Elastic stable intramedullary nailing (ESIN) in paediatric femur and tibia shaft fractures: Comparison between titanium and stainless steel nails.

INTRODUCTION: Elastic stable intramedullary nailing (ESIN) is currently considered the gold standard in surgical treatment of femur and tibial shaft fractures in school age paediatric patients. Although elastic intramedullary nails are available in both titanium (Ti) and stainless steel (SS) alloy, titanium nails are most commonly used. Nevertheless, there is still contrasting evidence as to whether the use of Ti nails can offer better outcomes in terms of fracture healing and stability over SS nails. The aim of this study was to compare outcomes and complications of Ti and SS ESIN for femur a

[Osteosynthesis with a rod in crushing injuries of the knee joint and periarthric fractures of the femur and crural bones].

The authors describe the treatment of 14 patients having crushing injuries of the knee joint of periarthric fragmentation fractures of femoral and crural bones with a wide zone of soft tissues damage. Economic resection of the joint with subsequent formation of a juncture between the femoral and tibial bones with a long rod (65--75 cm) seems to be mostly rational.

Can a linear external fixator stand as a surgical alternative to open reduction in treating a high-grade supracondylar humerus fracture?

OBJECTIVE: High-grade pediatric supracondylar humerus fractures are commonly treated with closed reduction and internal fixation with percutaneous pinning. When this fails, open reduction followed by internal fixation is the widely accepted procedure of choice. Use of a lateral external fixator was recently described as an optional procedure, but evidence is scarce. METHODS: We investigated the outcomes of upper limbs treated by either open reduction with internal fixation or closed reduction and external fixation. RESULTS: Twenty-one patients completed the long-term follow-up; 11 underwent

Presence of lesser trochanter fragments is a risk factor for malrotation in subtrochanteric femoral fractures: A multicenter, retrospective, observational study.

BACKGROUND: Subtrochanteric femoral fractures are challenging injuries that are often associated with complications such as malrotation. Although imaging indicators such as the lesser trochanter sign are commonly used to achieve proper rotational alignment during surgery, their reliability diminishes in the presence of lesser trochanter fragments. This study investigated the relationship between lesser trochanter fragments and rotational deformities in patients treated with antegrade intramedullary nailing for subtrochanteric fractures. METHODS: This multicenter retrospective analysis include

Should we remove flexible intramedullary nails after diaphyseal forearm pediatric fracture? A cohort study of 288 patients.

BACKGROUND AND PURPOSE: Routine removal of elastic stable intramedullary nails (ESIN) after pediatric diaphyseal forearm fractures remains common practice despite limited supporting evidence. We aimed to evaluate refracture risk following ESIN retention compared with implant removal and assessed complications and healthcare resource use. METHODS: In this retrospective cohort study, patients treated with ESIN for diaphyseal forearm fractures at a single center were included. Following a policy change, routine implant removal was discontinued in favor of retaining implants unless symptomatic. P

Clinical outcomes of intramedullary nailing for tibial shaft fractures caused by low-energy gunshot versus Gustilo-Anderson type I tibial shaft fractures.

BACKGROUND: Tibial shaft fractures caused by low-energy gunshot injuries represent a rare but challenging clinical problem. The optimal treatment approach, particularly in comparison with Gustilo-Anderson Type I open fractures, remains controversial. METHODS: This was a retrospective comparative study that included patients who underwent intramedullary nailing for tibial shaft fractures. Group 1 consisted of low-energy gunshot-induced fractures, while Group 2 included Gustilo-Anderson Type I open fractures. Demographic data, union time, Radiographic Union Score for Tibial Fractures (RUST), Jo

Calcaneal locking intramedullary nailing versus extensile lateral plating for displaced intra‑articular calcaneal fractures: a propensity‑weighted cohort study focusing on perioperative soft‑tissue burden and early recovery trajectories.

