Changes in QRS morphology during antidromic atrioventricular reentrant tachycardia.

We report a case of a 44-year-old male who underwent an electrophysiological study for symptomatic supraventricular tachycardia (SVT) with wide QRS complex. The SVT was diagnosed as an antidromic atrioventricular reentrant tachycardia (AVRT) via antegrade conduction of left-sided accessory pathway (AP). However, the QRS morphology changed during the SVT, and then the SVT was terminated spontaneously. The mapping of AP was performed during sinus rhythm, and the radiofrequency application successfully eliminated the AP, which rendered tachycardias non-inducible. This was a rare case of antidromi

Minimally decremental atriofascicular accessory pathway with bidirectional conduction.

Atriofacicular pathways of Mahaim type are typically decrementally conducting accessory pathways without retrograde conduction properties, located on the right ventricular free wall at the tricuspid annulus. We report a patient with an atriofascicular pathway with minimal anterograde decremental conduction. Both long and short V-H antidromic atriofascicular reentrant tachycardias were induced and mechanism confirmed with electrophysiologic testing. Additionally, orthodromic atriofascicular reentrant tachycardia with narrow and right bundle branch block morphologies were inducible. Mahaim pathw

Irregular orthodromic atrioventricular reentrant tachycardia using the left lateral accessory pathway due to intermittent mitral isthmus block.

Mitral isthmus block during left lateral accessory pathway ablation has been reported in the past. Here, we document for the first time an irregular atrioventricular reciprocating tachycardia due to alternating mitral isthmus block and mitral isthmus delay, resulting in different atrioventricular node conduction times and tachycardia cycle length.

Variable clinical features and ablation of manifest nodofascicular/ventricular pathways.

BACKGROUND: Manifest nodofascicular/ventricular (NFV) pathways are rare. METHODS AND RESULTS: From 2008 to 2013, 4 cases were identified with manifest NFV pathways from 3 centers. The clinical findings and ablation sites are reported. All 4 cases presented with a wide complex tachycardia but with different QRS morphologies. Case 1 showed a left bundle branch block/superior axis, case 2 showed a right bundle branch block/inferior axis, case 3 showed a left bundle branch block/inferior axis, and case 4 showed a narrow QRS tachycardia and a wide complex tachycardia with a left bundle branch bloc

Surgical treatment of supraventricular tachycardia: experience with 61 patients.

Sixty-one patients underwent operation for supraventricular tachycardia: 52 had tachycardia associated with the atrioventricular accessory pathways; 9 patients had other forms of tachycardia. Accessory pathways were successfully divided in 92.3% of the patients. Classic endocardial approach was used in 30 patients; closed heart ablation of the accessory pathways was performed in 22 patients. There were two reoperations for return of conduction through the accessory pathways. Six patients underwent cryoablation of the His bundle through the right atrial approach. Four patients underwent attempt

Catheter ablation of right-sided para-Hisian ventricular arrhythmias using a simple pacing strategy.

BACKGROUND: Ablation of para-Hisian ventricular arrhythmias (PH-VAs) is challenging because of the close relationship of the origin site and His bundle. Using pacing techniques to differentiate the near-field from far-field His activations, thereby avoiding atrioventricular block, has been reported in patients with para-Hisian accessory pathways. OBJECTIVES: We applied the same pacing technique and 3-dimensional mapping to guide radiofrequency (RF) ablation in patients with right-sided PH-VAs and investigated the clinical outcome of such cases. METHODS: Fourteen patients with right-sided PH-

New Onset Palpitations, Chest Pain, Dyspnea, and Diaphoresis in a 65-Year-Old Woman.

A woman with the new onset of cardiac symptoms was in atrial fibrillation with a wide QRS complex that was not typical of bundle branch block. After electrical cardioversion, a short PR interval and delta waves indicated the presence of an accessory atrioventricular pathway. This left lateral atrioventricular bypass tract was subsequently ablated.

The atrioventricular junctions in Ebstein malformation.

OBJECTIVE: To review the anatomical structure of the right atrioventricular junction, including the specialised atrioventricular conduction system, in hearts with Ebstein's malformation, to identify potential substrates for the abnormalities in conduction. METHODS: Five heart specimens representing the morphological spectrum of Ebstein malformation were examined grossly and histologically. RESULTS: On the endocardial surface, the atrioventricular junction was marked by a faint line in two hearts, and by a small ridge in the other three. Analysis of the right parietal junction in four hearts

Early dose of Adenosine, postRadiofrequency abLation of accessory pathwaY in determining acute procedural success (EARLY study).

