The value of tourniquet use for visibility during arthroscopy of the knee: a double-blind, randomized controlled trial.

PURPOSE: To study the value of tourniquet use during routine arthroscopy of the knee. METHODS: In this randomized, double-blind controlled trial, 245 patients were included from April 2005 until March 2007 and randomized into an inflated tourniquet group (n = 137) or a deflated tourniquet group (n = 108). The primary outcome was intraoperative visibility. Secondary outcomes were the need to intraoperatively inflate the tourniquet because impaired vision impeded the procedure, the number of times and reason (impaired vision or debris) for flushing the knee, the duration of the procedure (in mi

Intercondylar eminence avulsion fracture in children: long-term follow-up of 14 cases at the end of skeletal growth.

The purpose of this study was to report the long-term clinical outcome in a group of 14 patients treated for an avulsion fracture of the tibial spine. On the basis of the Meyers and McKeever classification, there were four type I avulsions, three type II and seven type III. Seven patients were treated nonoperatively and seven patients were operated according to one of the two different protocols: open reduction and internal fixation and arthroscopic reduction and internal fixation. At follow-up, four patients had clinical signs of joint instability, but no subjective instability. One patient w

Fractures of the posterolateral tibial plateau.

We reviewed the clinical records and operative notes of 28 patients with fractures of the posterolateral tibial plateau seen at our institution from 1949 to 1982. Five of the 28 patients had chronic depressions of the posterolateral tibial plateau after initial treatment elsewhere. All five were disabled because of significant functional instability when the knee was in flexion. There were 23 acute fractures, of which 4 were initially nondisplaced and treated nonoperatively. One nonoperative patient was lost to followup; the remaining three were rated as having had good or excellent results. N

Novel fat suppression technique for ultrashort echo time MRI using single-point Dixon phase modeling.

BACKGROUND: Fat suppression plays a vital role in numerous magnetic resonance imaging (MRI) examinations, particularly in the musculoskeleta … patella tendon, cartilage, posterior cruciate ligament (PCL), anterior cruciate ligament (ACL), and meniscus were evaluated in each knee. RESULTS: In the phantom experiment, there was a significant correlation between the estimated fat fraction and the actual fat fraction (R>0.98; P<0.05). The ex vivo experiment revealed a significant difference in contra …

Glaucocalyxin A delays the progression of OA by inhibiting NF-κB and MAPK signaling pathways.

BACKGROUND: Osteoarthritis (OA) is a common degenerative joint condition marked by inflammation and cartilage breakdown. Currently, there is … iated gene phenotypes. We also created a mouse model of OA induced by destabilization of the medial meniscus (DMM) instability, and GLA was administered intraperitoneally once every two days for eight weeks. Mice knee specimens were stained with hematoxylin-eosin, Safranin O/fast green, and immunohistochemical, and the Osteoarthritis Research Society Inter …

Case report: A case of injury to the infrapatellar branch of the saphenous nerve caused by medial approach in knee arthroscopy.

We present the case of a 72-year-old man who was referred to our department for treatment of pain on the anteromedial infrapatellar side of the right knee with sensory disturbance that began 2 years earlier. The patient previously underwent right knee arthroscopy at another hospital for a meniscus injury 2 years earlier, which relieved his knee pain, but pain and discomfort near the incision of the medial portal persisted. Given this situation, various physical treatments, such as ice compress, were administered postoperatively. However, the symptom was only partially relieved before discharge…

PCFP alteration was related to effusion-synovitis and tears of the medial meniscus. Clinical Anatomy of the Anterior Meniscofemoral Ligament of Humphrey: An Original MRI Study, Meta-analysis, and Systematic Review.

BACKGROUND: The anterior meniscofemoral ligament (aMFL) of Humphrey is an anatomically variable fibrous band of connective tissue that attaches between the lateral aspect of the medial femoral condyle and posterior horn of the lateral meniscus, running posterior to the anterior cruciate ligament and anterior to the posterior cruciate ligament (PCL). The presence of an intact aMFL may contribute to stabilization of the lateral compartment of the knee joint. PURPOSE: The original magnetic resonance imaging (MRI) arm of this study aimed to assess the aMFL incidence among Polish patients. The goa…

The Influence of Obesity and Meniscal Coverage on In Vivo Tibial Cartilage Thickness and Strain.

BACKGROUND: Obesity, which potentially increases loading at the knee, is a common and modifiable risk factor for the development of knee ost … d and uncovered regions of cartilage, which may indicate that the load-distributing function of the meniscus is not sufficient to moderate the effects of obesity. CLINICAL RELEVANCE: These findings demonstrate the critical effect of obesity on cartilage function and thickness in regions covered and uncovered by the menisci.

A comparative study of the quality of non-stimulated and stimulated tears in normal eye male subjects using the tear ferning test.

PURPOSE: To evaluate and compare the tear ferning patterns of non-stimulated and stimulated tears in normal eye male subjects using the tear ferning test. METHODS: Thirty-five male subjects aged 20-38 years with normal eyes were enrolled in the study. Subjects with dry eye or with a risk factor for eye dryness (eg, recent ocular surgery, contact-lens wearers, smokers, anemic, diabetes, and thyroid gland problems) were excluded from the study. Dry eye subjects were excluded based on the ocular surface diseases index scores, non-invasive tear break-up time, and tear meniscus height measurements…

Intra-articular clearance of labeled dextrans from naive and arthritic rat knee joints.

OBJECTIVE: Determine the effects of arthritis on the trans-synovial clearance of small and large model compounds following local delivery to the knee joint in a rat model. DESIGN: Intra-articular delivery was studied in rat knee joints in an osteoarthritis model of joint instability (medial collateral ligament and meniscus transection model or MMT). Fluorescently-labeled 10 kDa or 500 kDa dextran was injected in the arthritic or unoperated control (naive) joints 3 weeks after surgical destabilization, and the temporal clearance pattern was evaluated via in vivo regional fluorescence imaging, …

Popliteus tendon tenosynovitis.

This series of case suggests that the entity of tenosynovitis of the popliteus tendon is more common than once recognized. A high index of sucpicion and accurate palpation of the lateral aspect of the knee lead one to the diagnosis. Knowledge of this entity may prevent future misdiagnosis of tear of the lateral meniscus and unnecessary meniscectomy as experienced by Helfet, Holden, and myself. There is a definite correlation with activities requiring downhill walking or running. The runners invariably complained of the oneset of symptoms during downhill running rather than uphill running.
