rTMS for rapid relief of sleep disorders induced by influenza virus infection: a clinical retrospective study.

To explore the effect of repetitive transcranial magnetic stimulation (rTMS) on influenza-induced sleep disorders, verify its adjuvant value, and reveal the "influenza-immune inflammation-abnormal brain function-sleep disorder" pathway, a retrospective cohort study enrolled 55 adults (18-40 years, no comorbidities) with acute influenza (2023-2025, First Affiliated Hospital of Air Force Medical University). They were divided into positive control group (PC, n = 20, conventional treatment) and rTMS group (n = 16, conventional + rTMS: 10 Hz, 1x/day, 20-30 mins, 3000 pulses, 100% motor threshold,

Multimodality Intravascular Imaging Guidance (NIRS-IVUS-OCT) Compared to Angiography-Guided Percutaneous Coronary Intervention in Patients With Acute Myocardial Infarction: The NIRVUS Trial.

BACKGROUND: Imaging-guided percutaneous coronary intervention (PCI) improves outcomes, but the clinical utility of near-infrared spectroscopy (NIRS) remains unclear. AIMS: The aim of the study was to evaluate whether multimodality imaging with NIRS, intravascular ultrasound (IVUS) and optical coherence tomography (OCT) would improve stent strut coverage compared with angiography-guided PCI in patients with acute myocardial infarction (MI) and further to assess the impact of lipid-rich plaque (LRP) on vascular healing. METHODS: A total of 104 patients with MI were randomized to imaging- or an

The Occlusal Contact Area Is Associated with the Magnitude but Not Peak Timing of Changes in Chewing-Induced Brain Blood Flow in Mandibular Prognathism.

Background/Objectives: Jaw deformities affect not only facial aesthetics but also various oral functions. While previous studies have demonstrated that mandibular prognathism (MP) alters masticatory-induced brain blood flow (BBF), the temporal characteristics of these hemodynamic changes have remained unclear. In this cross-sectional observational study, we investigated the following two specific objectives: (1) whether food hardness affects not only the magnitude but also the temporal patterns of BBF changes during mastication and (2) how malocclusion is associated with these temporal hemodyn

Prolonged cerebral oxygenation surveillance with algorithm-based management: a neurocritical care bundle for extremely preterm infants.

BACKGROUND: Cerebral hypoxia-ischemia impairs brain development in extremely preterm infants and is associated with poor neurological outcomes. Near-infrared spectroscopy (NIRS) is a noninvasive continuous monitoring method for regional cerebral oxygen saturation (rcSO2). PURPOSE: This study evaluated the clinical feasibility and neurological impact of a neurocritical care bundle that incorporates prolonged multidisciplinary hemodynamic monitoring and a stepwise management algorithm. METHODS: Preterm infants with a gestational age (GA) ≤28 weeks or birth weight (BW) ≤1,000 g were prospective

Muscle oxidative capacity but not specific force is greater in aerobic versus resistance trained young adults.

PURPOSE: Near-infrared spectroscopy (NIRS) can be applied to assess skeletal muscle oxidative capacity ( m V ˙ O 2 ). Specific force (SF) and echo intensity (EI) represent muscle quality. However, it is unknown how exercise participation and biological sex impact m V ˙ O 2 , and if measures of muscle quality are related to m V ˙ O 2 . The aim was to assess training history- and biological sex-related differences in m V ˙ O 2 , SF, and EI in males and females. METHODS: To determine training history, 50 adults (23 females, 66% resistance trained, age:22 ± 3 years) completed strength

Lipoprotein(a), Cholesterol, Triglyceride Levels, and Vulnerable Coronary Plaques: A PROSPECT II Substudy.

BACKGROUND: Although lipoprotein(a) (Lp[a]) has been associated with acute myocardial infarction (MI), the relationship between Lp(a) and the presence of high-risk "vulnerable" coronary plaques has not been studied. OBJECTIVES: The aim of this study was to investigate whether specific lipoproteins are associated with pancoronary plaque volume and lipid deposition vs the development of non-flow-limiting high-risk vulnerable plaques. METHODS: In PROSPECT (Providing Regional Observations to Study Predictors of Events in the Coronary Tree) II, 3-vessel coronary artery imaging was performed with

Cerebral oxygen saturation in relation to end-tidal CO2 in cardiopulmonary resuscitation - Separate views of brain and body?

BACKGROUND: An increase in both regional cerebral oxygen saturation (rSO2) measured by near-infrared spectroscopy (NIRS) and end-tidal carbon dioxide (etCO2) during advanced life support for out-of-hospital cardiac arrest (OHCA) is associated with a higher likelihood of return of spontaneous circulation (ROSC) and may predict neurological outcome. However, it remains unclear which marker is more predictive for which outcome parameter. METHODS: In this prospective observational study, we assessed rSO2 and etCO2 in patients treated for OHCA in the metropolitan area of Vienna between 05/2017 and

Persistence of fatigue in the absence of pathophysiological mechanisms in some patients more than 2 years after the original SARS-CoV-2 infection.

