INTRODUCTION: Minimally invasive cardiac surgery through a small transverse sternotomy is a new promising technique that can be considered an alternative in most cases to aortic valve replacement thus reducing surgical trauma and subsequent time of hospitalization. The need to avoid the risks associated with femoro-femoral bypass has lead to the interest in aortic valve replacement (AVR) operations without femoral vessels cannulation. We want to emphasize a few important points of our technique, which differs somewhat from the one applied by Cosgrove and associates.
OBJECTIVE: This study deta

BACKGROUND: Ventricular tachycardia (VT) ablation is a high-risk procedure that often requires induction of VT for activation and entrainment mapping. VT induction, particularly those with rapid cycle lengths tends to predispose patients to acute hemodynamic instability, sometimes necessitating mechanical circulatory support (MCS) to maintain perfusion, especially during longer procedures.
PURPOSE: We sought to evaluate clinical and procedural outcomes in patients requiring MCS during VT ablation.
METHODS: We queried the 2017-2021 National Inpatient Sample (NIS) database and identified patie

BACKGROUND: Intracardiac echocardiography (ICE)-guided Zero-Fluoroscopy Workflow Group has been described in the literature. However, robust evidence simultaneously addressing procedural safety, efficiency, and operator experience remains limited. This study aims to systematically evaluate the procedural efficacy, intraoperative safety, and impact on operator experience of this technique.
OBJECTIVE: To compare the ICE-guided Zero-Fluoroscopy Workflow with Conventional fluoroscopy-guided workflow in the context of radiofrequency catheter ablation for atrial fibrillation (AF), and to quantify t

BACKGROUND: Though uncommon, pericardial effusion and cardiac tamponade are serious complications of left atrial appendage closure (LAAC). There are few data related to delayed pericardial effusions from this procedure.
METHODS: This is a single-center prospective analysis of 369 patients who underwent LAAC from December 2016 to March 2022 at a large teaching hospital. We compared patients who developed effusion (n = 5) to patients who did not (n = 364) to determine if there were any factors that predispose patients to developing acute (AEs) or delayed pericardial effusions (DEs). We compared

OBJECTIVE: In acute type A dissection long-term results of conservative aortic root surgery were compared with the outcome of primary valve and/or root replacement.
METHODS: Between 1985 and 1995, 199 patients (mean age 59 years, 154 men) were operated on. The aortic root was involved in the dissection process and valve incompetence of varying degree was present without exception. Replacement of a proximal aortic segment was standard procedure in all patients. The aortic valve was preserved in 126 patients: commissural suture resuspension (12 patients), root reconstruction with GRF-glue (gela

From January 1992 to June 1993, 100 consecutive patients with clinically documented paroxysmal supraventricular tachycardias underwent radiofrequency catheter ablation. Group 1 consisted of 46 patients (male:female = 9:37, age: 46 +/- 13 years) with slow-fast atrioventricular nodal reentrant tachycardia. Radiofrequency current was aimed at the slow pathway area which was identified by both anatomical and electrophysiological methods. A mean application of 8 +/- 9 was delivered at a mean power of 22 +/- 4 watts with a mean duration of 21 +/- 3 seconds. Selective ablation of slow pathway conduct

BACKGROUND: Transcatheter left atrial appendage closure (T-LAAC) represents an effective alternative to long-term anticoagulation in patients with atrial fibrillation (AF) and contraindications to oral anticoagulants. Transoesophageal echocardiography (TEE) is the standard imaging modality for procedural guidance, while micro-TEE may allow adequate imaging under conscious sedation, potentially simplifying procedural workflow. This study aimed to evaluate the efficacy and safety of micro-TEE compared with standard TEE guidance during T-LAAC.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) often leads to left ventricular (LV) overload, particularly in patients with biventricular cardiogenic shock. This increased afterload can hinder cardiac muscle recovery and cause pulmonary edema, thereby impairing systemic oxygenation. Although devices such as the Impella and intra-aortic balloon pump (IABP) are used to manage this issue, they require complex procedures and pose risks for bleeding and vascular complications. A transseptal approach for left atrial decompression using a single multistage femoral venous cannula provides

The relative safety and efficacy of aspirin versus dual antiplatelet therapy (DAPT; aspirin+clopidogrel) in patients who underwent transcatheter aortic valve implantation (TAVI) and did not have a long-term indication for oral anticoagulation remains controversial. Digital databases were searched to identify relevant articles. The major safety end point was bleeding, while the efficacy end points included after-TAVI ischemic and thrombotic events. Data were analyzed using a random effect model to calculate the pooled unadjusted odds ratio (OR) for dichotomous outcomes. Eleven studies comprisin

BACKGROUND: Bleeding is the most frequent complication associated with extracorporeal membrane oxygenation (ECMO) support in critically ill patients. Nonetheless, risk factors for bleeding have been poorly described especially those associated with coagulation anomalies and anticoagulant therapy during ECMO support. The aim of this study is to describe bleeding complications in critically ill patients undergoing ECMO and to identify risk factors for bleeding events.
METHODS: We retrospectively analysed ICU charts of adults who received either veno-venous (VV) or veno-arterial (VA) ECMO suppor

BACKGROUND: Raised intra-abdominal pressure (IAP) accompanied by evidence of organ dysfunction constitutes abdominal compartment syndrome (ACS). The ACS is now becoming an increasingly recognised fatal entity in the critically ill surgical and traumatized patients receiving critical care. The objectives were to determine the frequency of abdominal compartment syndrome (ACS) in critically ill surgical and traumatised patients and to identify the risk factors associated with its development in our patients.
METHODS: This descriptive study was conducted at Department of Surgery, Pakistan Institu

BACKGROUND: For patients with complex left ventricular outflow tract obstruction, including hypoplastic aortic anulus with or without severe diffuse subaortic stenosis, various aortoventriculoplasty procedures (e.g., Konno procedure and its modifications; extended aortic allograft root replacement) are important management options. In younger patients, however, reoperation for valve replacement is inevitably required, and anticoagulation issues pose additional problems. The pulmonary autograft provides a promising option for aortic valve replacement as part of the aortoventriculoplasty procedu

A 58-year-old man presented with tamponade and underwent an emergency pericardiocentesis. We made the diagnosis of methicillin-resistant Staphylococcus aureus pericarditis based on culture results and treated the patient with pericardial drainage and antibiotics as the first-line therapy. After temporary relief, reaccumulation of effusion developed. We successfully created a pericardial window using thoracotomy, and the patient's postoperative course was uneventful. Methicillin-resistant Staphylococcus aureus pericarditis is an extremely rare and life-threatening illness. No consensus exists c
