Results, outcome predictors, and complications after stapled transanal rectal resection for obstructed defecation.

PURPOSE: Obstructed defecation may be treated by stapled transanal rectal resection, but different complications and recurrence rates have been reported. The present study was designed to evaluate stapled transanal rectal resection results, outcome predictive factors, and nature of complications. METHODS: Clinical and functional data of 123 patients were retrospectively analyzed. All patients had symptoms of obstructed defecation before surgery and had rectocele and/or intussusception. Of them, 85 were operated on by the authors and 38 were referred after stapled transanal rectal resection ha

Gastrointestinal stromal tumors (GISTs) on prostate needle biopsy: A clinicopathologic study of 8 cases.

Gastrointestinal stromal tumors (GISTs) are typically not included in the differential diagnosis of spindle cell tumors seen on prostate needle biopsy. However, their recognition is critical due to their unique clinical management. We report the rare phenomenon of 8 cases of GISTs diagnosed on prostate needle biopsy. The mean patient age at diagnosis was 53.6 years (range: 42 to 65 years). Tumors variably presented with rectal fullness, urinary obstructive symptoms, and abnormal digital rectal examination. Four tumors were resected. One of these cases was shown to be primary in the rectum with

Symptom patterns and relative distribution of functional bowel disorders in 1,023 gastroenterology patients in Iran.

BACKGROUND AND AIMS: There are scanty data on functional bowel disorder (FBD) patterns in Iran. This first-time study tried to provide preliminary data on relative distribution of different types of FBD and their symptom patterns in Iranian patients. METHODS: A consecutive sample of 1,023 patients in an outpatient gastroenterology clinic in central Tehran was interviewed using two questionnaires based on Rome II criteria from December 2004 to May 2005 to detect FBD patients. RESULTS: Of 1,023 gastroenterology patients, 410 met Rome II criteria for FBD; functional constipation, 115 (28%); irr

Emerging trends in peptic ulcer disease and damage control surgery in the H. pylori era.

The prevalence of peptic ulcer disease (PUD) and the frequency of operation have been decreasing for decades. Immigration of patients harboring Helicobacter pylori may reverse these longstanding declines. The experience with a large public hospital population in an area of high immigration may portend future national trends. A 10-year retrospective study analyzed the changing demographics of PUD and the frequency and nature of surgical intervention. A total of 2,182 patients were diagnosed with PUD, 1,173 in the early period (1995-1999) and 1,009 in the recent period (2000-2004). The proportio

Jejunal diverticulosis: a potentially dangerous entity.

Although jejunal diverticulosis is a rare entity and usually asymptomatic, it may cause chronic symptoms and acute complications. Because of the rarity of the entity, diagnosis is often delayed, resulting in unnecessary morbidity and mortality. The purpose of this study was to draw attention to jejunal diverticula and their complications. The medical records of 8 consecutive patients with complications due to small-bowel diverticula treated at our department during the past 4 years were reviewed. All diverticula were located in the jejunum. Seven patients had acute complications, 3 patients ha

Effects of intestinal surgery on pulmonary, glomerular, and intestinal permeability, and its relation to the hemodynamics and oxidative stress.

PURPOSE: To investigate the pattern of changes of gut mucosal, glomerular, and pulmonary permeability in response to major resectional intestinal surgery, and to evaluate whether these changes are related to oxidative stress. METHODS: Eight patients undergoing elective intestinal surgery. Lactulose/mannitol ratio (LMR), urinary albumin/creatinine ratio (MACR), and extravascular lung water and its ratio to intrathoracic blood volume (EVLW/ITBV) were measured preoperatively and at different time points postsurgery. The oxidant/antioxidant balance was assessed by measuring thiobarbituric acid-re

Acute post-gastric reduction surgery (APGARS) neuropathy.

BACKGROUND: A variety of neurologic complications have been reported after weight loss surgery. Recently, a new term was introduced to describe cases of postoperative polyneuropathy - acute post-gastric reduction surgery (APGARS) neuropathy, a polynutritional, multisystem disorder characterized by protracted postoperative vomiting, hyporeflexia, and muscular weakness. The incidence, associations, and prognosis of this disorder have not been precisely defined. METHODS: A questionnaire about features of APGARS was mailed to all members of the ASBS. Surgeons were asked about their surgical and b

Main pancreatic ductal anatomy can direct choice of modality for treating pancreatic pseudocysts (surgery versus percutaneous drainage).

OBJECTIVE: To test the hypothesis that pancreatic ductal anatomy may predict the likely success of percutaneous drainage of pseudocysts of the pancreas. SUMMARY BACKGROUND DATA: Various modalities are currently applied to pseudocysts, with little or no data to aid in the choice of management strategy. Pancreatic ductal anatomy was assessed and a system to categorize ductal changes was established. METHODS: Patients with a diagnosis of pancreatic pseudocyst were evaluated from 1985 to 2000. Two hundred fifty-three patients have been included in this series. Pancreatic ductal anatomy was defin

Strictureplasty in diffuse Crohn's jejunoileitis: safe and durable.

