgical insults to the ear.

The ear surgery of the past 40 years (1936-1976) has been reviewed to demonstrate how this has influenced the development of modern microsurgery and the ear surgery of the future. The ear surgeon of tomorrow must have complete knowledge of the surgical anatomy and surgical pathology of the temporal bone, that pathology which has been produced by my generation of ear surgeons, and all techniques of all surgeons, for there is no single approach to surgery of the temporal bone. Only with complete and total knowledge of the temporal bone obtained by personal dissection can the surgeon succeed in t

OBJECTIVE: The purpose of this study is to analyse the effect of half canal wall down tympanomastoidectomy in the treatment of chronic otitis media or cholesteatoma.
METHOD: In this retrospective study, the half canal wall down tympanomastoidectomy technique was used at our hospital for chronic otitis media or cholesteatoma removal in 265 adult patients, representing 271 operated ears, with an average follow-up time of 8.4 years.
RESULTS: The post-operative cavities were slightly wider and straighter in 91.9 per cent of the ears. Fifteen per cent of the patients needed cavity cleaning every …

OBJECTIVE: To assess the reliability of diffusion-weighted magnetic resonance imaging in differentiating recurrent cholesteatoma from granulation tissue after intact canal wall mastoidectomy.
METHODS: A prospective study was conducted of 56 consecutive patients with suspected cholesteatoma recurrence after intact canal wall mastoidectomy who underwent diffusion-weighted imaging and delayed contrast magnetic resonance imaging of the temporal bone. The final diagnosis was recurrence in 38 patients and granulation tissue in 18 patients.
RESULTS: Cholesteatoma detection on diffusion-weighted ima…

The first part of this review of the temporal bone discussed anatomy of the temporal bone as well as inflammatory and neoplastic processes in the temporal bone region (1). This second part will first discuss trauma to the temporal bone and posttraumatic complications. The indications for common surgical procedures performed in the temporal bone and their postoperative imaging appearance are then presented. Finally, a few noninflammatory nonneoplastic entities involving the temporal bone are reviewed. They are relatively uncommon diagnoses compared with infectious or inflammatory diseases. Howe

OBJECTIVE: To estimate the incidence of tympanogenic labyrinthitis ossificans.
METHODS: The records of patients treated with mastoidectomy for various tympanogenic aetiologies from January 2007 to December 2011 were retrospectively reviewed. Patients whose high-resolution computed tomography scans showed evidence of labyrinthine calcification of the temporal bone were enrolled. Patients with a history of head and neck cancer, meningitis, and otosclerosis, and patients with cochlear implants, were excluded from this study.
RESULTS: A total of 195 patients were enrolled in this study; 4 of the…

PURPOSE: The aim of the study was to evaluate various presentations and treatment options for spontaneous cerebrospinal fluid (CSF) leakage originating in the temporal bone.
MATERIALS AND METHODS: Clinical data and imaging results for 18 ears (15 patients) presenting with spontaneous CSF leakage originating in the temporal bone were reviewed. Average follow-up period was 13.5 months. The main outcome measure was presence of persistent CSF leak postoperatively. A standard postauricular mastoidectomy was performed.
RESULTS: Fifteen patients diagnosed with spontaneous CSF leakage over an 8-year…

OBJECTIVES/HYPOTHESIS: To describe the results of temporal bone surgery in managing advanced or recurrent tumors of the parotid gland.
STUDY DESIGN: Retrospective review.
METHODS: Patients with advanced or recurrent tumors involving the parotid gland who underwent either mastoidectomy or temporal bone resection were reviewed for treatment outcomes.
RESULTS: Forty-nine patients were identified who required either mastoidectomy (n = 33) or temporal bone resection (n = 16) for malignancies involving the parotid gland. Facial nerve sacrifice was required in 35 patients (71.4%). Perineural invas…

We report the first case of isolated stapedial dislocation caused by indirect head trauma, and present imaging and surgical findings in the case of a 25-year-old woman who suffered hearing loss and dizziness after head trauma caused by a traffic accident. The pure tone average was 60 dB, with an air-bone gap of 50 dB. The stapedial reflex was positive with the probe on the affected ear. Computed tomography scans revealed a longitudinal fracture of the temporal bone and a dislocated stapedial superstructure in the tympanic cavity, adhering to the tympanic membrane. During surgery, it was found

OBJECTIVE: To evaluate the short- (6 mo) and long-term (5 yr) outcomes of modified Bondy technique, with particular reference to hearing results.
STUDY DESIGN: Retrospective study.
SETTING: Quaternary referral otology and cranial base center.
PATIENTS: Two hundred thirty ears of 222 patients were operated on, using a modified Bondy technique between 1983 and 2005. All patients had primary acquired epitympanic cholesteatomas with intact pars tensa and intact ossicular chain in normal or good-hearing ear. Preoperative audiometric results revealed a mean air conduction pure-tone average of 27.

STUDY DESIGN: Retrospective chart review at a tertiary referral center.
METHODS: Conchal cartilage, shaped as a shield, was used to replace the entire tympanic membrane and reconstruct the ossicular chain in patients with an absent incus and diminished space between the stapes superstructure and malleolar manubrium.
RESULTS: Between January 1998 and June 2005, 52 patients were treated. The mean age was 32.4 years (range, 7 to 72 years). The mean follow-up was 24 months (range, 12 to 36 months). Gra
