Clinical differences among the elderly admitted to the emergency department for accidental or unexplained falls and syncope.

It is difficult to distinguish unexplained falls (UFs) from accidental falls (AFs) or syncope in older people. This study was designed to compare patients referred to the emergency department (ED) for AFs, UFs or syncope. Data from a longitudinal study on adverse drug events diagnosed at the ED (ANCESTRAL-ED) in older people were analyzed in order to select cases of AF, syncope, or UF. A total of 724 patients (median age: 81.0 [65-105] years, 66.3% female) were consecutively admitted to the ED (403 AF, 210 syncope, and 111 UF). The number of psychotropic drugs was the only significant differen…

Fall-related self-efficacy, not balance and mobility performance, is related to accidental falls in chronic stroke survivors with low bone mineral density.

UNLABELLED: Chronic stroke survivors with low hip bone density are particularly prone to fractures. This study shows that fear of falling is … ated self-efficacy, but not mobility and balance performance, was the most important determinant of accidental falls. This psychological factor should not be overlooked in the prevention of fragility fractures among chronic stroke survivors with low hip BMD.

Acute aortic dissection presenting as case of accidental falling with flaccidity of left lower extremity.

Aortic dissection is often a catastrophic, life-threatening condition that may not always present with symptoms. Without intensive treatment, the mortality within the first 2 weeks of onset is reportedly as high as 80%. Acute aortic dissection classically presents with the symptoms of abrupt chest pain that sometimes radiates to the back, abdominal pain, or back pain alone. The patient may be cool and clammy, which is indicative of a shock syndrome. It can be associated with neurologic sequelae in as many as one third of all patients. Painless dissection occurs in approximately 5% of patients,

Delayed sudden death in an infant following an accidental fall: a case report with review of the literature.

Several controversies exist regarding ultimately lethal head injuries in small children. Death from short falls, timing of head injury, lucid intervals, presence of diffuse axonal injury (DAI), and subdural hematoma (SDH) as marker of DAI are the most recent controversial topics of debate in this evolving field of study. In this area of debate, we present a case of delayed death from a witnessed fall backwards off a bed in a 9-month-old black male child who struck his head on a concrete floor and was independently witnessed as "Работает" postfall for 72 hours until he was discovered dead in bed

Injuries following accidental low height-falls sustained by children under 1 year of age - A systematic review.

BACKGROUND: Low-height falls are a recurrent cause of accidental trauma in young children. They also represent the most commonly reported history provided by perpetrators of abuse. The study of traumatic injuries resulting from low-height falls is essential to provide the forensic expert with the necessary data to better distinguish accidental falls from cases of inflicted violence. Previously published clinical studies are sometimes contradictory, and systematic reviews on this topic remain scarce. OBJECTIVES: To describe the traumatic injuries resulting from accidental falls of less than 15…

Patterns of injury seen in deaths from accidental falls down a staircase: a study from South Delhi.

The present study is a retrospective analysis of 92 cases of death from an accidental fall down a staircase, which were autopsied at the All India Institute of Medical Sciences, New Delhi, India, during a ten-year period from 1996 to 2005. The cases were analysed with regard to age, sex, location, cause of death and pattern of injury. Males (62, 67.39%) outnumbered females (30, 32.61%). The commonest age group involved was 31-40 years (28.26%) followed by 21-30 years (18.47%) and 51-60 years (14.13%). The lowest number of cases (4.34%) was in the 11-20 years age group. All deaths were accident

[Central cervical cord syndrome: a case report on rehabilitation, with special references to accidental falls in the elderly].

An 81-year-old man with Parkinson's disease was admitted to our hospital with impaired function of all extremities. Four weeks before his symptoms developed, he had tripped on the steps, fallen and bruised his jaw. Following this episode he experienced a few more falls inside his house. On examination his greatest weakness was in the hands and wrists. He was hyper-reflexic in all extremities and had bilateral Babinski's sign. He could not walk and needed physical assistance in most of his daily living activities. X-ray films of the cervical spine showed significant degenerative changes. The ma

Comparison of suicidal and accidental fall injury patterns: A 10-year retrospective study in Hong Kong.

