We present a patient with a closed displaced distal tibia fracture with entrapment of the anterior tibial vessels in the fracture after tibial nailing. This complication was initially not recognised. After several debridements of the forefoot on the same side due to open metatarsal fractures and severe soft tissue injury, a free latissimus dorsi flap was used for covering the dorsum pedis. Preoperative angiography showed occlusion of the anterior tibial artery at the fracture line which was interpreted as a secondary occlusion due to an intima lesion of the vessel after injury. The entrapment

The results of 100 'Long Gamma Nails' (LGN) inserted in 97 patients, for the treatment of complex proximal femoral fractures and pathological lesions, are reported. The mean patient age was 74 (16-98). Seven significant complications occurred; four lag screws cut out, one broken nail, one infected non-union and one peri-prosthetic fracture. Five patients underwent revision either to total hip arthroplasty or exchange nailing. Seventy patients had follow-up of greater than 2 months (mean 10.8 months). Success was defined as stability of the fracture until union or death; this was achieved in 85

Introduction Operating theatres are resource-intensive environments and contribute disproportionately to healthcare-related carbon emissions. While the environmental impact of elective orthopaedic surgery has been described, there are limited procedure-level data for orthopaedic trauma surgery. Methods A single-centre, retrospective, theatre-based carbon footprint analysis was undertaken for five common orthopaedic trauma procedures performed in 2023: hip hemiarthroplasty, sliding hip screw fixation, femoral intramedullary nailing, ankle fracture fixation, and minor trauma procedures. Consum

INTRODUCTION AND IMPORTANCE: Langerhans cell histiocytosis (LCH) is a rare clonal proliferative disorder of dendritic cells, predominantly affecting children. Adult-onset LCH is uncommon and often misdiagnosed due to its atypical presentation. Pathological fractures as the initial manifestation are particularly rare in older adults.
CASE PRESENTATION: We report a 63-year-old female who sustained a pathological femoral fracture following minor trauma, with radiographs revealing cortical thinning and lytic changes. Subsequent imaging demonstrated multifocal osseous lesions involving the pelvis,

INTRODUCTION: No objective criteria exist to help surgeons choose between IM nailing and plate fixation for 4-part fractures of the proximal humerus. The goal of this study was to identify radiological criteria that would make one technique a better choice than the other.
MATERIAL AND METHODS: This was a comparative, multicentre, retrospective study of 54 cases of antegrade nailing and 53 cases of plating performed between 1st January 2009 and 31 December 2011 for 4-part fractures of the proximal humerus. All patients had a minimum radiological and clinical follow-up of 18 months. The functio

INTRODUCTION: Stable reconstruction of proximal femoral (PF) fractures is especially challenging due to the peculiarity of the injury patterns and the high load-bearing requirement. Since its introduction in 2007, the PF-locking compression plate (LCP) 4.5/5.0 has improved osteosynthesis for intertrochanteric and subtrochanteric fractures of the femur. This study reports our early results with this implant.
METHODS: Between January 2008 and June 2010, 19 of 52 patients (12 males, 7 females; mean age 59 years, range 19-96 years) presenting with fractures of the trochanteric region were treated

The treatment of a simple (AO/OTA classification 31A3.1) reverse oblique intertrochanteric hip fracture is a challenge for the orthopaedic surgeon. The surgical options include the use of side plates with various angled leg screws or intramedullary devices. The purpose of this study was to retrospectively assess our results of treating reverse oblique fracture with an expendable proximal femoral nail (EPFN) or with a dynamic condylar screw-plate (DCS: 95°) between January 2006 and July 2009. Thirty-three patients (6 males and 27 females, mean age 78 years) met the study inclusion criteria and

The results of early stable fixation of 64 metacarpal fractures due to low-velocity gunshot wounds in 49 patients were reviewed retrospectively. These reconstructive procedures were performed between 1 and 7 days after injury and involved stable internal fixation supplemented with primary iliac crest bone graft in 40 fractures with bone loss or comminution. Postoperatively, the hands were splinted with 90 degrees of metacorpophalangeal flexion while early interphalangeal motion was emphasized. Follow-up averaged 12 months, with a range of 4 months to 7 years. Primary bone union was achieved in

BACKGROUND: The aims of this study are to (1) determine whether fixation of metastatic long bone fractures with an intramedullary nail (IMN) influences the incidence of lung metastasis in comparison to arthroplasty or ORIF (Arthro/ORIF); and (2) assess this relationship in primary tumor types; and (3) to assess survival implications of lung metastasis after surgery.
METHODS: Retrospective cohort study investigating 184 patients (107 IMN, and 77 Arthro/ORIF) surgically treated for metastatic long bone fractures. Patients were required to have a single surgically treated impending or establishe

