Radial neck fractures in children treated with the centromedullary Métaizeau technique.

BACKGROUND: Radial neck fracture in children is infrequent but when not managed accurately can lead to complications. Different methods have been developed for the reduction and fixation of this fracture. The purpose of this retrospective study is to evaluate our results using the Métaizeau centromedullary technique. MATERIALS AND METHOD: Our series comprises 19 children with displaced radial neck fractures treated in our institution in the period 2004-2008. One patient was excluded because of a very complex fracture dislocation of the elbow (exclusion criteria). Two patients refused to parti

Elastic stable intramedullary nailing is best for mid-shaft clavicular fractures without comminution: results in 60 patients.

INTRODUCTION: Elastic stable intramedullary nailing (ESIN) of displaced mid-shaft clavicular fractures is a minimally invasive technique which was reported to be an easy procedure with low complication rates, good cosmetic and functional results, restoration of clavicular length and fast return to daily activities. Recent studies, however, also report on higher complication rates and specific problems with the use of this technique. This prospective study compares ESIN with non-operative treatment of displaced mid-shaft clavicular fractures. METHODS: Between December 2003 and August 2007, 120

[Specific features with of intramedullary osteosynthesis with blocking in fractures of the shin bones at different levels].

The results of closed intramedullary osteosynthesis with blocking were analyzed in 180 patients with open (in 22) and closed (in 158 patients) fractures of shin bones fulfilled within the period from 3 days to 3.5 months after trauma. In 16 patients the intramedullary canal was drilled out. The exact reposition of the shin-bone in closed intramedullary osteosynthesis was obtained in 142 patients: the terms of consolidation in this case were (6.4+/-1.3) months. The examination of all the patients in the period from 6 months to 2 years after operation has shown consolidation of the shin-bone fra

Avascular necrosis of femoral head after gamma-nailing for unstable intertrochanteric fractures.

We reported on 7 cases of avascular necrosis of the femoral head after treatment of an unstable intertrochanteric fracture with the Asian Pacific gamma-nail. The incidence was about 1.16% (7 of 604) in our series. Good reduction and good implant position were achieved in all 7 men. Avascular necrosis was found about 6 months to 3 years after the initial operation, and all the fractures were solidly united at the final diagnosis. The possible etiologies were initial high energy trauma and combining basal neck fracture and iatrogenic damage of the blood supply to the femoral head.

Huckstep intramedullary nailing for non-union of humeral shaft fractures.

BACKGROUND: Various techniques of internal fixation for non-union of humeral shaft fractures have been attempted, leading to union rates of between 50 and 90% with persisting non-union causing pain and disability. Some of these techniques have led to shoulder and elbow dysfunction. METHODS: Eight patients treated with Huckstep nail fixation for humeral shaft non-union were reviewed and the rate of union was determined. Elbow and shoulder function were assessed as well as the presence of pain or disability. RESULTS: Union was achieved in seven of the eight patients (87.5%). Four patients had

Late open nailing for neglected femoral shaft fractures.

The results of open nailing in 14 cases of neglected closed femoral shaft fractures were reviewed. The mean patient's age was 28 years (range, 17-50). The mean delay to definitive treatment was 4.6 months (range 2-10). All fractures were malaligned with an average shortening of 4.9 cm (range 2.5-6.5). All cases underwent one-stage open nailing using the Küntscher nail fixation. The femur was shortened at the fractured end of one or both fragments to accomplish reduction in nine cases. The mean length of the bone resected was 9 mm (range 5-15). Only local callus was used for bone grafting in al

Barriers to Discharge of Hip Fracture Patients From An Academic Hospital: A Retrospective Data Analysis.

INTRODUCTION: Adherence to best practices for care of hip fracture patients is fundamental to decreasing morbidity and mortality in older adults. This includes timely transfer from the hospital to rehabilitation soon after their surgical care. Hospitals experience challenges in implementing several best practices. We examined the potential barriers associated with timely discharge for patients who underwent a hip fracture surgery in an academic hospital in Ontario, Canada. METHODS: We conducted a retrospective cross-sectional review of a local database. We used descriptive statistics to chara

What Are the Functional Results and Complications With Long Stem Hemiarthroplasty in Patients With Metastases to the Proximal Femur?

