Health Equity in a Post 'Roe Versus Wade' America.

For almost five decades, the US Supreme Court had protected the right to abortion care in the United States. However, the Court's decision in 'Dobbs versus Jackson Women's Health Organization' in 2022 established that the US Constitution does not confer a right to abortion, effectively overturning the Court's own previous judgment in 'Roe versus Wade' from 1973. How could a decision that overturns the precedent for nationwide access to abortion affect women, children, and physicians? How will state laws impact healthcare equity regarding reproductive rights going forward? How will geography af

From village health volunteers to paid care givers: the optimal mix for a multidisciplinary home health care workforce in rural Thailand.

BACKGROUND: Thailand is a rapidly aging society, which places high demand on home health care services for the elderly. The shortage of health care workforce in rural areas is a crucial obstacle to the delivery of adequate home health care services. The appropriate skill-mix between multidisciplinary health team and care givers (CGs) is an attractive solution for improving home health care services in rural Thailand. This study assessed the potential of trained CGs to provide home health care services and projected what the optimal mix for a multidisciplinary home health care team in rural Tha

A Cross-Cultural Study of Filial Piety and Palliative Care Knowledge: Moderating Effect of Culture and Universality of Filial Piety.

Filial piety is a Confucian concept derived from Chinese culture, which advocates a set of moral norms, values, and practices of respect and caring for one's parents. According to the dual-factor model of filial piety, reciprocal and authoritarian filial piety are two dimensions of filial piety. Reciprocal filial piety is concerned with sincere affection toward one's parent and a longstanding positive parent-child relationship, while authoritarian filial piety is about obedience to social obligations to one's parent, often by suppressing one's own wishes to conform the demands of the parent. Treatment variables associated with outcome in emergency department patients with suspected sepsis.

BACKGROUND: Early treatment is advocated in the management of patients with suspected sepsis in the emergency department (ED). We sought to understand the association between the ED treatments and outcome in patients admitted with suspected sepsis. The treatments studied were: (i) the time to antibiotics, where time zero is the time the patient was booked in which is also the triage time; (ii) the volume of intravenous fluid (IVF); (iii) mean arterial pressure (MAP) after 2000 ml of IVF and (iv) the final MAP in the ED. METHODS: We performed a retrospective analysis of the ED database of pati

[Exposure to professional violence by young physicians in the hospital: MESSIAEN national study].

BACKGROUND: Hospital professional violence is defined as hostile and aggressive behavior exerted by health professionals on other health professionals. No quantitative study has been carried out to date on French hospital professional violence among young physicians, while recent qualitative studies have suggested a potential high frequency. The main objective was to determine the prevalence of exposure of young doctors to hospital violence. The secondary objective was to determine their characteristics and consequences as well as to determine if students and young physicians (resident and you

Factors Associated With Recovery Room Intravenous Opiate Requirement After Pediatric Outpatient Operations.

BACKGROUND: Many children recovering from anesthesia experience pain that is severe enough to warrant intravenous (IV) opioid treatment within moments of admission to the postanesthesia care unit (PACU). Postoperative pain has several negative consequences; therefore, preventing significant PACU pain in children is both a major clinical goal and a moral/ethical imperative. This requires identifying patient-level and perioperative factors that may be used to predict PACU IV opioid requirement. This should allow for the development of personalized care protocols to prevent clinically significant

Potential of human twin embryos generated by embryo splitting in assisted reproduction and research.

BACKGROUND: Embryo splitting or twinning has been widely used in veterinary medicine over 20 years to generate monozygotic twins with desirable genetic characteristics. The first human embryo splitting, reported in 1993, triggered fierce ethical debate on human embryo cloning. Since Dolly the sheep was born in 1997, the international community has acknowledged the complexity of the moral arguments related to this research and has expressed concerns about the potential for reproductive cloning in humans. A number of countries have formulated bans either through laws, decrees or official stateme

[At the beginning of the third millennium: what kind of surgery, what kind of surgeon?].