Following an acute infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), a substantial percentage of patients report the persistence of debilitating symptoms, often grouped in a syndrome termed 'long COVID'. We sought to identify potential pathophysiological mechanisms responsible for the persistence, in some long COVID patients, of symptoms related to fatigue/exercise intolerance (excessive or early fatigue, excessive or early dyspnoea, muscle weakness, and myalgias) more than 2 years after the original infection. Twelve patients who reported persistent symptoms (Long C

Twelve-week nitrate-rich beetroot extract supplementation improves lower limb vascular function and serum angiogenic potential in postmenopausal women.

Postmenopausal women have reduced nitric oxide (NO) bioavailability and impairments within the microcirculation and lower limb conduit arteries, key factors contributing to cardiovascular disease (CVD) development and mortality. Dietary nitrate supplementation has shown promise in improving vascular function, yet most studies are limited to short-term interventions and upper-limb conduit artery function assessment. We investigated whether a 12-wk nitrate-rich beetroot extract supplementation improves lower-limb conduit artery and skeletal muscle microvascular function, alongside serum angiogen

Sex differences in the prefrontal cortex and muscle oxygenation during exercise until exhaustion in endurance-trained individuals.

Biological sex influences exercise performance, largely owing to anatomical and physiological differences in brain areas involved in cognitive motor control and in respiratory and locomotor muscles related to workload. We used near-infrared spectroscopy data to examine sex differences in haemodynamic responses and oxygenation patterns in the prefrontal cortex (PFC), m. intercostales and m. vastus lateralis during incremental exercise in 74 endurance-trained individuals. Changes (Δ) in oxyhaemoglobin (O2-Hb), deoxyhaemoglobin (H-Hb) and tissue saturation index (TSI) were analysed using a two-wa

Advances and Challenges in Artificial Womb for Respiratory Support in Extremely Low Gestational Age Neonates: A Narrative Review.

BACKGROUND: Respiratory management of extremely low gestational age neonates (ELGANs) is a major challenge in neonatal intensive care, as they face high mortality and morbidity due to complications such as respiratory distress syndrome and bronchopulmonary dysplasia. Traditional respiratory support methods (e.g., mechanical ventilation) have limitations, potentially leading to adverse outcomes like chronic lung injury and intracranial hemorrhage. Artificial womb technology (AWT), based on extracorporeal membrane oxygenation (ECMO) principles, has emerged as a potential alternative by simulatin

Autofluorescence Imaging of Parathyroid and Thyroid Under Visible and Near-IR Light Excitation.

Identifying parathyroid glands during surgery is challenging and time-consuming due to their small size (3-5 mm) and camouflaged appearance in the background of the thyroid, lymph nodes, fat, and other neck structures. For the gland itself, it is also important to differentiate abnormal ones from normal ones. Accidental damage or removal of the normal glands can result in complications like hypocalcemia, which may necessitate lifelong medication dependence, and, in extreme cases, lead to death. The study of autofluorescence optical properties of normal and abnormal parathyroid glands and the s

Increased Blood Pressure Variability is Associated with More Time Spent Outside Personalized Limits of Cerebral Autoregulation in Postpartum Preeclampsia.

BACKGROUND: Preeclampsia is associated with altered cerebral autoregulation (CA) and cerebrovascular injury, including intracranial hemorrhage and cerebral oedema, which are major causes of postpartum maternal mortality. Impaired autoregulation increases susceptibility to cerebral hypoperfusion or hyperperfusion. Preeclampsia is also associated with sympathetic hyperreactivity resulting in increased blood pressure variability (BPV). We investigated whether higher BPV in postpartum patients with preeclampsia correlated with more time spent outside the personalized limits of CA. METHODS: This i

Evaluating the Neural Underpinnings of Motivation for Walking Exercise.

OBJECTIVE: Motivation is critically important for rehabilitation, exercise, and motor performance, but its neural basis is poorly understood. Recent correlational research suggests that the dorsomedial prefrontal cortex (dmPFC) may be involved in motivation for walking activity and/or descending motor output. This study experimentally evaluated brain activity changes in periods of additional motivation during walking exercise and tested how these brain activity changes relate to self-reported exercise motivation and walking speed. METHODS: Adults without disability (N = 26; 65% women; 25 [sta

Study of Physiological Adaptations in Vertical Kilometer Runners: Focus on Cardiorespiratory and Local Muscle Demands.

Background: Research into key performance factors in trail running, particularly in vertical kilometer (VK) races, is crucial for effective training and periodization. However, recent studies on metabolic and cardiorespiratory responses during VK races, especially using field tests, are limited. Objectives: Therefore, the aim of this study is to evaluate the metabolic and cardiorespiratory responses during a VK field test, identifying differences based on sex and performance level, as well as key performance factors and their deterioration due to fatigue. Fifteen trained trail runners (ten mal

Effect of paediatric caudal injection volume on optic nerve sheath diameter and regional cerebral oximetry: A randomised trial.

BACKGROUND: Caudal injections commonly used for neuraxial anaesthesia in children can displace cerebrospinal fluid cranially causing safety concerns in terms of raised intracranial pressure. Optic nerve sheath diameter (ONSD) is a noninvasive surrogate for the measurement of intracranial pressure. Regional cerebral oximetry (CrSO 2 ) can monitor brain oxygenation, which may decrease by a reduction in cerebral flow due to increased intracranial pressure. OBJECTIVES: Comparing how caudal injection volumes of 0.8 and 1.25 ml kg -1 influence ONSD and CrSO 2 within the first 30 min after injection