BACKGROUND: Falls from a height are a common cause of major trauma in urban areas. This study aims to investigate the differences in patient characteristics and injury patterns between suicidal and accidental falls from a height in Hong Kong. METHOD: From 2015-2024, patients aged ≥ 18 years recorded under the 'fall from height ≥ 2 m' category of the trauma registry in a tertiary hospital in Hong Kong were retrospectively reviewed. Patient characteristics and the nature of the falls were noted. Abbreviated injury scale (AIS) was the primary outcome. Injury severity score (ISS), skeletal fractu…

Accidental death from a fall from height at workplace turned out to be a COVID-19 death.

Coronavirus 2019 disease (COVID-19) has rapidly evolved into a pandemic during the first months of 2020. The general extent of organ involvement, as well as microscopic changes in the organs of the diseased remains insufficiently documented so far. Herein we are reporting the case of a Caucasian male who apparently died in a workplace allegedly by a fall from height and was found to be SARS-CoV-2 positive just prior to the autopsy. The circumstances of his death required a medicolegal investigation. Gross autopsy findings included minor external injuries and focal subarachnoid hemorrhage toget

Intracranial impalement with entrance site in the mandibular region: postmortem elucidation of an accidental fall on a wooden plant stick.

A 78-year-old woman with a history of transient ischemic attacks was found in the doorway of her house in a somnolent and unresponsive condition. In the right mandibular region, a small skin wound was localized, which was surgically treated. Six days after admission to the hospital, an exploratory craniotomy was performed because of abnormal CT findings. Apart from tissue lesions and hemorrhages a small bone fragment was detected in the right cerebral cortex, which was removed. After 11 days in hospital, the patient died from failure of central regulatory functions. At the forensic autopsy, a

Impact of accidental falls on 1-year prognosis or rehospitalization in elderly hospitalized patients with heart failure undergoing cardiac rehabilitation.

AIMS: Accidental falls (AFs) occasionally occur during hospitalization in older patients with heart failure (HF); however, limited evidence exists regarding their clinical significance. We aimed to evaluate the impact of AFs on prognosis, one-year HF readmission, and associated risk factors in hospitalized patients with HF. METHODS AND RESULTS: This single-centre, retrospective, observational study included 462 patients (mean age: 83.0 years; 47% female) hospitalized with acute decompensated HF and followed for one year post-discharge. Among patients who experienced AFs (n = 32; median days t…

Fatal facial-intracranial impalement injury in an accidental fall from a height: an autopsy case report with a review of the literature.

Impalement injury to the face and head is of clinical and forensic pathological significance because of its diagnostic pitfalls. This injury often penetrates the orbital, nasal or oral cavity into the skull, but impaling other sites of facial bone is rare. The present case was a rare type of facial-intracranial impalement injury with a direct cerebral arterial laceration. The victim was a 44-year-old man who fell from a height of about 2.5m onto a broken chair and died 6 days later despite surgical treatment. The clinical diagnosis was a 'small abrasion' of the face and 'traumatic intracerebra

Differences in Trauma Injury Patterns and Severity Between Intentional and Accidental Falls From a Height: A Japanese Nationwide Trauma Database Study.

Background Fall from a height is a common cause of trauma requiring emergency care; in many cases, the trauma team needs to urgently develop the initial treatment strategy. The mechanism of injury (intentional or accidental) is an important factor in predicting trauma patterns and severity. We aimed to describe how the severity of injuries in each body region contributes to overall trauma severity and skeletal trauma patterns in intentional and accidental falls. Methods Data accumulated between January 1, 2004 and May 31, 2019 were obtained from a nationwide trauma database. Patients aged ≥18 …

Effectiveness of an Early Intervention in Mild Hyponatremia to Prevent Accidental Falls in Hospitalized Older Adults-A Crossover Ecological Clinical Trial.

Background: Falls in hospitalized patients cause injuries of varying severity and even death. There is a link between falls and low blood sodium levels in older patients. Identifying and treating hyponatremia could help prevent falls and reduce hospital stays. The purpose of this study was to evaluate the effectiveness of the correction of hyponatremia on reducing the incidence of falls and the mean stay of hospitalized patients aged more than 65 years. Methods: A crossover ecological clinical trial was conducted in adult hospitalization units of a hospital in Madrid (Spain) over 12 months. Pa
