Effects of reduced dietary magnesium on platelet production and function in hamsters.

The increased vulnerability of animals fed a magnesium (Mg)-deficient (MD) diet to ischemia-induced myocardial necrosis has been attributed to changes in myocardial electrolyte metabolism. However, a variety of hematologic changes have also been reported in MD and some of these, such as an increase in platelet aggregability, may contribute to the increased myocardial vulnerability. In the present study, we quantified the effect of MD on platelet and megakaryocyte abundance as well as on platelet aggregability with and without an administered calcium channel blocker (nifedipine). Hamsters were

Hypomagnesemia and bronchial hyperreactivity. A case report.

In a 37-year-old woman, a heavy smoker and an alcoholic, bronchial hyperreactivity to histamine (PC20 FEV1 0.8 mg/ml) was related to hypomagnesemia (0.55 mmol/l). After acute magnesium repletion (24 h i.v. infusion MgSO4 6 g, serum magnesium 1.05 mmol/l), histamine PC20 FEV1 increased up to 9.8 mg/ml. The role of magnesium in modulating smooth muscle contractility is discussed and the importance of checking serum magnesium in patients with airway obstruction is suggested.

Skeletal myopathy and magnesium depletion: cellular mechanisms.

Muscle weakness and abnormal electrical activity of muscle cells have been described in magnesium-depleted patients and animals. The cellular mechanisms by which magnesium depletion causes the skeletal myopathy are not known. We review here the available clinical and experimental data on the effects of magnesium on: skeletal muscle cellular bioenergetics; excitation-contraction coupling, and biochemical and functional integrity of the cellular membrane. The data suggest that abnormalities in one or more of the above pathways may mediate the myopathy of magnesium depletion.

Acid-base and electrolyte abnormalities due to capreomycin.

We report a patient with tuberculosis treated with a five-drug regimen who experienced severe acid-base and electrolyte abnormalities including hypomagnesemia, hypokalemia, hypocalcemia, and a hypochloremic metabolic alkalosis. These disturbances are believed to be due to treatment with capreomycin, which produced renal magnesium wasting and possible tubular damage. Therefore, we recommend frequent determinations of serum electrolytes, magnesium, and calcium in patients treated with capreomycin.

Vitamin D deficiency in welfare institutions for the aged.

Serum concentrations of 25-OH-D in a group of 47 elderly people (70--94 years old) living in homes for the aged were lower than those in a matched control group living in their homes. No differences between the groups were noted in serum ionized calcium, alkaline phosphatase, inorganic phosphate, magnesium or parathyroid hormone. The low serum concentration of 25-OH-D may be due to less outdoor activities and/or a smaller dietary vitamin D intake in the institutionalized group. The importance of preserving an adequate vitamin D status in geriatric patients is emphasized.

Bone injuries due to tetany or convulsions during hemodialysis.

Two patients with long standing uremia sustained bone injuries due to tetany in one, and convulsions secondary to hemodialysis-induced alkalosis and hypomagnesemia in the other. In the former, there were multiple fractures involving the scapula, clavicles and femoral necks. In the latter, there was a slipped capital femoral epiphysis. Caution should be exercised against tetany or convulsive episodes which may cause bone injuries during hemodiaylsis in severely osteoporotic patients. In patients with progressive renal osteodystrophy, prevention of severe osteopenia by timely kidney transplant o

Effects of 1,25-dihydroxycholecalciferol on intestinal calcium transport in cortisone-treated rats.

The administration of glucocorticoids may decrease intestinal calcium absorption in vivo and the active transport of calcium in rat duodenum in vitro. It has been suggested that this apparent "anti-vitamin D-like" effect of steroid hormones may be related to alterations in vitamin D metabolism. In order to test this hypothesis, vitamin D-deficient control and cortisone-treated rats were given an intraperitoneal injection of 5.5 IU of 1,25-dihydroxycholecalciferol (1,25-DHCC), the probable end-organ active vitamin D metabolite in the intestine, and 16 h later studies of duodenal calcium transpo
