Klotho Within Cardio-Renal-Hepatic-Metabolic Health: Integrating Mineral Metabolism, Organ Crosstalk, and Therapeutic Opportunities.

The cardio-renal-hepatic-metabolic (CRHM) axis represents the bidirectional network through which cardiovascular disease, chronic kidney dis … ess, inflammation, and fibrotic remodelling. Collectively, these contribute to cardiac hypertrophy, vascular calcification, hepatic steatosis, insulin resistance, and progressive renal dysfunction. Importantly, Klotho biology is influenced by tissue context, isoform, disease stage, age and sex. However, human evidence remains largely associative and is derived predomina …

Bioactive Nanocomposite Scaffolds by Melt Electrospinning of Poly-ε-Caprolactone/Polyethylene Oxide/Polyethylene Glycol-Hydroxyapatite Blends for Bone Tissue Engineering.

Bone defects that are beyond the body's natural repair capacity remain a significant challenge in bone tissue engineering. In recent years, various synthetic materials have been introduced as potential alternatives for repairing these defects, but most of them have limitations. In particular, calcium phosphate cements (CPCs) possess desirable bioactivity but poor mechanical strength. Polymethyl methacrylate (PMMA) exhibits high structural stability but lacks adequate degradability and bioactivity. These obstacles highlight the need to develop materials that balance structural integrity with bi

Giant Hemangiomatous Adrenal Cyst Mimicking a Hemorrhagic Hepatic Cyst: A Case Report.

INTRODUCTION: Adrenal hemangiomatous cysts are rare. Giant cysts of the right adrenal gland may be difficult to distinguish from hepatic cysts owing to their close anatomical proximity to the liver. Here, we report a case of a hemangiomatous adrenal cyst mimicking a hemorrhagic hepatic cyst. CASE PRESENTATION: A 48-year-old woman presented with a 10-day history of abdominal pain and distention. The laboratory findings revealed marked inflammation and severe anemia. Contrast-enhanced CT demonstrated a 28-cm cystic lesion extending from the right hepatic lobe, with peripheral calcification and

The growth plate: Zonal architecture, plasticity, and endocrine control of linear growth.

The growth plate (physis) is a highly specialized cartilaginous organ that drives longitudinal bone growth and ultimately determines adult stature. Its zonal architecture, including the resting, proliferative, hypertrophic, and calcification zones, integrates stem-like progenitor activity, clonal chondrocyte expansion, matrix remodeling, vascular invasion, and replacement by bone. Here, we review how the structural organization of the growth plate emerges from the interplay among extracellular matrix composition, sulfation pathways, and canonical paracrine signaling pathways, including Ihh-PTH

Pre-Parathyroidectomy PTH as an Integrated Biomarker of Glandular Remodeling and Skeletal Turnover in Secondary Hyperparathyroidism.

Secondary hyperparathyroidism (SHPT) is a major component of chronic kidney disease-mineral and bone disorder (CKD-MBD), reflecting progress … rrence. Severe SHPT is also increasingly recognized as a systemic CKD-MBD phenotype associated with vascular calcification, cardiovascular risk, metabolic instability, and impaired quality of life. Within this framework, preoperative PTH is best interpreted as an integrated biomarker within a broader assessment of glandular remodeling, skeletal metabolic activity, endoc …

Severe calciphylaxis in a post-kidney transplant patient with chronic allograft dysfunction and unmodifiable anticoagulation.

Calciphylaxis is a rare but life-threatening disorder characterised by progressive vascular calcification, thrombosis, and painful skin necrosis, with high morbidity and mortality. Although most cases are reported in patients with end-stage kidney disease receiving dialysis, it can also occur in kidney transplant recipients, particularly in the setting of chronic allograft dysfunction and persistent disturbances in mineral metabolism. We report the case of a 70-year-old man who developed biopsy-confirmed calciphylaxis during chronic allograft failure secondary to chronic T-cell-mediated reject…

PFAS exposure during pregnancy: Implications for placental health and functioning.

