Computational modeling and analysis of thoracolumbar spine fractures in frontal crash reconstruction.

OBJECTIVE: This study aimed to reconstruct 11 motor vehicle crashes (6 with thoracolumbar fractures and 5 without thoracolumbar fractures) and analyze the fracture mechanism, fracture predictors, and associated parameters affecting thoracolumbar spine response. METHODS: Eleven frontal crashes were reconstructed with a finite element simplified vehicle model (SVM). The SVM was tuned to each case vehicle and the Total HUman Model for Safety (THUMS) Ver. 4.01 was scaled and positioned in a baseline configuration to mimic the documented precrash driver posture. The event data recorder crash pulse

Comparison of soft-tissue profiles in Le Fort I osteotomy patients with Dolphin and Maxilim softwares.

INTRODUCTION: To correct dentofacial deformities, a combination of orthodontic treatment and orthognathic surgery is needed. Prediction software packages are beneficial in treatment planning and achieving improved outcomes, but before using any software, its reliability and reproducibility must be assessed. The aim of this study was to evaluate the accuracy of 2-dimensional Dolphin (version 10; Dolphin Imaging & Management Solutions, Chatsworth, Calif) and 3-dimensional Maxilim (Medicim, Sint-Niklaas, Belgium) softwares in predicting the soft-tissue profiles of patients who had Le Fort I osteo

Critical computed tomographic diagnostic criteria for frontal sinus fractures.

PURPOSE: Diagnosis and treatment of frontal sinus fractures (FSFs) have progressed over the previous 30 years. Despite advances in computed tomography, there is no current diagnostic uniformity with regard to classification and treatment. We developed a statistically valid treatment protocol for FSFs based on injury pattern, nasofrontal outflow tract (NFOT) injury, and complication(s). These data outlined predictable injury patterns based on specific computed tomographic findings critical to the diagnosis and ultimate treatment of this potentially fatal injury. MATERIALS AND METHODS: A retros

Regional differentiation of cranial suture-associated dura mater in vivo and in vitro: implications for suture fusion and patency.

Despite its prevalence, the etiopathogenesis of craniosynostosis is poorly understood. To better understand the biomolecular events that occur when normal craniofacial growth development goes awry, we must first investigate the mechanisms of normal suture fusion. Murine models in which the posterior frontal (PF) suture undergoes programmed sutural fusion shortly after birth provide an ideal model to study these mechanisms. In previous studies, our group and others have shown that sutural fate (i.e., fusion vs. patency) is regulated by the dura mater (DM) directly underlying a cranial suture. T

Distraction Osteogenesis Versus Conventional Fronto-Orbital Advancement for the Treatment of Unilateral Coronal Synostosis: A Comparison of Perioperative Morbidity and Short-Term Outcomes.

Fronto-orbital advancement and remodeling (FOAR) remains the most widely practiced treatment of unicoronal craniosynostosis (UCS) despite recent studies of ocular dysfunction and aesthetic shortcomings in the long-term. The aim of the study was to compare perioperative morbidity and short-term outcomes of a recently developed, nondevascularizing, distraction-based treatment of UCS with conventional FOAR. To do so, the authors compared the first 6 patients who were treated with a new osteotomy/distraction approach to the last 6 patients who underwent traditional FOAR for the treatment of UCS wi

An atypical case of head trauma with late onset of contrecoup epidural hematoma, cerebellar contusion, and cerebral infarction in the territory of the recurrent artery of Heubner.

We encountered a case of head trauma with very unusual delayed events. A 68-year-old woman was admitted to our hospital after receiving a direct impact to her occiput in a traffic accident. Head CT scans showed a thin acute epidural hematoma in the posterior fossa corresponding to a linear fracture, followed by late onset of contrecoup left frontal epidural hematoma and subsequent cerebellar contusion in the right cerebellar hemisphere. Fifteen days after the trauma, the patient developed mild motor weakness of right upper extremity. MRIs demonstrated an infarct in the territory of the left re

Novel model of frontal impact closed head injury in the rat.

Frontal impact, closed head trauma is a frequent cause of traumatic brain injury (TBI) in motor vehicle and sports accidents. Diffuse axonal injury (DAI) is common in humans and experimental animals, and results from shearing forces that develop within the anisotropic brain. Because the specific anisotropic properties of the brain are axis-dependent, the anatomical site where force is applied as well as the resultant acceleration, be it linear, rotational, or some combination, are important determinants of the resulting pattern of brain injury. Available rodent models of closed head injury do

[Rhinosurgical concept in management of fronto-basal defects with cerebrospinal rhinorrhea].