INTRODUCTION: Post ablation of the accessory pathway (AP), the patient is observed in the catheterization laboratory for a variable period for resumption of pathway conduction. Aim of the study was to determine whether the administration of intravenous adenosine at 10 min after ablation of AP would have the same diagnostic accuracy as waiting for 30 min in predicting the resumption of AP conduction. METHODS: This was a prospective interventional study conducted in two centers. Post ablation of the AP, intravenous adenosine was administered at 10 min to look for dormant pathway conduction. The

A rare case of narrow QRS tachycardia with "hidden" concealed left-sided accessory pathway.

A 54-year-old-woman with narrow QRS tachycardia (heart rate, 210 beats per minute) was admitted to the authors' hospital. Tachycardia terminated after administration of adenosine. The 12-lead electrocardiogram after conversion to sinus rhythm was unremarkable. During electrophysiological study, tachycardia could be easily induced by programmed atrial stimulation without jump phenomenon. The cycle length was 260 ms. During tachycardia, the atrial activation sequence in the coronary sinus (CS) was distal-to-proximal. However, during decremental ventricular pacing, retrograde conduction showed ex

Catheter ablation for recurrent tachyarrhythmias. Clinical experience with two different techniques of ablation in 21 patients.

Between 1984 and 1988, 21 patients underwent catheter ablation for drug refractory arrhythmias. Nine patients presented atrial flutter, atrial fibrillation or atrial tachycardia, nine had supraventricular tachycardia (one AV nodal reentrant tachycardia, one reciprocating tachycardia due to concealed accessory pathway and seven WPW syndrome). Three had ventricular tachycardia. Fourteen patients were treated with direct current shock ablation (DC) and seven patients with radiofrequency ablation (RF). Eight patients underwent ablation of the His bundle. In six patients permanent AV block could be

[Dispersion of junctional tissue and its consequences in a case of sudden infant death].

This article reports a case of sudden infant death syndrome in a 1-month-old male. Some important abnormalities of the cardiac conduction system were found, consisting of persistent fetal dispersion of the atrioventricular node and His bundle, accessory atrioventricular pathways of Mahaim type and resorptive degeneration. These changes could be considered as a morphological substrate for cardiac arrhythmias.

Double ventricular response to an extrastimulus in a patient with triple atrioventricular pathways.

A patient with the Wolff-Parkinson-White Syndrome was studied through recordings of the intracardiac potentials and programmed atrial stimulation. During programmed atrial stimulation at progressively shorter coupling intervals (A1-A2 intervals), the His deflection was always recorded after the ventricular complex. Thus, at coupling intervals between 295 and 250 msec, there was a double ventricular response, one through the accessory pathway (QRS complex of the Wolff-Parkinson-White morphologic pattern) and the other by the normal atrioventricular pathway (normal QRS complex or with pattern of

Ventricular preexcitation following catheter ablation of the His bundle in concealed WPW syndrome.

Catheter ablation of the His bundle is reported in a patient with drug-refractory supraventricular tachycardia meeting standard criteria of concealed WPW syndrome (conduction of the accessory atrioventricular pathway in retrograde direction only). After successful ablation of the His bundle, anterograde conduction along the accessory pathway, to our surprise, became apparent. During twelve months of follow-up, the patient was free of the tachycardia recurrences off antiarrhythmic drugs. While the clinical problem has been solved, the patient may--in case he should develop atrial fibrillation--

Fast pathway ablation using cryotherapy for a patient with a long PR interval in sinus rhythm and AVNRT.

A 53-year-old man with previous aortic valve surgery presented with paroxysmal narrow complex tachycardia, induced by exercise. His PR interval was greater than 400 ms when in sinus rhythm and atrioventricular nodal reentry tachycardia (AVNRT) was diagnosed with invasive electrophysiological studies. Single echoes were repeatedly inducible with single-paced extrastimuli. Cryotherapy was then used to ablate the fast pathway using single echoes to monitor anterograde slow pathway and retrograde fast pathway function during ablation.
