Impact of mastoidectomy on simple tympanic membrane perforation repair.

OBJECTIVES/HYPOTHESIS: Mastoidectomy has long been identified as an effective method of treatment for chronic ear infection. The effect of mastoidectomy on patients without evidence of active infectious disease remains highly debated and unproven. The objective in the study was to examine the impact of mastoidectomy on the repair of uncomplicated tympanic membrane perforations. STUDY DESIGN: Retrospective study of patients at tertiary referral center. METHODS: Four hundred eighty-four patients who underwent surgical repair of simple tympanic membrane perforations were identified and reviewed…

Incidence of dehiscence of the facial nerve at surgery for middle ear cholesteatoma.

OBJECTIVE: To identify the incidence of dehiscence of the facial nerve in patients undergoing surgery for cholesteatoma and to describe its relevance with iatrogenic facial nerve injury. STUDY DESIGN AND SETTING: This was a retrospective study including 117 tympanoplasties with or without mastoidectomy performed in 49 men and 66 women. RESULTS: The presences of FND after exenteration of disease are 33.3% of total surgical procedures, 33% of the initial procedures, and 37.5% of the revision procedures. The locations of facial nerve dehiscence were 87.2% in only the tympanic segment, 7.7% in o…

Vertigo and hearing disturbance as the first sign of a glioblastoma (World Health Organization grade IV).

OBJECTIVE: To describe vertigo and hearing disturbance as a first sign of glioblastoma. STUDY DESIGN: Case report. SETTING: Ear, Nose, and … lieved and the patient received radiation therapy. Tumor progression indicated a recraniotomy and a mastoidectomy. The tumor was only partially resected, and required chemotherapy. It subsequently developed otoliquorrhea and required a remastoidectomy. Histopathology of a pathological fracture of the X thoracic vertebra revealed a metastasis of the known …

Reconstruction of the vein of Labbé by using a short saphenous vein bypass graft. Technical note.

Protection of the vein of Labbé is a significant concern during surgery that involves retraction of the temporal lobe. A cranial base surgical approach, especially one via the presigmoid-petrosal route, carries considerable risk to this venous complex. A case is presented in which a large dominant vein of Labbé was injured during resection of a petroclival meningioma. This vein drained all the sylvian venous circulation as well as the lateral temporal surface; no connection to another venous system was noted. The vein was successfully reconstructed using a short saphenous vein bypass graft. Si

Iatrogenic facial nerve injury during otologic surgery.

Perhaps the most devastating complication in otologic surgery is that of inadvertent injury to the facial nerve. A review of 22 patients who had sustained an iatrogenic facial nerve injury was conducted. Although the most common procedure being performed during the injury was mastoidectomy (55%), a surprising number of patients had injury during tympanoplasty (14%) or during removal of exostoses (14%). The most common area of injury to the facial nerve in this series was in the tympanic segment. In 79% of the patients, the facial nerve injury was not detected at the time of surgery. All patien…

Electrical vestibular stimulation and space motion sickness.

Electrical vestibular stimulation (EVS) in dynamic balance condition was studied in order to search for a new provocative test of space motion sickness (SMS). SMS is usually attributed to a sensory conflict caused by exposure to microgravity. Vestibular information is conflicting but also unusual and insignificant. EVS is in accordance with this feature because it is not the adequate stimulus of the vestibular receptors. EVS was achieved by means of binaural electrical stimulation. Effects of EVS were potentiated by compelling the subject to maintain dynamic balance on a seesaw. The quantifica

[Otologic manifestations presenting and predominating in necrotizing angiitis].

The authors report on 5 observations of necrotising angiitis with a specifically otological onset, namely 4 Wegener's syndromes and one periarteritis nodosa. In all 5 cases, the initial symptomatology consisted of otological signs in a feverish context, producing pictures of serous or sero-purulent otitis. Its resistance to usual forms of therapy led in 3 cases to the installation of transtympanic aerators, and in 2 cases to the performance of an antro-attico-mastoidectomy. One of the observations led to the discovery in the middle ear of a histological aspect specific to the disease, from whi…

Clinical Characteristics, Concomitant Malformations and Hearing Status in Congenital Microtia - A Retrospective Analysis of 635 Cases.

PURPOSE: This study aims to investigate the clinical characteristics of congenital microtia, providing valuable insights for its diagnosis and treatment. METHODS: A retrospective analysis was conducted on the medical records of 635 patients with congenital microtia admitted to our hospital between July 2017 and July 2021. Patients were categorized based on the presence of concomitant malformations, including craniofacial, cutaneous, and limb skeletal deformities. Mastoid CT scans were used to assess the prevalence of inner ear malformations and middle ear inflammation. Additionally, pure-tone…

Glomus tumors, … e and neck showed an avidly enhancing 3.7 cm x 1.8 cm x 1.2 cm mass within the right middle ear and mastoid cavity, extending into the external auditory canal and through the eustachian tube into the nasopharynx. Comparison of Post-auricular Soft Tissue and Post-auricular Soft Tissue With Autologous Bone Pate in Obliteration of the Canal Wall Down Mastoidectomy Cavity.

Background The surgical management of chronic otitis media (COM) with squamous disease is canal wall down mastoidectomy (CWDM). Canal wall down procedures require the obliteration of the newly formed cavity to mitigate complications. Soft tissue flaps, including Rambo flap, Hong Kong flap, Palva flap, and inferior-based fascio-periosteal flap, as well as autologous bone pâté, have been the most successful and commonly used materials for obliteration over the past two decades. Although each flap has its advantages, the Palva flap is considered superior, primarily because of its ease of use and …
