Before selected hopeless teeth were extracted, the authors orthodontically erupted these teeth to augment the three-dimensional topography of future implant recipient sites. This controlled "biodynamic orthodontic modeling" manipulates teeth, with as little as one fourth to one third of their apical attachment intact, by constructively increasing their local osseous dimensions in the vertical plane. Such vertical remodeling, especially of the strategic labial plate and crest, allows a more ideal placement of the implant (within 1 to 3 mm apically, depending on implant type) in relation to the

Case history is presented of an indigenous Zambian girl. Initial surgical repositioning is shown and at a later stage when the tooth had erupted into the nasolabial vestibule and an orthodontic correction was performed. Surgical trauma as well as orthodontic forces may have precipitated root resorption; however, the aim of saving a tooth has been achieved.

The purpose of this study was to investigate the eruption speed and rate of angulation change of a cyst-associated mandibular second premolar after marsupialization of a dentigerous cyst in preadolescents. Premolars on the cyst and noncyst sides and size of the cyst were retrospectively examined with a total of 42 pairs of normalized panoramic radiographs in 20 patients who underwent neither extraction nor orthodontic traction of the cyst-associated mandibular second premolar after marsupialization. Sixteen premolars at the intraosseous eruption stage in a total of 9 preadolescents without cys

Impaction or retention of first and second permanent molars is an uncommon condition with diverse therapeutic approaches. To ascertain the success rate of different treatment possibilities, a retrospective study was made of 25 patients with a total of 43 permanent molars with eruption disturbances. In most cases, the nonerupted teeth were mandibular second molars (65%), followed by maxillary second molars (21%). Their position, degree of impaction (inclusion), clinical features, repercussion on the neighboring teeth, type of treatment, and outcome were evaluated. Infraocclusion was often assoc

Close root proximity between the maxillary central incisors presents a problem in the surgical management of a maxillary palatal expansion. During the surgical fracture of this interdental area, the possibility exists for a separation to occur between the root surface and the bone. If this does occur, it is paramount that the gingival attachment remain intact. Asymmetric separation places more stress on the mesial gingival attachment because of the anatomy of the gingival fiber apparatus. Gingival detachment results in epithelial downgrowth in an apical direction, which in turn prevents bone a

Panoramic and periapical radiographs of 278 patients with cleft lip, cleft palate, or both (158 males and 120 females), age 5 to 18 years, were examined to determine the frequency of missing second premolars and the possible association between the cleft side and the side from which the premolar was absent. The prevalence (18%) of missing premolars found in this study is significantly higher than is found in the general population. A considerably higher incidence of missing second premolars was found in the maxilla compared with the mandible both for unilateral and bilateral missing teeth. The

Impacted Incisor With Dilaceration refers to a dental deformity characterized by an angulation between crown and root causing noneruption of the incisor. Surgical extraction used to be the first choice in treating the severely dilacerated incisor. In this article, a horizontally impacted and dilacerated maxillary central incisor was diagnosed radiographically. By combining two stages of the crown exposure surgery with light force orthodontic traction, the impacted dilacerated incisor was successfully moved into proper position. However, long-term monitoring of the stability and periodontal hea

A case report is presented on a female adult with a Class III mandibular protrusion. Resolution of her dentoskeletal problem required a combination of comprehensive multibonded orthodontic mechanotherapy and orthognathic surgery. The patient preferred a multilingual bracket appliance because of esthetics. Both surgical and orthodontic treatment were quite successful in the correction of the facial profile and occlusion. Both psychological and esthetic satisfaction were achieved.

This is a case report of a 23-year-old black woman with a skeletal Class II Division 1 malocclusion and an anterior open bite. The maxillary central incisor root was severely dilacerated, a maxillary lateral incisor and canine were transposed, a maxillary canine was impacted near the orbital rim, and a mandibular second premolar was congenitally missing. Selective extractions and attention to detail provided outstanding occlusal function and improved facial esthetics.