Technological progress and the evolution of biological knowledge lead us to expect major developments in the fields of organ replacement, minimally invasive surgery and tumour therapy in the coming decades. It is widely believed that xenotransplantation may offer a possible solution to the problem of the current chronic shortage of donors, though their use at present encounters formidable immunological obstacles, particularly as regards discordant transplants. Genetic engineering and the realisation of transgenic donors as well as new strategies aimed at modifying the immune status of the dono

Understanding resilience among workers in elderly care: insights from a mixed-methods study.

BACKGROUND: Workers in elderly care face demanding conditions that may affect both staff well-being and quality of care. Resilience has been identified as a protective factor in this context; however, evidence on its predictors in elderly care settings remains limited. This study investigated the personal-level and organizational-level predictors of individual resilience among healthcare workers employed in residential facilities for older adults and people with disabilities. METHODS: As part of the European Horizon 2020 project Support4Resilience, a cross-sectional mixed-methods study was co

Knowledge, attitude, and acceptance of euthanasia among medical students and doctors in Pakistan: A cross-sectional study.

Globally, euthanasia is a strongly debated and ethically complex issue, with views shaped by cultural, religious, and moral precepts. In conservative cultures like Pakistan, euthanasia discussions are sensitive and occasionally ignored. This study aims to evaluate medical professionals' and students' knowledge, attitudes, and acceptance of euthanasia in Pakistan. From May 16 to June 16, 2025, a cross-sectional survey was conducted involving 10 Pakistani medical institutions. The study utilized convenience sampling to select MBBS first- and final-year students and practicing doctors with a mini

Health Inequalities, Intersectionality, and Justice.

UNLABELLED: Policy Points Population health science that uses large data sets to study health inequalities increasingly draws on intersectionality, the idea that overlapping forms of disadvantage shape health inequalities. This approach is essential to the pursuit of health equity because it provides a more complete and sound evidence base for population health policy. As this approach has gained traction, it has drifted from intersectionality's founding ideas about social power and justice. This Perspective develops a framework of justice for the field that consists of a multidimensional acco

Ethics of male circumcision in minors: a scoping review of parental authority, adolescent involvement, and implications for sexual medicine practice and policy.

INTRODUCTION: Male circumcision in minors remains widely practiced in many regions of the world, although its prevalence varies considerably across cultural, religious, and geographic contexts. Beyond its medical dimensions, circumcision is closely linked to religion, culture, masculinity, family identity, and public health agendas. Ethical debates commonly involve tensions between parental authority, bodily autonomy, assent, and children's rights, particularly because minors are often minimally involved in decision-making processes. METHODS: A scoping review was conducted to examine ethical,

Patient and Family Perspectives on Statutory Duty of Candour Following Serious Adverse Patient Safety Events.

BACKGROUND: The Statutory Duty of Candour (SDC) legally requires healthcare organisations to notify patients, families, and carers who experience a serious adverse patient safety event (SAPSE), provide an apology, and explain steps taken to prevent recurrence. However, despite growing international adoption of SDC, evidence on its implementation and impact on patients and families ('consumers') remains limited. This study examined consumer awareness, understanding, and lived experiences of SDC in Victoria, Australia, two years after its introduction. METHODS: A mixed-methods study integrated

Exploring the Impact of Home-Based Support on Informal Caregivers: A Qualitative Study in Hyderabad, Pakistan.

Background An aging population in Pakistan is relying more and more on informal caregivers as organized elder care systems are hard to come by. In cities such as Hyderabad, caregiving is largely a family-oriented responsibility, and it is deeply rooted in culture and religion. Women disproportionately shoulder the burden of caregiving and experience emotional, physical, and financial distress as a result. While the global literature acknowledges caregiving as fundamental to health systems, there is scant research evidence on the experiences of caregivers in low-resource South Asian settings. T
