Effect of Preoperative Nutritional Status on Postoperative Functional Recovery of Hip Joint in Elderly Patients with Intertrochanteric Fractures.

OBJECTIVE: The state of nutrition of senior patients with intertrochanteric fracture of femur before operation affects the patients' tolerance to the operation, the body recovery, healing of the wound and clinical prognosis. For these patients, the poor state of nutrition may lengthen the time of being in hospital, leading to poor hip recovery and clinical outcome. But currently, the relationship between functional recovery of hip joint after operation of intertrochanteric fracture in elderly patients and camp condition has not been reported. To investigate the effect of preoperative nutritiona

Is new always better: comparison of the femoral neck system and the dynamic hip screw in the treatment of femoral neck fractures.

BACKGROUND: Hip fractures in the elderly population are common and the number of patients is rising. For young and geriatric patients with undisplaced fractures osteosynthesis is the primary type of treatment. The dynamic hip screw (DHS) is around for many years and proved its value especially in displaced fractures. Since 2018 the femoral neck system (FNS) is available as an alternative showing promising biomechanical results. The aim of this study is to evaluate clinical results of the FNS and compare it to the DHS. MATERIALS AND METHODS: Patients older than 18 years with Garden I-IV fractu

Fractures of the femoral neck: a review and personal statement.

EPIDEMIOLOGY: The number of hip fractures will increase enormously in the decades to come as will the cost of treatment of these patients do. In the USA the annual cost has estimated to be nearly $10 billion. Hip fractures, therefore, represent an enormous socio-economic and medical problem and challenge (orthopaedic) surgeons an anaesthetists to find the cheapest and most effective way to treat them. At the same time the search for preventive measures should be continued. Biphosphonates and hip protectors seem to be able to decrease the risk of suffering a hip fracture with 50%. CLASSIFICATI

Complications associated with volar locking plate fixation for distal radius fractures in 1955 cases: A multicentre retrospective study.

PURPOSE: Since volar locking plates (VLPs) have the benefits of more stable fixation and fewer complications, VLP osteosynthesis is now the preferred osteosynthesis method in the operative management of distal radius fractures (DRF). Along with the increases in operative management of VLP, the character and frequency of complications have changed. Thus, this multicentre study aimed to identify the characteristics of patients with DRFs who were treated with VLP fixation, describe the complication types and rates related to the procedure, and compare the results with those found in the literatur

Free tissue coverage of chronic traumatic wounds of the lower leg.

Thirty-eight consecutive patients who underwent 42 free flaps for chronic wounds of the lower leg were identified over an 11-year period. All wounds were open for a minimum of 1 month (mean, 40 months; median, 8 months; range, 1 month to 30 years). The average age was 37 years (range, 7 to 68 years), there were 31 male patients and seven female patients, and the average follow-up time was 30 months (range, 12 to 72 months). The original injury was an open fracture in 28 patients, wound dehiscence after open reduction and internal fixation of a closed fracture in nine patients, and a shrapnel w

Analgesic efficacy of flupirtine in primary care of patients with osteoporosis related pain. A multivariate analysis.

OBJECTIVE: Although chronic pain in elderly patients with osteoporosis is extremely common it has rarely been addressed in pharmacotherapy studies. The efficacy and tolerability of flupirtine (CAS 56995-20-1, Trancopal Dolo) up to 600 mg/day was investigated under daily practice conditions. DESIGN: This was an open-label, multicentre, prospective, observational phase IV study on 869 patients performed in 290 practices (mainly orthopedists) throughout Germany. MAIN OUTCOME MEASURE: Decrease in pain scores on a visual analogue scale (VAS, from 0 = none to 10 = maximum) after an average 3-week

Dual mobility versus standard cups in total hip replacement for displaced femoral neck fractures (Duality): an international, multicentre, randomised, controlled, superiority trial.

BACKGROUND: Dislocation is the most common early surgical complication in patients with hip fractures treated with total hip replacement (THR). Dual mobility total hip replacement (DM-THR) was developed to increase joint stability, but no randomised trial has investigated its efficacy and safety. We aimed to compare the risk of dislocation in patients receiving DM-THR versus conventional THR. METHODS: We included people aged 65 years or older with a displaced femoral neck fracture attending 20 Swedish and 24 UK hospitals in this pragmatic, registry-based, randomised superiority trial. Is only Anterior Stabilization Enough in Three-Column Injury of Subaxial Cervical Spine? - A Long-Term Retrospective Analysis of 78 Patients.

STUDY DESIGN: This was a retrospective study. PURPOSE: The purpose was to retrospectively evaluate long-term outcome of anterior stabilization in three-column injury of the subaxial cervical spine. OVERVIEW OF LITERATURE: Literature shows varied results regarding the approach to be chosen. Most studies prefer a combined approach since biomechanically forms more stable construct. The isolated posterior approach is preferred by many as it is easy to reduce and fix three-column injuries. There are very few studies which show the isolated anterior approach to be better than the other two. MATER

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

Delayed Presentation of Complete Ileal Transection Following Blunt Trauma Abdomen: A Condition to Cognize.

Isolated small bowel perforation is a rare presentation of blunt abdominal trauma, and most cases present immediately following the trauma. Delayed presentation of such cases beyond one week of trauma is extremely rare, and various pathophysiological mechanisms were described for the same. We present a 20-year-old male patient who sustained blunt abdominal and pelvic trauma, underwent open reduction and internal fixation for right acetabular fracture, and later developed features of acute peritonitis after one month. On laparotomy, complete terminal ileal transection was found and an ileostomy

Acute Esophageal Necrosis Secondary to Gastric Volvulus Presenting With Massive Aspiration of Upper Gastrointestinal Contents: A Case Report.

Acute esophageal necrosis (AEN), or black esophagus, is a rare endoscopic syndrome typically encountered in critically ill patients. AEN associated with gastric volvulus is particularly uncommon and may be underrecognized when volvulus appears clinically asymptomatic. An 87-year-old woman admitted for pelvic fractures was incidentally found on CT to have a large paraesophageal hernia with organoaxial gastric volvulus and suspected gastric outlet obstruction, without gastrointestinal symptoms. She developed acute hypoxemic respiratory failure; during emergent intubation, a large volume of upper

First-time traumatic anterior shoulder dislocation: Approach for the primary health care physician.

Traumatic anterior shoulder dislocation is a very common injury encountered in emergency rooms as well as in the primary health care physician's office. This injury occurs either in the setting of competitive or recreational sports injuries or as a high-energy injury during a fall or a road traffic accident. Common complications such as a recurrent dislocation can be predicted, monitored and prevented. Early appropriate treatment of associated cuff tears or fractures is associated with improved outcomes. There is a plethora of literature on the assessment and management of the primary anterior

Oligosegmental correction of post-traumatic thoracolumbar angular kyphosis.

Seventeen patients with rigid throacolumbar angular kyphosis due to neglected fractures or dislocations were treated by a standardized single-stage monosegmental or bisegmental anterior discectomy and posterior closing extension wedge osteotomy. The two- or three-level reduction-fixation (RF) instrumentation was used posteriorly for correction and fixation. Choosing angled pedicle screws according to preoperative measurements, the method can always correct the kyphosis to the approximate sagittal curvature that is planned to create preoperatively. The average precorrection of thoracolumbar kyp