The purpose of the present investigation was to test the hypothesis that permanent disk displacement in the temporomandibular joint (TMJ) is one causal factor in the development of mandibular midline asymmetry. Unilateral, nonreducing disk displacement was therefore created in growing rabbits. The right TMJ in seven experimental animals was surgically opened, and the disk was anteriorly displaced with the posterior disk attachment kept intact above the condyle. The right TMJ in seven sham animals was surgically opened and closed without any disk manipulation. Seven additional rabbits served as

Treatment of children with long face and with Class III malocclusions is difficult because most of the available appliances treat one of the two problems at the cost of aggravating the other one. In this article, we present the steps of design and fabrication of a modified protraction headgear that can be useful for such a patient. A patient treated with this approach is presented.

Abnormal cervical muscle function can cause abnormal head posture, adversely affecting the development and morphology of the cervical spine and maxillofacial skeleton, which in turn leads to facial asymmetry and occlusal abnormality. There can be morphologic abnormalities of the mandibular fossa, condyle, ramus, and disk accompanying the imbalance of the cervical and masticatory muscles activities. Two normally growing Japanese female patients with Class II Division 1 malocclusion presented with TMJ symptoms and poor head posture as a result of abnormal sternocleidomastoid and trapezius cervic

External apical root resorption is a multifactorial problem encountered in all disciplines of dentistry, but it is most commonly seen in cases treated orthodontically. Specific tooth movements that are most likely to exacerbate external apical root resorption are poorly understood. Purpose of the present investigation was twofold: (1) to quantify apical and incisal movements of the maxillary central incisor in the sagittal and vertical planes from cephalograms and (2) to use stepwise multivariate linear regression analyses to see which tooth movements and skeletodental relationships are most p

At the University of the Pacific, mixed dentition treatment is an integral part in the graduate program in orthodontics. One patient had a severe skeletal ANB discrepancy of 7 degrees, lower incisors crowding, and 12 mm of overjet. He was considered an excellent candidate for Phase I treatment to reduce the severity of the problem. It is our philosophy to take advantage of the growth potential in a patient by initiating Phase I treatment between the ages of 7 and 9 years, depending on the eruption of the permanent first molars and incisors and the severity of the malocclusion. We use leeway sp

This prospective controlled study investigated the net effects of the Twin Block functional appliance taking into account the effects of normal growth in an untreated control group. The treatment group consisted of 36 subjects, mean age of 12.4 years, consecutively treated with Twin Block appliances for an average period of 0.9 years. Each subject had immediate pre- and posttreatment lateral cephalograms. The control group consisted of 27 subjects with a mean age of 12.1 years. These patients were observed for a mean time of 1.2 years and had radiographic investigation at the initial consultat

One of the important aspects of diagnosis in the mixed dentition is the determination of the tooth size-arch length relationship. Such a determination is often made before eruption of the permanent canines and first and second premolars. The most commonly used prediction method of Tanaka and Johnston is based on data from a sample of children of Northern European descent. The accuracy of this method when applied to a different ethnic population is questionable. In this study, 201 dental plaster casts of Asia-Pacific-American subjects, all of whom were under the age of 21 years, were used. The

Heredity is one of the possible factors associated with congenitally missing mandibular incisors. We report four cases--a mother and three daughters--in an attempt to examine this hypothesis. Different treatment modalities have been used to treat malocclusions stemming from this problem. The treatment philosophy of consolidating mandibular spaces and obtaining a stable occlusion, thereby preventing the removal of maxillary teeth or the use of a mandibular prosthesis, is presented.

A prospective randomized clinical trial was conducted to evaluate the early treatment of Class II, Division 1 malocclusion in prepubertal children. Facial and occlusal changes after treatment with either a headgear or a Fränkel function regulator are reported. Molar and canine relationships, overjet, intermolar and intercanine distances were measured from casts taken every 2 months, and mounted on a SAM II articulator. Cephalometric radiographs were taken annually. The results indicate that both the headgear and function regulator were effective in correcting the malocclusion. A common mode of

Nickel is a strong biological sensitizer and consequently may induce a delayed hypersensitivity reaction (type IV immune response). Because nickel is a component of the majority of the orthodontic alloys, the objectives of this cross-sectional study were to determine the prevalence of nickel hypersensitivity reaction before, during, and after orthodontic therapy with conventional stainless steel brackets and wires; to evidence the induction of this reaction by the orthodontic appliances; and to characterize the nickel hypersensitive persons. Nickel patch tests and a questionnaire were used to