BACKGROUND: Animal studies have linked prenatal poly- and perfluoroalkyl substances (PFAS) exposures with impaired placental structure and function. In humans, only few studies have investigated such associations. OBJECTIVE: We studied whether PFAS, individually and as a mixture, affected placental function. METHODS: In 367 pregnant women, we quantified 13 PFAS in serum collected at 19.3 gestational weeks (median). Placental weight was recorded at delivery. Histological examination of placental tissues allowed estimation of vascular perfusion (percentage of villi with syncytial knots, capill

Histological Analysis of Arterial and Venous Grafts Used in Coronary Bypass for Patients With Renal Insufficiency: A Prospective Multicentre Observational Study.

OBJECTIVES: Chronic kidney disease (CKD) is associated with metabolic dysfunctions that accelerate atherosclerosis, posing significant challenges for patients undergoing coronary artery bypass grafting (CABG). In this population, arterial calcification and reduced saphenous vein patency are common complications. This multicentre prospective study aims to evaluate the impact of renal dysfunction on the histological characteristics of arterial and venous grafts used in CABG. METHODS: Vascular graft specimens collected during CABG were prospectively analysed and stratified into 3 groups based on

Effect of sodium thiosulfate on preventing renal ischemia-reperfusion injury in high-fat diet-fed rats: the role of renal mitochondrial quality.

OBJECTIVE: Sodium Thiosulfate (STS), a clinically approved agent for cyanide poisoning and vascular calcification, possesses antioxidant, anti-inflammatory, mitochondrial preservation, and metal chelation capabilities, rendering it a promising candidate for managing ischemia-reperfusion (IR) injury. The detrimental impact of high-fat diets (HD) on the outcomes of IR during renal surgeries is well-documented. However, the potential of STS to ameliorate renal IR injury in rat fed with high fat diet is not known. METHODS: Male Wistar rats were fed a standard diet (SD) or a high-fat diet (HD) for…

Investigating the associations between uncarboxylated matrix gla protein as a proxy for vitamin K status and cardiovascular disease risk factors in a general adult population.

PURPOSE: Vitamin K is an activator of vitamin K dependent proteins, one of which is the potent inhibitor of vascular calcification, matrix Gla protein (MGP). The purpose of this study is to investigate the association between an inverse proxy of functional vitamin K status, plasma dephospho-uncarboxylated MGP (dp-ucMGP), and cardiovascular disease risk factors (CVDRFs). METHODS: In a cross-sectional population-based health examination study of 4,092 individuals aged 24-77 years, the vitamin K status was assessed using plasma dp-ucMGP. All participants were linked to Danish National Prescripti…

Identification of potential bioactive phytochemicals for the inhibition of platelet-derived growth factor receptor β: a structure-based approach for cancer therapy.

Platelet-derived growth factor receptor beta (PDGFRβ) belongs to the receptor tyrosine kinase (RTK) protein family and is implicated in several disorders such as hematopoietic, glial, and soft-tissue cancer, non-cancerous disorders, including skeletal defects, brain calcification, and vascular anomalies. The research on small molecule inhibitors targeting PDGFRβ in cancer treatment has seen promising developments, but significant gaps remain. PDGFRβ, receptor tyrosine kinase, is overexpressed in various cancers and plays an important role in tumor progression, making it a potential therapeutic

Differences in prevalence of reduced and low bone mineral density between lumbar spine and femoral neck in peritoneal dialysis patients using dual-energy X-ray absorptiometry (DXA).

The pattern of chronic kidney disease-mineral bone disease has changed following the increase in elderly patients receiving dialysis, with e … with LS T scores a mean of 0.34 higher than FN. DXA scanning at the LS can be affected by overlying vascular calcification and electron densities in the bowel, and as such we suggest that FN T scores are used to report BMD in PD patients.