BACKGROUND: Displaced intra-articular calcaneal fractures (DIACFs) are among the most challenging hindfoot injuries and are often associated with persistent disability, reduced quality of life, and treatment-related complications [1, 2]. Although extensile lateral open reduction and plate fixation can restore calcaneal anatomy, it is accompanied by substantial soft-tissue morbidity [3-6]. Locking intramedullary nailing (IMN) is less invasive, yet comparative studies often emphasize overall complication rates and long-term functional scores, with limited quantification of perioperative soft-tis

Iatrogenic Compartment Syndrome After Delayed Primary Closure of the Tibial Fracture-Related Leg Fasciotomy Wound: A Case Report.

CASE: Compartment syndrome can occur after tibial fracture and requires prompt diagnosis and immediate fasciotomy. Because of post-traumatic swelling, delayed primary wound closure can be difficult requiring significant tension on the skin. Closing the skin in this setting theoretically puts the patient at risk of elevated compartment pressures, although compartment syndrome has never been reported in these circumstances. We describe a case of compartment syndrome that developed after delayed primary skin closure of a single incision 4-compartment fasciotomy wound after tibial fracture. CONCL

Late Medial Migration of Lag Screw After Gamma3 Nailing Threatening the Internal Iliac Vessels: A Case Report.

CASE: A 77-year-old woman underwent Gamma3 Nail (Stryker) for an extracapsular fracture of the neck of femur on the left side. Eight months later, the lag screw had migrated medially into the pelvis and was in contact with the internal iliac vessels. After a multidisciplinary team meeting, embolization of the internal iliac artery was carried out prophylactically before the removal of screw and conversion to a total hip replacement. CONCLUSION: Although there are increasing number of case reports on late medial migration of lag screw, medial migration into pelvis with vascular complication is

Long-term Comparison of Retrograde and Antegrade Femoral Nailing.

Antegrade or retrograde nailing for femoral shaft fractures remains the gold standard, but long-term data on functional outcomes after intramedullary nailing are lacking. In a retrospective review of prospectively collected patient registry data, patients with an isolated femoral shaft fracture treated with antegrade or retrograde femoral nailing from 1997 to 2012 were interviewed and their medical records analyzed. Functional reported outcome data were obtained via the visual analog scale (VAS) for pain and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) at 5 to 17

Fracture of a Carbon Fiber Reinforced Intramedullary Femoral Nail.

Carbon fiber reinforced implants have emerged as an alternative to traditional stainless steel and titanium implants because of the improved visibility on advanced imaging and excellent biomechanical strength. This can be advantageous in patients with concern about fracture healing, tumor recurrence, or additional imaging requirements. However, limited long-term data are available to present complications from the use of these implants. We present the case of a failure of a carbon fiber reinforced intramedullary femoral nail used in a closed distal femur fracture.

Prognostic factors affecting survival of patients with pathologic humerus shaft fractures treated with intramedullary nailing without tumor removal.

BACKGROUND: Intramedullary nailing is the procedure of choice for pathologic fractures in humeral shaft because it allows early pain relief and mobilization. The aim of this study was to analyze the prognostic factors affecting survival of patients with pathologic humeral shaft fractures treated with intramedullary nailing without tumor removal. METHODS: We performed a retrospective study by evaluating the records of patients treated in our clinic between 2003 and 2018 for pathologic humerus shaft fractures with a minimum follow-up of one year. Trochanteric fracture-implant motion during healing - A radiostereometry (RSA) study.

Cut-out complication remains a major unsolved problem in the treatment of trochanteric hip fractures. A better understanding of the three-dimensional fracture-implant motions is needed to enable further development of clinical strategies and countermeasures. The aim of this clinical study was to characterise and quantify three-dimensional motions between the implant and the bone and between the lag screw and nail of the Gamma nail. Radiostereometry Analysis (RSA) analysis was applied in 20 patients with trochanteric hip fractures treated with an intramedullary nail. The following three-dimensi
