BACKGROUND: Deterioration of postoperative local biomechanical environment is a primary contributor to varus deformity of the femoral head in patients with intertrochanteric fractures treated with proximal femoral nail antirotation (PFNA) fixation. Theoretically, increasing the number of threaded anchors can optimize the local biomechanical environment and enhance the fixation stability. According to this principle, a larger gap between the intramedullary nail and the surrounding femoral shaft at the distal locking screw may compromise fixation stability by reducing the number of thread anchor

PURPOSE: The number of total knee replacements (TKRs) performed per year has been increasing annually and it is estimated that by 2030 demand would reach 3.48 million procedures per year in the United States Of America. The prevalence of periprosthetic fractures (PPFs) around TKRs has followed this trend with incidences ranging from 0.3% to 3.5%. Distal femoral PPFs are associated with significant morbidity and mortality. When there is sufficient bone stock in the distal femur and a fracture pattern conducive to fixation, locking compression plating (LCP) and retrograde intramedullary nailing

BACKGROUND: The timing of fixation of femoral fractures in multiply injured patients with severe thoracic trauma is discussed controversially. Some authors recommend damage control surgery, whereas other authors prefer early definitive treatment. The aim of our study was to investigate the effect of early definitive fixation of femoral fractures on outcomes in multiply injured patients with severe thoracic trauma.
METHODS: Between May 1, 1998 and December 31, 2004, 578 severely injured patients were admitted to our institution. Forty-five patients met the inclusion criteria for the study coho

BACKGROUND: Children aged 6 months to 5 years with diaphyseal femur fractures are typically treated with spica casting, as recommended by the American Association of Orthopaedic Surgeons clinical practice guideline. We aimed to determine the incidence of secondary interventions after early spica casting for femur fractures in children aged 6 years or less.
METHODS: This was a retrospective cohort study of patients aged 6 years or less with diaphyseal femur fractures treated with early spica casting at a single Canadian tertiary care, level 1 trauma pediatric centre between January 2005 and Ma

We report a case series of three patients who sustained open Gustilo-Anderson Type IIIB tibia fractures associated with extensive segmental bone and soft tissue loss. The patients initially were treated with serial wound irrigations, débridements, and external fixation. After the soft tissue envelope was reconstructed successfully, each large segmental bone defect was reconstructed with a cylindrical titanium mesh cage packed with a composite of cancellous allograft and demineralized bone matrix putty and stabilized with a statically locked intramedullary nail. The mean segmental bone loss was

BACKGROUND: Nonunion of the humeral shaft presents a complex surgical challenge, with ongoing debate surrounding optimal fixation and grafting strategies. This study evaluates the clinical and radiographic outcomes of a novel surgical approach, intramedullary nailing combined with circumferential (360°) cortical strut allografting, termed the Sarcophagus technique. We hypothesized that the Sarcophagus technique would be a safe and effective treatment strategy for humeral shaft nonunion, offering union rates comparable to conventional techniques.
METHODS: We conducted a retrospective case seri

BACKGROUND: Simvastatin increases the expression of bone morphogenetic protein 2 (BMP-2) in osteoblasts, therefore it is important to investigate the influence of statins on bone formation, fracture healing and implant integration. The aim of the present study was to investigate the effect of simvastatin, locally applied from intramedullary coated and bioactive implants, on bone integration using biomechanical and histomorphometrical analyses.
METHODS: Eighty rats received retrograde nailing of the femur with titanium implants: uncoated vs. polymer-only (poly(D,L-lactide)) vs. polymer plus dr

BACKGROUND: Atypical femur fracture (AFF) is a clinically important complication of bisphosphonate (BP) use in the treatment of osteoporosis. The benefits of long-term BP therapy in preventing osteoporotic fractures have been shown to outweigh the risks of treatment. Discontinuation of BPs or "drug holidays" have been implemented as a strategy to reduce the risk of rare complications such as AFF.
CASE REPORT: We present the case of a 70-year-old postmenopausal woman who suffered bilateral AFF ten years after discontinuation of BP treatment. Management of this patient included fixation of the

