Bilateral fractures of the tibia: a severe injury associated with multiple trauma.

Trauma which results in bilateral fractures of the tibia represents a serious physical insult to the victim. A review of our experience at the Harborview Medical Center in the treatment of 14 consecutive patients with this injury provides insights into the management of multiply injured patients with long bone fractures. All of the 28 tibial fractures were the result of high-energy forces, and 16 were open injuries. Thirteen of the patients had sustained severe multiple trauma and three died as a result of their injuries. All surviving patients were followed until complete healing and long-ter

Closed Reduction and Percutaneous Pinning Versus Open Reduction and Internal Fixation for Type II Lateral Condyle Humerus Fractures in Children Displaced >2 mm.

BACKGROUND: The optimal treatment for pediatric lateral condyle fractures displaced >2 mm, but with little joint displacement is unclear. The purpose of this study was to assess clinical and radiographic outcomes and complication rates of patients undergoing open reduction internal fixation (ORIF) versus closed reduction and percutaneous pin fixation (CRPP) of lateral condyle fractures with >2 mm of displacement and no obvious articular surface incongruity. METHODS: A retrospective review of all children with acute lateral condyle fractures with 2.1 to 5.0 mm of displacement treated from 2006

[Monteggia lesions in children--a clinical trial].

BACKGROUND: The Monteggia lesion is a rare injury of the elbow in children. This lesion is defined as a shaft fracture of the ulna combined with a luxation of the radial head. The primary problem in this injury is the proper diagnosis because the misalignment of the radial head is often missed. The purpose of this study was to retrospectively analyse the results after Monteggia lesions in children. PATIENTS AND METHODS: Ten children between two and nine years of age (six boys and four girls) were clinically assessed using the Morrey score. They were treated in our department between December

[Growth prognosis after para-epiphyseal tangential epiphysial injuries of the lower extremity exemplified by the distal femur: outcome after injuries of epiphyses of the distal femur].

Fractures of the distal end of the femur are rare. Premature partial closure of the physis may occur after metaphyseal fractures. After epiphyseal fractures it has a high incidence of occurrence. The cause of physeal arrest is not known. Stimulation of the physes ends in leg length discrepancy and was seen in 50% of all cases. Spontaneous correction of an axis deviation is possible after antecurvation up to an age of 5 years. But this should be only accepted during therapy of metaphyseal fractures of the distal femur. Side-to-Side deviations remodel in all cases. Aim of therapy in distal epiph

Biomechanical evaluation and analysis of a reused telescopic intramedullary femoral nail in a child with achondroplasia.

INTRODUCTION: Telescopic intramedullary nails (TIMN) have represented a significant advancement in limb lengthening procedures. However, their elongation capacity is limited to 5cm in the bones of patients with achondroplasia. Consequently, techniques involving TIMN reutilization have been developed. This reuse presents mechanical and safety challenges due to material fatigue and repetitive loading, which may compromise the structural integrity of the implant. This study evaluates the biomechanical performance and potential damage of a reused TIMN. METHODS: An experimental analysis was conduc

Comparison of hindfoot arthrodesis nail and humeral intramedullary nail for tibiotalocalcaneal arthrodesis.

BACKGROUND: Tibiotalocalcaneal (TTC) arthrodesis is a standard procedure for severe hindfoot deformities. While both hindfoot arthrodesis nails (HAN) and repurposed humeral intramedullary nails (HIN) are used for fixation, direct comparisons of their outcomes are scarce. PURPOSE: This study aimed to directly compare the surgical, clinical, and functional outcomes of HAN and HIN fixation in TTC arthrodesis. STUDY DESIGN: Retrospective comparative cohort study. METHODS: This retrospective cohort study included 55 patients who received TTC arthrodesis from 2019 to 2023. Based on the implant us

Subtalar joint involvement with tibiotalocalcaneal intramedullary nail arthrodesis.

Tibiotalocalcaneal (TTC) fusion with an intramedullary nail (IMN) has been utilized for a myriad of indications in hindfoot and ankle reconstruction. However, some controversies remain on the optimal position of the hindfoot. Previous studies have reported on the potential medialization of the rearfoot during insertion of the IMN, but few studies have examined the potential affect on the subtalar joint. We performed the present cadaveric study in order to assess the involvement of a 12-mm IMN with the posterior facet of the calcaneus. A 3-mm guide wire (for a standard TTC IMN) was inserted in

Limb Lengthening With Precice Intramedullary Lengthening Nails in Children and Adolescents.

BACKGROUND: The Precice intramedullary bone lengthening nail has been used in our department since 2013. We sought to determine the efficacy and safety of intramedullary limb lengthening with Precice nails in children and adolescents. METHODS: We retrospectively investigated patients 18 years and younger who underwent lower-limb lengthening using the Precice nail. Radiologic and clinical outcome data were obtained from a prospective database. The minimum postimplantation follow-up was 12 months. Between March 2013 and March 2020, 161 patients underwent limb lengthening with a Precice nail; 76

Mechanical Failures in Magnetic Intramedullary Lengthening Nails.