PURPOSE: As an alternative to resection, strictureplasty may allow for preservation of intestinal length and avoidance of short-bowel syndrome in patients with diffuse Crohn's jejunoileitis. However, the long-term durability of the procedure and its safety have not been confirmed. The purpose of this study was to report our experience with strictureplasty for diffuse Crohn's jejunoileitis. METHODS: Between 1984 and 1999, 123 patients underwent a laparotomy that included an index strictureplasty for diffuse jejunoileitis. Patient history, operative details, and postoperative morbidities were o

Results of surgical treatment (modified Sugiura-Futagawa operation) of portal hypertension associated to complete splenomesoportal thrombosis and cirrhosis.

BACKGROUND: Hemorrhagic portal hypertension, secondary to both intrahepatic and extrahepatic portal hypertension, is an uncommon entity. In this condition, the extrahepatic and the intrahepatic obstruction of the portal vein, due to chronic liver disease, produce a more severe form of hemorrhagic portal hypertension that is more difficult to control. The results of surgical treatment (modified Sugiura-Futagawa operation) in this subset of patients is analyzed. METHODS: Among 714 patients with a history of hemorrhagic portal hypertension, 14 cases were found with histologically proven liver ci

Adapted enhanced recovery after surgery pathway in emergency abdominal operations: A randomized controlled trial.

BACKGROUND: Emergency abdominal surgeries comprise most of emergencies and are high-risk procedures owing to their urgency and the limited time for optimization of comorbidities. Enhanced recovery after surgery (ERAS) protocols, in an adapted form, can potentially improve patient outcomes in emergency settings. The present study aimed to evaluate the safety, efficacy, and feasibility of adapted ERAS protocols in emergency abdominal operations. METHODS: This open-label, single-center, superiority, randomized controlled trial was conducted over 18 months. Patients with acute abdomen planned for

Timing of Nasogastric Tube Removal and Its Impact on Postoperative Bowel Function Recovery and Feeding Following Major Bowel Surgery in Children.

BACKGROUND: Although studies in adults have shown that early removal of the nasogastric tube (NGT) after major bowel surgery is safe and more beneficial than delayed removal, adoption of this practice in children has been slow due to limited supporting evidence. This study aimed to compare postoperative outcomes between early and delayed nasogastric tube removal in children undergoing major bowel surgery. METHODS: A total of 102 children who underwent major bowel surgery were randomized into two groups: Group I (early removal) and Group II (delayed removal), with 51 patients in each group. In

Temporal trends in postoperative outcomes after major abdominal surgery: a population-based cohort study.

BACKGROUND: Enhanced recovery after surgery protocols and minimally invasive surgery are strategies to mitigate postoperative complications, therefore complication rate is expected to decrease over time. We estimated temporal trends and variation in clinical and health system outcomes. METHODS: This was a population-based cohort study in Ontario, Canada, using data from the Institute of Clinical Evaluative Science. Male and female adults who underwent major elective abdominal surgery (gastroesophagectomy, pancreaticoduodenectomy, partial hepatectomy, colorectal resection) during 2005-21 were

Should we let the sun set or rise on a small bowel obstruction: mortality and readmission rates in operative versus nonoperative management.

BACKGROUND: Recurrent small bowel obstruction (SBO) can result in significant morbidity and mortality. In this study, we aimed to compare readmission and its impact on operatively and nonoperatively managed patients admitted with SBO. METHODS: We performed a retrospective analysis using the National Readmission Database 2016-2017 and included all patients (≥ 18y) who were non-elective admissions for SBO during the index hospitalization. Patients were dichotomized into two groups: Operative group (OM) and nonoperative group (NOM). Primary outcomes included rates of readmission and mortality up

Direct Acting Oral Anticoagulants Following Gastrointestinal Tract Surgery.

Direct-acting oral anticoagulants (DOACs) vary in bioavailability and sites of absorption in the gastrointestinal tract (GIT). Data on DOAC use after major GIT surgery are limited. The aim of this case series was to report the impact of surgical resection or bypass of the GIT on rivaroxaban and apixaban peak plasma concentrations. This was a case series of patients who received rivaroxaban or apixaban after GIT surgery, during the period of July 1, 2019, to December 31, 2020. Peak plasma concentrations of rivaroxaban and apixaban were assessed for the expected concentrations. Of the 27 assesse

Previous upper abdominal surgery is a risk factor for nasogastric tube reinsertion after pancreaticoduodenectomy.

BACKGROUND: Pancreaticoduodenectomy without subsequent nasogastric tube management has not been widely adopted due to delayed gastric emptying, the specific and frequent morbidity associated with this surgical procedure. We assessed the feasibility of pancreaticoduodenectomy without use of nasogastric tubes and the risk factors for subsequent nasogastric tube reinsertion. METHODS: We retrospectively reviewed 465 patients who underwent pancreaticoduodenectomy at a single institution between 2010 and 2019. Primary endpoint was the rate of nasogastric tube reinsertion.