INTRODUCTION: Treatment of proximal humeral fractures with plate osteosynthesis or intramedullary nail fixation in humeral shaft fractures with a proximal locking bolt carries the risk of iatrogenic injury of the axillary nerve. The purpose of this anatomical study is to define a more reliable safe zone to prevent iatrogenic axillary nerve injury using the humeral head instead of the acromion as a (radiographic) reference point during operative treatment.
MATERIALS AND METHODS: Anatomical dissection and labeling of the axillary nerve and branches was performed on 10 specially embalmed human s

Twenty-two elderly patients (average age, seventy-two years) who had an atrophic, unstable, ununited fracture of the humeral diaphysis were managed with plate-and-screw fixation and application of an autogenous bone graft from the iliac crest. Fifteen of the patients had had at least one previous operation in an attempt to obtain union of the fracture. One patient had an active infection and two had a quiescent infection, all with Staphylococcus epidermidis. The average duration of nonunion before the patients were first seen by us was two years and four months (range, five months to sixteen y

The blocking intramedullary osteosynthesis (BIO) of the forearm bones possesses quite a number of advantages in comparison with the bone and transosseous methods. The BIO allows the restoration of the anatomy and biomechanics of the forearm, it shortens the term of disability and restores the function of the extremity in proximate terms. The BIO of the forearm bones consists of great number of nuances, each of them is really important. It is necessary to have the X-ray unit with electron-optical image for the intraoperative monitoring of operation stages and to follow the well-defined algorith

PURPOSE/BACKGROUND: Therapy of metacarpal neck fractures depending on radiographically measured palmar angulation is discussed controversially in the literature. Some authors describe normal hand function of malunited metacarpal neck fractures with a palmar angulation up to 70°; others define 30° as the uppermost limit to maintain normal hand function. However, the methods of measuring palmar angulation are not clearly defined. Here, we present a new method to measure palmar angulation using ultrasound. The aim of this prospective study is to compare the radiographic methods of measuring palma

We present our therapeutic strategy for the treatment of type IIIB open tibial fractures. It involves emergency internal stabilisation of the bone by locked intra-medullary nailing when appropriate and skin cover using either a pedicled or free muscle flap. Where there is bone loss, a cancellous iliac graft is performed at the same time. Eighteen cases of type IIIB open tibial fractures treated between 1986 and 1995 were analysed. There were 17 men and 1 woman; the average age was 35 years. Each of the 18 patients underwent wound debridement as a primary emergency procedure with no secondary r

Between 1995 and 1997 we implanted 45 Marchetti nails. These nails were implanted for traumatic fractures in 39 patients, but also for pathologic fractures in 5 patients. Among these 39 patients, 7 were lost of view, the average age was 60 years. Among these 39 patients, 4 have got an other treatment before which failed. 10 fractures were placed D1, 8 fractures placed D2, 5 fractures placed D3, 16 fractures placed D4 according to the classification of Hackethal modified by De la Caffinière. The fracture type was classified according to A. O. classification: 7 A1, 11 A2, 7 A3, 2 B1, 6 B2, 1 B3,

Management of patients with deformities due to congenital pseudarthrosis of the tibia continues to be controversial. The use of free vascularized fibular grafts in the reconstruction of these deformities appears to be successful in this patient population but is not without potential for significant complications. Long-term evaluation, at least to skeletal maturity, is necessary to judge the success of any procedure done for any pediatric patient. This case illustrates the natural history of the vascularized free fibular graft for congenital pseudarthrosis of the tibia in a young child. Compli

Rapid restoration of maximal extremity function has stimulated the aggressive treatment of tibia fractures with intramedullary nails. Rod migration is a well-described complication of flexible or unlocked intramedullary rods. Interarticular extrusion of an unlocked tibial rod occurred in a patient after a second accident. Arthroscopy was used to evaluate the chondral injury, as well as to advance the rod.