BACKGROUND: Magnetic intramedullary lengthening nails (MILNs) have gained popularity in recent years for the treatment of limb-lengthening discrepancies. We sought to catalog mechanical failures and their prevention and management in a large, single-institution series. We specifically assessed the rate of mechanical failures, the types of failures observed, and management strategies. METHODS: We retrospectively reviewed 377 patients (420 limbs) who underwent limb lengthening with early (P1) or later (P2) MILN iterations with ≥12-month follow-up. Mechanical failure was defined as mechanical br

Acute Disseminated Intravascular Coagulation After Prophylactic Nailing of the Femur in a Patient with Metastatic Prostate Cancer: A Case Report.

CASE: We present a case of acute disseminated intravascular coagulation (DIC) after prophylactic femoral intramedullary stabilization in a patient with metastatic prostate cancer. Preoperative international normalized ratio of 1.4 was attributed to malnutrition, and the patient was not medically optimized. DIC developed 1 hour postoperatively and was managed with blood product resuscitation. At the 4-month follow-up, the patient presented with bilateral pulmonary emboli and was transitioned to palliative care. CONCLUSIONS: DIC after intramedullary stabilization in patients with metastatic bon

[Orthopedic management in children with fibrous dysplasia of bone].

The method and results of treatment in 9 children with fibrous dysplasia of bone and 3 children with Albright syndrome are presented. Curettage and filling of the defect with autologous and/or lyophilized grafts failed in all 5 attempted cases. Due to pathological fracture or deformity Rush nailing combined with multilevel osteotomies has been performed within 5 femoral bones and 4 tibial bones. Healing was undisturbed, neither fracture nor deformity recurred. Valgus osteotomy has been done in 2 children with pseudoarthrosis of the femoral neck. In one case osteotomy healed, in another bony un

Iatrogenic Z-deformity of the proximal femur in an osteoarthritic hip. Subtrochanteric osteotomy with intramedullary fixation for arthroplasty of the hip. A case report.

Hip osteoarthritis in its final stage may require arthroplasty. Sometimes an extreme Z-deformity of the proximal femur is present, if a previous valgus trochanteric osteotomy has been performed. To correct a Z-deformity of the proximal femur during total hip arthroplasty, a subtrochanteric corrective osteotomy was performed in a 52 year-old woman. Intramedullary fixation of the osteotomy was secured using a standard long-stem isoelastic femoral component alone.

Early semiclosed intramedullary stabilization in osteogenesis imperfecta.

Fourteen patients with severe osteogenesis imperfecta (OI) were treated operatively by closed or semiclosed intramedullary stabilization, with nonextensible rods and nails. Altogether, 55 bones (both upper and lower limbs) were stabilized primarily. In infants with a severe form of OI (Sillence classification), the operative technique was modified significantly from those presented earlier in the literature. Early intramedullary stabilization, even soon after birth, seems justified in selected patients with severe OI. The possibilities for motor development are improved and later insertion of

Unusual Femur Stress Fractures in Children With Osteogenesis Imperfecta and Intramedullary Rods on Long-term Intravenous Pamidronate Therapy.

In this report, we describe 6 children with osteogenesis imperfecta with unusual stress femoral fractures. All children were on long-term cyclic pamidronate treatment. All fractures occurred without trauma or with minimal trauma and were located in the subtrochanteric or the diaphyseal regions of the femur over preexisting intramedullary rods. These fractures have very similar features to the reported minimal trauma atypical femoral fractures in adults on long-term bisphosphonate treatment. These fractures raise concerns about the role of prolonged remodeling suppression and microdamage accumu

The standard Gamma nail or the Medoff sliding plate for unstable trochanteric and subtrochanteric fractures. A randomised, controlled trial.

We studied 217 patients with an unstable trochanteric or subtrochanteric fracture who had been randomly allocated to treatment by either internal fixation with a standard Gamma nail (SGN) or a Medoff sliding plate (MSP, biaxial dynamisation mode). Their mean age was 84 years (65 to 99) and they were reviewed at four and 12 months after surgery. Assessments of outcome included general complications, technical failures, revision surgery, activities of daily living (ADL), hip function (Charnley score) and the health-related quality of life (HRQOL, EQ-5D). The rate of technical failure in patients

Antibiofilm-coated intramedullary nailing for tibial and femoral diaphyseal fractures at high-risk infection: outcomes of 103 consecutive patients.

BACKGROUND: Fracture-related infection (FRI) is a devastating complication of intramedullary nailing of tibial and femoral diaphyseal fractures. The present study evaluates the clinical efficacy of an antibacterially coated intramedullary nail in patients with high-risk tibial and femoral diaphyseal fractures. Open fractures, fractures requiring fasciotomy or closed fractures with Tscherne grade II or III soft-tissue damage were considered fractures at high risk of infection. METHODS: A single-centre observational study was conducted between January 2022 and December 2024, in accordance with

Femur Fractures Distal to Long and Short Cephalomedullary Nails: A Comparative Multicenter Study.

INTRODUCTION: Cephalomedullary nails (CMNs) are commonly used to treat intertrochanteric hip fractures. Subsequent fractures distal to an existing CMN are a challenging complication with patterns and management that vary based on nail length. Whether short or long CMNs allow for less complicated surgical revision or better postoperative outcomes is unknown. The aim of this study was to compare the treatment strategies and outcomes of fractures distal to short versus long CMNs. METHODS: This multicenter retrospective study identified 58 patients who underwent surgical management of a fracture
