Restriction in lateral bending range of motion, lumbar lordosis, and hamstring flexibility predicts the development of low back pain: a systematic review of prospective cohort studies.

BACKGROUND: Low back pain (LBP) is an increasingly common condition worldwide with significant costs associated with its management. Identification of musculoskeletal risk factors that can be treated clinically before the development of LBP could reduce costs and improve the quality of life of individuals. Therefore the aim was to systematically review prospective cohort studies investigating lower back and / or lower limb musculoskeletal risk factors in the development of LBP. METHODS: MEDLINE, EMBASE, AMED, CINAHL, SPORTDiscus, and the Cochrane Library were searched from inception to Februa

A cross-sectional analysis of sagittal knee laxity and isokinetic muscle strength in soccer players.

In this cross-sectional study, sagittal knee laxity and isokinetic strength of knee extensor and flexor muscle groups were measured and differences related to leg dominance were evaluated. A total of 44 healthy male soccer players (who had trained regularly at least for the last five years) and 44 sedentary people as their control counterparts were involved in this study. All participants were tested using a KT-1000 knee arthrometer for knee laxity. Isokinetic concentric knee peak torque and hamstring/quadriceps (H/Q) ratio were also measured at 60, 180, 300 degrees/s through a Cybex 2 - 340 d

Extensor apparatus of the knee: anatomy and injury patterns.

The extensor mechanism of the knee is essential for maintaining normal daily function and for activities such as walking and athletic performance. It comprises the quadriceps muscles and tendon, patella and peripatellar fat pads, patellofemoral joint and retinacula, patellar tendon, and tibial tuberosity. Disruption of any component can result in pain, instability, or loss of active extension. Owing to its superficial location and continuous biomechanical loading, it is particularly vulnerable to acute trauma, repetitive microtrauma, chronic degeneration, and traction apophysitis in the pediat…

Soft Tissue Surgical Technique for Obligate Dislocation of the Patella.

BACKGROUND: Obligate dislocation of the patella (ODP) is a subtype of congenital disorders and dislocations of the patella. Severity of the condition varies; symptoms range from minor disabilities to impaired ambulation. Definitive treatment for ODP is surgery, with the choice of surgical procedure determined by skeletal maturity. CASE SERIES: This report documents a soft tissue surgical correction for 2 patients with ODP using (1) lateral release of the retinaculum, (2) Z-lengthening of the quadriceps tendon, (3) patellar tendon split transfer (medial side only), and (4) vastus medialis obli

Evaluation of Reference Values for the Patellotrochlear Index in the North Indian Population: A Cross-sectional Magnetic Resonance Imaging-based Study.

BACKGROUND: The patella plays a crucial role in the extensor mechanism of the knee joint, significantly enhancing the efficiency of the quadriceps muscle. The alignment and height of the patella are critical for maintaining knee joint functionality and preventing knee-related disorders. The Patellotrochlear Index (PTI) is a novel measurement method that provides a more accurate assessment of patellar height and its relationship with the femoral trochlea, particularly in the North Indian population. OBJECTIVE: This study aims to establish normal PTI values in the North Indian population and ev…

Intraoperative Surgeon-Performed versus Conventional Anesthesiologist-Performed Continuous Adductor Canal Block in Total Knee Arthroplasty: A Randomized Controlled Trial.

A continuous adductor canal block (cACB) for pain control in total knee arthroplasty (TKA) is always performed by an anesthesiologist. A surgeon-performed cACB during surgery is somewhat questionable in terms of feasibility, reproducibility, and efficacy. This study was divided into two phases. In Phase 1 study, an experimental dissection of 16 cadaveric knees to expose the saphenous nerve and related muscles around the adductor canal was conducted. The extent of dye after injection via a catheter inserted into the adductor canal at the time of TKA was evaluated. In Phase II, a randomized cont

Relationship between geometry of the extensor mechanism of the knee and risk of anterior cruciate ligament injury.

The complex inter-segmental forces that are developed across an extended knee by body weight and contraction of the quadriceps muscle group transmits an anteriorly directed force on the tibia that strain the anterior cruciate ligament (ACL). We hypothesized that a relationship exists between geometry of the knees extensor mechanism and the risk of sustaining a non-contact ACL injury. Geometry of the extensor mechanism was characterized using MRI scans of the knees of 88 subjects that suffered their first non-contact ACL injury and 88 matched control subjects with normal knees that were on the …

Effects of ligation of lateral intermuscular septum perforating vessels on blood supply to the femur.

INTRODUCTION: With a subvastus approach to the femur, the vessels that perforate the lateral intermuscular septum (LISP-vessels) must be lig … a complex and rich anastomotic network and play an important role in the perfusion of the femur and quadriceps muscle group. The ligature could be compensated for by this anastomotic network. Branches to the periosteum separate from the LISP vessels immediately after perforating the lateral intermuscular septum. The linea aspera turned out to be an important area f …

Virtual reality-augmented neurorehabilitation improves motor function and reduces neuropathic pain in patients with incomplete spinal cord injury.

BACKGROUND: Neurorehabilitation interventions to improve lower limb function and neuropathic pain have had limited success in people with chronic, incomplete spinal cord injury (iSCI). OBJECTIVE: We hypothesized that intense virtual reality (VR)-augmented training of observed and executed leg movements would improve limb function and neuropathic pain. METHODS: Patients used a VR system with a first-person view of virtual lower limbs, controlled via movement sensors fitted to the patient's own shoes. Four tasks were used to deliver intensive training of individual muscles (tibialis anterior,

Possible mechanism of quadriceps femoris weakness in patients with ruptured anterior cruciate ligament.

PURPOSE: The purpose of this study was to test the hypothesis that loss of afferent feedback due to rupture of anterior cruciate ligament (ACL) is the cause of quadriceps femoris (QF) weakness through gamma loop. Two experiments were designed to prove our hypothesis. METHODS: In experiment 1, the maximal voluntary contraction (MVC) of knee extension and integrated electromyogram (I-EMG) of vastus medialis (VM), vastus lateralis (VL), and rectus femoris (RF) were measured in 13 patients with ruptured ACL and 7 healthy volunteers before and after injection of anesthetic agent into the knee.