Elastic intramedullary nailing (ESIN) has been proposed as an alternative minimal-invasive method for the operative management of mid-shaft fractures of the clavicle. However, a relevant complication rate has been reported in previous cohorts. The present retrospective single-centre study aimed to analyse the complications following ESIN in adult patients with clavicular mid-shaft fractures (Allman type I) and their impact on functional and patient-perceived outcome measures. Results were compared to a control group receiving locking plate osteosynthesis. The clinical course and outcome of ope

INTRODUCTION: The effects of reaming for preparation of intramedullary fixation in long bone fractures have been widely studied. We compared pulmonary and systemic effects between conventional reaming with reamer irrigator aspirator and unreamed nailing in an acute porcine trauma model with a standardized femur fracture.
MATERIALS AND METHODS: In a standardized porcine model, (moderate blunt chest trauma, abdominal injury and femoral shaft fracture), the femur was submitted to intramedullary nailing after resuscitation and normalization of pulmonary function. The treatment groups included 3 r

Large osseous defects of the hindfoot and ankle pose a surgical challenge. Tibiotalocalcaneal (TTC) arthrodesis utilizing a structural allograft may be required to fill the osseous void, preserve limb length and achieve fusion. Several authors have reported small case series on this topic, however outcomes have varied and no systematic review of this data has been published to date. The primary aim of this study is to report rates of osseous union, limb salvage and complications in patients undergoing TTC arthrodesis with a structural allograft. A total of 11 publications were identified that

UNLABELLED: Fifteen patients with 26 nonunions of diaphyseal forearm fractures were treated with interlocking intramedullary nails and iliac bone grafts. The purpose of our study was to evaluate the rate of successful healing and complications that developed after using this technique. The mean followup was 31 months (range, 25-38 months). The average length of bone loss after freshening the bone ends was 20 mm (range, 10-30 mm). Radiographic union was achieved in 14 patients (96%). Compared with the uninjured contralateral arm, the mean loss of flexion and extension motion at the wrist was 27

INTRODUCTION: Proximal humerus fractures (PHFs) are among the most common upper limb fractures, particularly in elderly patients after low-energy falls. The pucker sign is a rare but clinically important finding caused by penetration of a fracture fragment through the deltoid muscle, leading to entrapment of subcutaneous tissue and visible skin dimpling. This sign indicates soft tissue interposition that may prevent closed reduction and threaten skin vascularity if unrecognized.
CASE REPORT: A 31-year-old male with a history of multiple sclerosis presented with a proximal humerus fracture (AO

AIMS: The use of circular frames in the management of tibia fractures and deformity correction is well established in the literature. The Taylor Spatial Frame (TSF) is the most widely used hexapod device globally. However, the majority of published papers are small in patient numbers, contain primary and revision cases, and outcomes are therefore unclear. In this study we evaluate the clinical and radiological outcomes of patients with tibial fractures treated primarily with a TSF.
METHODS: Patients were identified from a prospective trauma database at a UK major trauma centre. An analysis of

INTRODUCTION: Surgical treatment should be considered for metacarpal fractures that have severe angulation or shortening. Percutaneous pinning is a useful technique in that it is minimally invasive. Insertion of Kirschner wires using retrograde pinning is easier than that using antegrade or transverse pinning. On the other hand, anterograde pinning is also used for metacarpal shaft and neck fractures, but some complications such as perforation of the metacarpal head or laceration of the extensor tendons can occur. The purpose of this study is to evaluate the functional outcomes of simple retro

Although osteosynthesis using K-wires is a commonly performed procedure, the outcome is not always preferable. We aimed to identify the prognostic factors for postoperative complications in paediatric patients with both-bone forearm fractures. We reviewed 160 paediatric patients who underwent K-wire osteosynthesis for either distal or diaphyseal forearm fractures. We defined the occurrence rate of postoperative complications as the primary outcome and performed a multivariate logistic regression analysis. Variables such as age, sex, mechanism of injury, presence of an open fracture, ulnar or r

INTRODUCTION: The use of an intramedullary fibular allograft together with a locking plate fixation has been recently described in biomechanical studies to provide additional medial support and prevent varus malalignment for displaced proximal humeral fractures with promising results. The aim of this study was to evaluate the clinical and radiographic outcome of a locking plate with fibular allograft augmentation in unstable humeral fractures.
METHODS: We prospectively assessed the functional outcome and complications in 17 patients with proximal humeral fractures with disrupted medial column

BACKGROUND: Bisphosphonates (BPs) are the common drugs used for the treatment of postmenopausal osteoporosis. Short term benefits of the BPs are well known. However, there are concerns regarding their long term use. The aim of the study was to analyze the association between atypical femoral fractures and BP misuse/abuse as well as study the outcome of management of these fractures.
MATERIALS AND METHODS: A retrospective study of a prospectively studied patients who presented with atypical femoral fractures between January 2010 and August 2012 and were followed up upto June 2014. The cohort c