BACKGROUND: Traditional treatments for pathological fractures of the proximal femur resulting from metastatic bone disease include fixation with intramedullary nailing supplemented with polymethylmethacrylate, osteosynthesis with a plate-screw construct and polymethylmethacrylate, or endoprosthetic reconstruction. Despite the frequent practice of these treatments, treatment outcomes have not been rigorously compared. In addition, very few studies examine specific approaches to endoprosthetic reconstruction such as long stem hemiarthroplasty. QUESTIONS/PURPOSES: This study examines survival, f

Open distal tibial shaft fractures: a retrospective comparison of medial plate versus nail fixation.

PURPOSE: Studies comparing open reduction internal fixation (ORIF) vs. intramedullary nailing (IMN) for distal tibia shaft fractures focus upon closed injuries containing small patient series with open fractures. As such, complication rates for open fractures are unknown. To characterize complications associated with ORIF vs. IMN, we compared complications based on surgical approach in a large patient series of open distal tibia shaft fractures. METHODS: Through retrospective analysis at an urban level I trauma center, 180 IMN and 36 ORIF patients with open distal tibia fractures from 2002 to

What's New in Osteogenesis Imperfecta.

UNLABELLED: Osteogenesis imperfecta (OI) is a diverse group of genetic disorders mainly caused by pathogenic variants in COL1A1 and COL1A2 that interfere with type I collagen production, resulting in bone fragility and multisystem issues. Since 2019, the understanding and treatment of OI have improved considerably. Advances in molecular genetics have broadened the OI classification to 22 types, with new variants affecting pathways beyond collagen synthesis, including osteoblast differentiation and bone mineralization. Bisphosphonates continue to be the primary pharmacologic treatment, effectiv

Treatment of metastatic bone lesions in the upper extremity: indications for surgery.

Pathological fractures caused by metastatic malignant disease have been the subject of increasing interest in recent years. This article describes our experience with the treatment of metastatic bone disease of the upper extremity and our attempt to clarify the indications for different surgical procedures. Of 53 patients with metastatic lesions to the upper extremity, 20 who had been surgically treated were analyzed retrospectively. These comprised 13 men and 7 women with a mean patient age of 62 years. The most common primary tumors to metastasize were lung and liver, with the humerus involv

Suprapatellar Nailing With Condylar Screw Support in Complex Proximal Tibia Fractures: Technical Experience from a Case Series.

INTRODUCTION: Complex intra-articular proximal tibia fractures (Schatzker types V and VI) are difficult to manage due to articular involvement, malalignment, and soft-tissue compromise. Conventional plating techniques carry risks of infection and soft-tissue morbidity. Suprapatellar intramedullary nailing (IMN), combined with condylar screws, offers a minimally invasive option with potential biomechanical and clinical advantages. MATERIALS AND METHODS: A prospective case series was conducted at a tertiary trauma center in Chennai, India, including 10 consecutive adults with Schatzker type V (

Early outcomes of magnetic intramedullary compression nailing for humeral fractures.

PURPOSE: The optimal treatment protocol for humeral shaft fractures at risk for nonunions is controversial. Here, we aim to describe magnetic intramedullary compression nailing as an option for these challenging scenarios and to evaluate its clinical and radiographic outcomes. METHODS: This retrospective case series was performed at an urban university-based level-1 trauma center. Patients aged 18-65 who underwent fixation of their at-risk humerus shaft fracture using the PRECICE nail were included in this investigation. These fractures are characterized by a persistent distraction gap, minim

What Factors Are Associated With Delayed Wound Closure in Open Reduction and Internal Fixation of Adult Both-bone Forearm Fractures?

BACKGROUND: Delayed wound closure is often used after open reduction and internal fixation (ORIF) of both-bone forearm fractures to reduce the risk of skin necrosis and subsequent infection caused by excessive swelling. However, no studies we are aware of have evaluated factors associated with the use of delayed wound closure after ORIF. QUESTIONS/PURPOSES: (1) What proportion of patients undergo delayed wound closure after ORIF of adult both-bone forearm fractures? (2) What factors are associated with delayed wound closure? METHODS: The medical records of all patients who underwent ORIF wit

Multifocal humeral fractures.

INTRODUCTION: Multifocal humeral fractures are extremely rare. These may affect the neck and the shaft, the shaft alone, or the diaphysis and the distal humerus. There is no classification of these fractures in the literature. MATERIALS AND METHODS: From 2004 to 2010, 717 patients with humeral fracture were treated surgically at our department. Thirty-five patients presented with an associated fracture of the proximal and diaphyseal humerus: synthesis was performed with plate and screws in 34 patients, and the remaining patient had an open fracture that was treated with an external fixator.