BACKGROUND: Cerebrospinal fluid (CSF) leak, with its potential infectious complications remains today a challenge in diagnosis and surgical treatment. With the introduction of endonasal microsurgery, new minimally invasive techniques for the repair of these defects have been pioneered. Nasal approaches to the anterior skull base include external-transfacial as well as endonasal approaches for reconstruction. It is important to have multiple possible approaches for the correction so that the optimal method for the individual pathology may be selected. The purpose of this paper is to clarify the

Saethre-Chotzen syndrome: long-term outcome of a syndrome-specific management protocol.

AIM: To assess the long-term outcomes of our management protocol for Saethre-Chotzen syndrome, which includes one-stage fronto-orbital advancement. METHOD: All patients born with Saethre-Chotzen syndrome between January 1992 and March 2017 were included. Evaluated parameters included occipital frontal head circumference (OFC), fundoscopy, neuroimaging (ventricular size, tonsillar position, and the presence of collaterals/an abnormal transverse sinus), polysomnography, and ophthalmological outcomes. The relationship between papilledema and its associated risk factors was evaluated with Fisher'

Arachnoid cysts and pneumosinus dilatans.

The association of intracranial arachnoid cysts and pneumosinus dilatans is considered to be relatively rare, and has previously been described in only a limited number of cases. In this study the CT and/or MRI examinations of a total of 30 cases with middle cranial fossa arachnoid cysts were retrospectively evaluated in an attempt to note any association with pneumosinus dilatans of the paranasal sinuses. In four patients with surgically treated, large, middle cranial fossa arachnoid cysts, obvious paranasal sinus changes, consistent with pneumosinus dilatans, were noted. The frequency of the

Craniofacial resection. An alternative incision and skull base repair.

Neoplasia of the superior group of paranasal sinuses (frontal, ethmoidal and sphenoidal) has an extremely poor prognosis. Resection, preceded or followed by radiotherapy offers a 5 year survival of less than 30% (1). Satisfactory surgical excision has been undermined by difficulties of access to the cribriform plate area, a common site for residual disease. Furthermore the practice of piecemeal removal invariably resulted in inadequate clearance. The craniofacial approach allows not only excellent access to the anterior skull base but also in bloc excision. The results of treating 7 patients w

Perineurioma-like EMA-positive calvarial neoplasms: clinicopathological study of eight cases.

BACKGROUND: Incidental skull lesions are increasingly detected due to the widespread use of brain imaging. We have encountered an unusual cohort of unclassifiable calvarial spindle cell lesions showing phenotypic overlap with perineurioma and meningioma, which we sought to further characterize. MATERIAL AND METHODS: Cases of unclassifiable low-grade spindle cell lesions arising in the calvarium were collected. Clinical, radiological and pathological findings were reviewed. All cases were uniformly stained for a panel of meningothelial and perineurial immunomarkers. RESULTS: Eight cases were

Failure of the tyrosine kinase inhibitors osimertinib and erlotinib to manage a patient with EGFR-mutant lung adenocarcinoma: a case report.

BACKGROUND: Lung adenocarcinoma, a type of non-small cell lung cancer (NSCLC), is the most common type of lung cancer among non-smokers. Lung adenocarcinoma in exon 19 deletion (E19del) mutation-positive cases respond well to treatment with tyrosine kinase inhibitors (TKIs). Our case demonstrates the development of resistance to first- and third-generation TKIs in a 48-year-old woman with epidermal growth factor receptor (EGFR) mutation-positive advanced NSCLC. CASE DESCRIPTION: A 48 years old woman with no smoking history and no family history of cancer was diagnosed with EGFR mutation-posit

Devastating neurologic consequences of localized scleroderma en coup de sabre of the scalp-2 case studies.

BACKGROUND: En coup de sabre (ECDS) is a rare variant of localized scleroderma which can be associated with neurologic symptoms including seizures, focal neurologic deficit, and movement disorders. Little is known about the disease course in ECDS. We describe two patients with a history of localized scleroderma ECDS of the scalp who developed worsening neurologic symptoms coincidental in timing with extension of preexisting skin and skull lesions and evidence of inflammatory pathology on brain biopsy. CASE REPORT: Our first patient was diagnosed with localized scleroderma ECDS at the age of 1

Movements of the pelvic bones of expectant mothers during vaginal delivery.

Subtle but demonstrable movements in the expectant mother's pelvis occur during vaginal delivery in all the pelvic joints and anatomical planes of the body (sagittal, frontal, and transverse). The purpose of these movements is to gradually expand the space in the lesser pelvis via widening of the individual pelvic planes so that the newborn's head can enter the pelvic inlet, safely pass through the narrow planes of the pelvis, and through the pelvic outlet. From the point of view of biomechanics, these movements are described in literature as counternutation and nutation of the sacrum and ilia
