Acute appendicitis presenting with scrotal symptoms is a rare event, occurring when a patent processus vaginalis persists. We present a case where ultrasound demonstrated an inflamed appendix and a scrotal abscess, allowing the correct surgical management in a difficult clinical situation. In a child presenting with scrotal signs and vague lower abdominal symptoms, an ultrasound assessment of the right iliac fossa should always be performed.

In Hacettepe University Children's Hospital, between 1976 and 1993 two patients among 302 with Hirschsprung's disease were diagnosed with appendiceal perforation (AP) at initial admission. Both patients were less than 2 months of age. One of them was a boy with total colonic aganglionosis and the latter a girl with long-segment disease. In both cases the site of AP was the base, and periappendicitis without mucosal involvement was detected. The present cases and review of the literature suggest that longer aganglionic segment carries a higher risk of AP.

A prospective study of 200 consecutive open emergency appendicectomies was carried out. All patients were informed before operation that they would be discharged the following day if possible. The operation was performed through a standard muscle-splitting incision. At the end of the operation the wound was infiltrated with bupivacaine and a diclofenac suppository was given. Two hundred and forty adults undergoing acute appendicectomy were randomised to receive either cefoxitin or ceftriaxone 1 g intravenously at induction of anaesthesia. In a retrospective study the frequency of infertility was estimated in women operated for perforated or non-perforated appendicitis before the age of 25. Of the 48 patients with simple perforation 19% were infertile, while this frequency was 31% in 16 patients with Douglas abscess. In the control group of 58 patients 12% could not have children. It is concluded that it is unlikely that appendicitis with perforation will cause infertility unless there is a Douglas abscess.

Primary adenocarcinoma of the appendix is the rarest form of malignant tumor of the appendiceal stump is described. As with other malignancies, surgical treatment is described. As with other malignancies, surgical treatment is most effective during the early stages of disease. In our series most of the patients who had metastatic disease had fairly rapid downhill courses. The Childs-Phillips plication operation was performed in forty-two patients, as treatment for recurrent small bowel obstruction in nineteen and as prophylaxis against future intestinal obstruction in twenty-three. Intestinal obstruction occurred later in only one patient. Eight of ten patients with acute peritonitis died in the postoperative period.
